구강냉동요법의 항암치료 유발성 점막염에 대한 예방효과

Abstract

종양환자의 항암치료에 있어서 흔히 발생하는 구강점막염은 오심, 구토증세와 함께 환자가 가장 고통스러워하는 부작용증의 하나로써 통증이 매우 심하며, 환자가 정상적인 식생활을 유지하는데 큰 저해요소가 될 뿐 아니라 항암제 사용을 기피하는 요인으로 작용하기도 한다. 이를 예방하려는 연구가 이루어져 왔으나 현재까지 항암제에 의해 유발된 구내염을 효과적으로 예방하는 방법은 없다. 구강냉동요법(oral cryotheraphy)은 5-Fluorouracil, methotrexate등 상대적으로 혈장내 반감기가 짧은 약물의 투여 동안에 국소적으로 혈관수축을 일으켜 구강점막으로 가는 혈류를 감소 시킴으로써 약제의 독성을 줄이는 것이다. 구강냉동용법이 항암제가 최고혈장농도에 이르는 동안 계속 실시될 수 있다면 구강내 점막이 항암제에 노출되는 정도가 감소되고 그만큼 점막염도 줄어들 수 있다. 저자는 15명의 항암제치료를 받은 종양환자에서 구강냉동요법을 병용한 항암치료와 병용없이 항암치료를 각각 실시한 후 점막염의 빈도 및 그 정도를 비교분석하여 다음과 같은 결론을 얻었다. 1. 항암치료유발성 점막염빈도는 구강냉동요법을 병용한 경우 13%, 구강냉동요법을 병용하지 않은 경우 27%에서 발생하였다. 구강냉동요법치료는 항암치료유발성 점막염의 빈도를 감소시켰으나 유의성은 없었다(P＞0.05). 2. 구강냉동요법치료는 항암제 투여환자들 15명중 3명에서 점막염의 정도를 감소시켰으나(grade 1에서 grade 0으로) 유의성은 없었다(P＞0.05). 이상으로 구강냉동요법은 유의성은 없었으나 항암치료유발성 점막염의 빈도와 그 정도를 감소시킬 수는 있었으며 향후 증례수를 늘리고 각 항암제에 따른 효과를 비교하는 더 많은 연구가 요구된다.;Mucositis is a significant dose-limiting toxicity associated with anti cancer chemotherapy. This study has carried out under the hypothesis that oral cryotherapy would cause local vasoconstriction and temporally decrease blood flow to the oral mucous membrane. Thus we could expect that as the oral mucous membrans would have less exposure to anti cancer drug, mucositis would less develop. The author studied the effect of oral cryotherapy during their chemotherapy cycles. The 15 cancer patients received oral cryotherapy during their first cycle of chemotherapy and during the following cycle they ceased to receive cryotherapy. The result suggested that the incidence of mucositis was 13% with oral cryotherapy and 27% without oral cryotherapy (P>0.05) and the severity of mucositis was also decreased with oral cryotherapy. Even though the author didn't get the results of significant decrease of incidence and severity of chemotherapy incuced mucoitis with cryotherapy, oral cryotherapy can reduce the incidence and severity of the anti cancer chemotherapy induced mucositis. Thus oral cryotherapy can be routinely recommendable for patient receiving cancer chemotherapy. However further trial with large sample should be necessary and comparison of dose and type of chemotherapeutic agent with oral cryotherapy should be tried.
