부신피질호르몬 국소도포가 Guinea Pig 표피의 Langerhans 세포에 미치는 영향에 관한 연구

Abstract

Langerhans cells are immunocompetent, dendritic cells of bone maarow derivation that reside within the epidermis. Langerhans cells have been implicated as playing a key role in the processing and presentation of antigens to lymphocyte in contact allergic cell-mediated immune reactions. The present study was undertaken in order to clarify the effect of topical glucocorticosteroids on the number and the morphological alterations of epidermal Langerhans cells. Three kinds of topical glucocorticosteroids with different potency were applied to the back of guinea pig twice a day for 14 days. Skin specimens were taken 2nd, 7th, 14th day of the application and 7th day after cessation of application. The ATPase stain and electron microscopic observations were performed to assess the numbers and morphological alterations of epidermal Langerhans cells. The following results were obtained. 1. The number of epidermal Langerhans cells significantly decreased after 7 days applications of glucocorticosteroids in all experimental groups compared with control (P＜0.05). 2. In each experimental group, the decrease of the number of epidermal Langerhans cells was more significant in 0.5% concentration than those in 0.1% concentrations (P＜0.05). 3. The difference of effects on Langerhans cell density between corticosteroids was significant (P＜0.05). The most potent was DFC which was followed by TAC & HC in decreasing order. 4. The morphological changes of epidermal Langerhans cells were prominent from 7th day of the application of glucocorticosteroids. Many Langerhans cells showed shortness of dendritic processes, decreased staining properties, enlargement of nuclear chromatin, swelling of mitochondria, dilatation of perinuclear cisternae and on endoplasmic reticulum. In summary, decrements in Langerhans cell density were correlated with the concentration and potency of the topical glucocorticosteroids.;Langerhans 세포는 골수에서 기원하며 주로 표피에서 발견되는 수지상 세포이다. Langerhans 세포는 접촉과민 반응의 유발, 피부이식편 거부반응 및 표피세포-임파구 반응등에서 중요한 역할을 한다. 연구자는 부신피질호르몬제 국소도포가 표피 Langerhans 세포의 수와 형태에 미치는 영향을 알아보고자 백색 guinea pig의 배부에 3종의 역가가 다른 부신피질호르몬제를 각각 0.1 %, 0.5 % 농도로 하여 1일 2회 14일간 국소도포하였다. 도포기간에 따라 표피 L-angerhans 세포의 변화를 관찰하기 위하여 도포 2일, 7일, 14일, 도포 중단 7일 후에 피부를 생검하고 ATPase 염색 및 전자현미경검사를 시행하여 다음과 같은 결과를 얻었다. 1. 모든 실험 약물 도포군은 대조군에 비해 도포기간 14일의 전 기간에서 표피 Langerhans 세포 수의 유의한 감소를 보였으며 (P〈0.05 ), 도포 중단 7일후에는 정상수준으로 회복되었다. 2. 모든 실험약물 도포군은. 0.5 %농도에서 0.1 %농도에 비하여 도포 7일과 14일에 유의한 표피 Langerhans 세포 수의 감소를 보였다 ( P 〈 0.05 ). 3. 0.1 %와 0.5%의 동일 농도에서 도포 7일과 14일에 diflucortolone valerate, triamcinolone acetonide, hydrocortisone acetate순으로 유의적인 감소를 보였다.(P〈0.05). 4. 도포 7일째부터 표피 Langerhans 세포의 수지상 돌기가 짧아지고 염색성이 떨어졌으며 전자현미경적으로는 이염색질의 증가, 미토콘드리아의 종창, 핵주위조 및 내형질세망의 확장등이 관찰되었다. 이상의 연구결과로서 부신피질호르몬 국소도포제의 농도와 역가가 높을수록 표피 Laugerhans 세포 수의 감소현상과 형태적 변화가 현저함을 알 수 있었다.
