정신보건센터의 정신보건사회복지사와 정신장애인이 인식하는 정신장애인 성에 관한 연구

Abstract

정신장애인도 한 사람으로서 자신의 행복을 추구하며 성적권리를 누릴 수 있다는 것은 모두 인정하지만, 아직까지 정신보건서비스의 한 영역으로 성에 대해 다루기 어려운 부분으로 남아있는 것 역시 현실이다. 이에 본 연구에서는 정신보건사회복지사가 인식하는 정신장애인 성과 정신장애인이 인식하는 정신장애인 성에 대해 살펴보고, 정신장애인 성생활 및 성지식 실태를 알아보았다. 이를 통해 추후 정신보건서비스 영역에서 정신장애인의 성에 대해 어떠한 방향으로 접근해야 할 것인지에 대하여 올바른 태도와 역할들을 이해하는데 도움을 주고자 하였다. 본 연구의 대상은 전국의 모델형 정신보건센터 42개에 종사하는 정신보건사회복지사 110명과, 이용하고 있는 정신장애인 200명을 대상으로 설문조사를 실시하였다. 정신보건사회복지사는 102부의 설문지가 회수되어 92%의 회수율을 보였고, 정신장애인은 199부의 설문지가 회수되어 99%의 회수율을 보였으며 응답이 불성실한 1개의 설문지는 제외하고 최종적으로 198부의 설문지를 분석 자료로 활용하였다. 본 연구의 결과는 다음과 같다. 첫째, 정신장애인 성의식에 관하여 정신장애인이 정신보건사회복지사보다 성의식, 성역할, 동성애 등에서 더 보수적이며, 전통적인 생각을 가지고 있는 것으로 나타났다. 그러나 성과 성욕, 자위행위 등에 대해서는 정신보건사회복지사와 정신장애인 모두 자연스러운 것으로 받아들이는 것으로 나타났고, 성매매 역시 두 집단 모두 부정적으로 인식하고 있다. 반면 피임에 대해서는 정신보건사회복지사가 정신장애인 보다 매우 중요한 요소로 인식하는 것으로 나타났으며, 이는 정신보건사회복지사가 정신장애인의 성교육이나 성상담을 할 때 더욱 집중하여 접근하게 되는 부분이 될 수 있다. 둘째, 정신장애인 성교육 영역에 있어 정신보건사회복지사와 정신장애인의 영역별 성교육 우선순위에 차이가 있다. 두 집단이 인식하는 성교육이 필요한 우선순위는 임신과 피임, 성가치관과 윤리이나 그 이후에 성교육 필요성을 느끼는 우선순위는 다르다. 정신보건사회복지사는 정신장애인에게 표면적으로 나타날 수 있는 부분에 대하여 성교육이 필요하다고 인식하고 있다. 반면, 정신장애인은 남녀의 성역할, 남녀관계, 신체적·심리적 발달 등의 우선순위에서 볼 수 있듯이 좀 더 인간 내면에 있는 문제나 관계상에서의 성교육을 우선순위로 보고 있다. 셋째, 정신장애인의 성생활에서 정신장애인에게 이성교제가 활발하게 이뤄지지 않음을 알 수 있다. 또한 이성교제 시 상대방과의 친밀감을 느낄 수 있는 활동이 활발하게 이뤄지지 않으며, 상대방의 애정에 기초한 성관계가 이뤄지기 보다 가벼운 관계에서 이뤄진다. 정신장애인의 성지식은 연령 및 진단에 따라 통계적으로 유의미하지 않으며, 전반적으로 성지식이 높지 않게 나타났다. 현재 기관에서 받은 성교육이 정신장애인의 성지식을 획득하는 데에는 도움이 되지 못했다는 결과가 나왔으며, 보다 효과적인 성교육이 이뤄져야 함을 나타낸다. 본 연구 결과를 바탕으로 정신보건사회복지사가 갖추어야 할 정신장애인 성문제에 대한 개입방안 및 방향에 대한 제언은 다음과 같다. 첫째, 정신보건사회복지사는 정신장애인이 신체적으로 성인이나 성장과정에서 겪어야 하는 성적 고민이나 가치들에 대한 충분히 고민이 부족했음을 이해하고, 정신장애인이 자신의 성적 가치를 정립할 수 있도록 성교육과 성상담을 진행해야 한다. 둘째, 정신장애인의 성생활에 있어 피임과 임신 등 구체적인 지식과 대처방법 등을 교육시켜야 한다. 셋째, 정신장애인에게 의미 있는 인간관계를 맺을 수 있는 기본적인 기술과 사회기술과 더불어, 구체적으로 데이트 상대와의 친밀한 관계를 맺을 수 있는 데이트 기술에 대한 교육이 있어야 한다. 넷째, 정신장애인의 성지식에 대한 교육이 체계적으로 이루어져야 한다. 다섯째, 정신장애인이 긍정적인 분위기에서 이성을 만날 수 있는 분위기를 조성하는 것도 필요하다. 또한 후속연구를 위한 실천적 제언은 다음과 같다. 첫째, 정신장애인의 성에 대해 측정할 수 있는 신뢰도 있는 척도 개발이 필요하다. 둘째, 정신장애인의 생애주기별 발단단계에 따른 욕구에 맞춘 구체적이고 체계적인 성교육 매뉴얼 개발이 필요하다. 셋째, 정신장애인 성을 다룰 수 있는 전문가용 성교육 교재 개발도 필요하다. 넷째, 정신장애인 가족이 인식하는 정신장애인 성에 관한 연구도 필요하다.;We admit that the mentally disabled persons are able to have sexual rights by seeking after one’s happiness as human-being. However, currently difficulties are still left to handle sex as a part of mental health service. Therefore, the research shows how mental health social worker recognize the sex of the mentally disabled persons and how the mentally disabled persons recognize the sex and finally current status of their sexual life and knowledge of sex. It is willing to give you help to understand proper duties and attitudes about how to approach to sex of the mentally disabled persons within the field of mental health services. This research is done with 110 mental health social workers who works in 42 model community mental health center, nationwide and 200 mentally disabled persons who visit the center. The results are as the following. Firstly, regarding the thinking of the mentally disabled persons sex, the mentally