탁솔은 vinca alkaloid계 반합성 항암물질로 폐, 유방, 난소 종양에 단일요법 또는 병합요법으로 광범위하게 투여되고 있으며 강력한 항암작용을 가지고 있어 다른 화학요법에 실패한 경우에도 우선적으로 투여된다. 탁솔은 고용량 투여 또는 저용량의 반복투여에 의해 심각한 말초신경병증을 유발하는데 많게는 탁솔을 투여받은 환자의 90%까지도 말초신경병증이 초래된다. 탁솔로 야기되는 말초신경병증은 대부분 감각신경을 침범하여 사지의 저린감과 감각저하를 초래하는데 이의 기전에 대해서는 아직 정확하게 알려져 있지 않다. 때때로 이러한 부작용으로 인해 항암제 투여를 중단하게 되기 때문에, 항암제 투여 후 나타나는 말초신경병증을 예방 또는 치료하기 위해서는 다양한 약제의 개발과 투여시도가 필요하며 이에 선행되어야 할 것은 적절한 실험동물모델의 설정이다. 이번 실험의 목적은 탁솔을 흰쥐에게 투여했을 때 사람에서와 같은 말초신경병증이 유발되는지를 알아보고 만일 말초신경병증이 나타난다면 그 형태는 어떠한지를 신경전도검사를 통해 관찰하고자 하는 것이다. 본 실험결과 흰쥐에서 탁솔로 유도된 말초신경병증은 인체에서 보고된 바와 마찬가지로 신경전도검사에서 축삭성 병변과 탈수초성 병변이 동반된 소견을 보였으며 운동신경과 감각신경을 모두 침범하였다. 또한 탁솔 동물실험모델의 난제였던 탁솔의 치명적인 독성은 전처치 없이도 탁솔을 5배 희석 투여함으로써 막을 수 있어서 본 실험방법이 탁솔로 유도한 말초신경병증의 간단하고 효과적인 동물실험모델이 될 것으로 생각된다. ; Taxol (Paclitaxel), a widely used antineoplastic agent, is known to produce a dose-related, predominantly sensory neuropathy in human. The incidence of taxol-induced neuropathy is reported up to 90% in clinical situation, but so far no effective treatment is available. To assess effective treatment of taxol neuropathy, an appropriate experimental animal model as well as electrophysiological characterization of taxol-induced neuropathy is essential. Sixty male Sprague-Dawley rats were injected with taxol (6-12mg/Kg) twice intravenously at a week interval. The survivals underwent limb(tibial nerve) and tail nerve conduction studies at 2 weeks interval for 12 weeks. Motor and sensory nerve conduction velocities and action potential amplitudes were compared with those of 10 control rats. Eight (13%) among the taxol-injected rats were survived. Reduction in sensory conduction velocities and sensory nerve action potential amplitudes were noted in tibial nerves, and similar changes were noted in both motor conduction velocities and compound muscle action potential amplitude. The tail nerve conduction studies failed to demonstrate reduced sensory or motor conduction velocities. The time onset of these electrophysiological changes were about 2 weeks after taxol injection, and recovery was not noted until 12 weeks. The nerve conduction study results of this rat model showed electrophysiological findings similar to the taxol neuropathy in human. Moreover, through this experimental study, we have found that 5 times dilution of taxol preparation solution before intravenous injection, almost got rid of the lethal side effect of taxol treatment. Consequently, this animal model is expected to provide an effective way to evaluate the experimental taxol induced neuropathy.
