백내장 수술후에 발생하는 각막난시의 변화에 대한 임상적 고찰

Abstract

백내장수술후 인공수정체를 삽입한 눈에있어서 각막난시의 발생은 시력을 저하시키는 중요한 원인이 된다. 저자는 백내장으로 인하여 수정체 낭외적출술과 후방인공수정체 삽입술을 시행받은 눈에서, 각막난시의 발생 및 시간경과에 따른 각막난시의 변화, 각막난시의 안정화 시기를 알아보기 위하여 본 조사를 시행하였다. 1990년 1월 부터 1990년 9월 까지 이화여자대학병원 안과에서 백내장으로 진단받고, 수정체 낭외적출술 및 후방인공수정체 삽입술을 시행받은 환자중, 20주 동안 추적조사가 가능했던 60명(60안)을 대상으로, 각막만곡계(Cabnnon사)를 사용하여 수술전과 수술후의 각막만곡도를 측정, 다음과 같은 결과를 얻었다. 1. 수술전 평균 각막난시는 0.19±0.09diopter로 직난시였다 2. 수술후 1일째 초기각막난시는 평균 5.70±1.03diopter였고, 시간이 지남에 따라 점차 난시도가 감소하여 수술후 4주에 평균 각막난시는 2.48±0.08diopter였으며, 수술후 20주에 최종 각막난시는 평균 0.42±0.9diopter였다 3. 수술후 발생한 각막난시의 축은 모두 직난시였고, 시간 경과에 따른 큰 변화는 없었다. 4. 수술후 1일째 초기각막난시가 평균 5.7diopter보다 큰 군과, 평균 5.7diopter보다 작은 군에서의 최종각막난시는 각각 0.59±0.32diopter, 0.24±0.09diopter였으며, 초기각막난시의 정도와 최종각막난시의 정도와는 상관관계가 없었다(r=0.096, p>0.05) 5. 수술후 굴절력이 안정되는 평균 시기는 13.72±4.32주였다. 6. 수술후 1일째 츠기각막난시가 평균 5.70diopter보다 작은 군과, 평균 5.70diopter 보다 큰 군에서의 굴절력의 안정시기는 각각 16.02±3.14주와 10.08±2.06주였으며, 초기각막난시의 정도와 굴절력의 안정화 시기 사이에는 유의한 상관관계가 있었다(r=0.712, p<.05) 7. 본 조사 결과에서 굴절력의 안정 시기가 평균 13.72±4.32주이며, 초기각막난시가 심한 경우 더 늦게 굴절력의 안정을 보인 것으로 보아 백내장수술후 최소한 16주 이상 각막난시를 관찰해야 하고 최종적인 안경처방은 16주 이상 경과를 관찰한후 시행한 굴절검사로 해야하겠다.;This study was undertaken to evaluate the course of corneal astigmatism following extracapsular cataract extraction with posterior chamber intraocular lens implantation. Among the patients who underwent extracapsular cataract extraction with posterior chamber intraocular lens implantation at Ewha Woman's University Hospital from January 1990 to September 1990, 60patients (60eyes) were included in followup period over 20weeks. The corneal curvatures were examined by keratometer (Cannon^(R)) preoperatively and postoperatively. Results were as follows : 1. The preoperative mean corneal astigmatism was 0.19±0.09diopters, with-the-rule. 2. The mean initial corneal astigmatism on postoperative 1day was 5.70±1.03 diopters, the mean corneal astigmatism on postoperative 4weeks was 2.48±0.08 diopters. The mean final corneal astigmatism on postoperative 20weeks was 0.42±0.19 diopters. 3. The axis of corneal astigmatism after operation were all with-the-rule. There was no significant change during passage of fime. 4. There was no significant relationship between the amount of initial corneal astigmatism and the amount of final corneal astigmatism after 1the operation. 5. The mean time of stabilization after operation was 13.72±4.32weeks. 6. The lesser amount of initial corneal astigmatism on post operative 1day, the faster time of stabilization of corneal astrgmatism were shown. 7. The mean time of stabilization after operation was 13.72±4.32weeks and due to the fact that mean time of stabilization was delayed in greater initial corneal astigmatism, it is necessary to observe corneal astigmatism at least for 12weeks after operation and the final glasses prescription should be done with refraction after 16weeks of observation.