disabled persons are more conservative than the mental health social workers about the thing of sex, sexual duties and homosexuality. They have traditional thoughts. However, both groups accept sex, sexual desire, and masturbation naturally. And they see sex trade negatively. The other side, the mental health social workers think contraception more important than the mental disabled do. It explains that the mental health social workers emphasize this when they educate and consult the sex of the mentally disabled persons. Secondly, regarding sex education, both have difference in the order of priority. The first and second priorities are pregnancy & contraception and sexual morals & value for both groups. However, after that, both have different perception for necessities of sex education. The mental health service workers regard the external thing as important in sex education. The other side, the mentally disabled persons choose sexual duties of men and women, relationship, and physical and mental development. They have priority on more internal and relationship in sex education. Thirdly, we can find out that dating is not popular among the mentally disabled persons. Also, during date, intimacy is lack. Sexual relationship is not based on love. The sex knowledge of the mentally disabled persons is statistically significant by age and diagnosis, and mostly sex knowledge is no good enough. Sex education from current institute doesn’t help a log for the mentally disabled persons to acquire knowledge of sex. Thus, it is necessary to have more effective sex education. One the basis of this research, the followings are suggestions which the mental health social workers should have regarding plan and directions of sex issue of the mentally disabled persons. First, the mental health social workers should understand that they are lack of care on sexual worry and value which the mentally disabled persons experience. In addition, the mental health social workers should educate and consult the mentally disabled persons in order to make sexual value by themselves. Second, the mental health social workers should educate the mental disabled about contraception and pregnancy with detailed knowledge and handling methods. Third, the mental health social workers should inform basic techniques such as getting meaningful relationship as well as social techniques. Together with them, intimate techniques should be educated in details. Fourth, systematic sex education is needed about the sex knowledge of the mentally disabled persons. Fifth, create a sound atmosphere is needed which the mentally disabled persons can meet the other sex. Also, the below are suggested for further research. First, standard development is necessary to check sex of the mentally disabled persons. Second, sex education manual is needed which fit the development level of life cycle – the mentally disabled persons. Third, sex education materials are necessary to handle sex of the mentally disabled persons. Fourth, research on sex of the mentally disabled persons is needed which the recognition of their family.
