가족치료에 있어서 Bowen의 이론과 그 적용에 관한 연구

Abstract

가족치료는 세계 제2차대전 이후인 1940년대 후반에서부터 1950년대에 걸쳐 전쟁으로 인한 가족붕괴 현상에서 유래되는 가족에 관한 잇슈의 대두와 사회제도 자체가 인간문제를 약화시키는 요인임을 자각하는 현상과 더불어 사회사업계는 물론 집단치료의 새로운 한 형태로 개인치료와 집단치료의 중간위치로 인간관계에 관여하는 모든 전문분야에 대두되게 되었다. 미국의 유명한 가족치료자인 Dr. David O1son은 가족치료의 개념을 치료자가 가족을 하나의 전체적 체계로 간주하고 치료의 대상으로 여기고 실시하는 모든 형태의 치료를 일컷는다고 정의하였다. 이러한 가족치료는 1953년 Bell이 가족집단치료( Family group therapy )라는 방법으로 전가족을 함께 면접한 것이 이정표가 되었으며 1960년대 이후 Palo Alto의 정신연구소( M. R. 1 ) 와 New York의 가족연구소 ( F. R. I )가 공동으로 Family Process라는 전문지를 창간함으로써 인간의 정서문제 치료 뿐만 아니라 가족이 지니고 있는 심리적 기능에 대한 이론 및 실제의 보급이 적극적으로 이루어지고 인정받기 시작했다. Bowen은 멘닝거 진료소 ( Menninger Clinic )에서 소아정신분열증 환자의 치료를 위해 환자와 그의 모 또는 부모를 1∼2개월 간 공동으로 생활하게 함므로써 환자와 가족의 유기적 관계를 조사연구를 통해서 발견하여, 독창적인 가족치료 이론의 개념을 제시하였다. Bowen이 제시한 중요한 개념 중에는 정서적 역기능에 대한 개념과 삼각관계론등이 있고 특히 삼각관계론은 그의 가족치료 이론의 기본개념이 되고있다. 본 연구자는 Bowen의 가족치료 이론을 중심으로 소아정신분열증 가계의 가족내 삼각관계내에 치료자를 끌어들임으로써 부모와 치료자를 삼각관계화 시키는 것을 이용함으로써 가족치료 목표에 도달할 수 있게 하였다. 따라서 본 연구자는 이를 위해 우선 이론적 준거가 되는 Bowen의 이론을 살펴본 다음 서울 K정신병원에서 2개의 사례를 적용시켜 보았다. 본 연구자는 2개의 사례연구 결과 정신분열증 환자의 가족치료에 Bowen의 삼각관계론이 상당히 중요하게 적용이 된다는 것을 발견했다. 또한 본 연구자는 앞으로 이 방면에 좀더 많은 사례연구가 필요할 것이라는 결론을 얻게 되었다.;A post-world War Ⅱ phenomenon, family therapy began in the late 1940 and early 1950s when the divorce rate was vising and there was increasing awareness that social systems were a prime source of human ills. In this thesis, it is pointed out family therapy, especially Bowen's theory. The Bowen's theory involves two main variables. One is the degree of anxienty, and the other is the degree of integration of self. There are several Variables having to do with anxiety or emotional tension. Among these are intensity, duration and different kinds of anxiety. There are for more variables that have to do with the level of integration of the differentiation of self. This is the principal subject of Bowen's theory. And, the centural concept in Bowen's theory is the undifferentiated family ego mass. The notion of the undifferentiated family ego ass which Bowen initially observed in the 1950s has been refined over past 20 years. Some suggest that his entire system is made up six interlocking concepts. But now is eight differentiation of self-scale. Nuclear family emotional system. Family projection process. Multigenerational fransmission process, sibling position. Triangulation, Emotional cut off, Societal regression. Sociental regression and emotional cut off. -These concept. Were added to the theory in 1972, 1975. The concepts of triangulation is very important concepts. It is the basic building block or "molecule" of an emotional system. Detriangulation - the process of differentiation involes a detrianguling of the triangle - is the goal of therapeutic system. The basic effort of this therapeutic system is to help individual family menbers toward a higher level of differentiation of self. In the modern industrial society. Especially in the case of Korean, which has the rapid change to nuclear family. Bowen's model is effective. From this point of view, it is very effective to study Bowen's Model(therapeutic Model) for schizophrenia's family therapeutic process.
