정신장애인의 성의식, 성생활, 성지식에 대한 연구

Abstract

성에 대한 논의가 좀더 다양하고 폭넓게 진행되고 있지만 아직 우리사회는 정신장애인의 성문제에 관해서는 연구와 개입이 거의 이루어지지 않고 있다. 정신건강전문가들은 정신장애인들이 건강하고 정상적인 성생활을 누리지 못하고 있다는 현실을 충분히 인식하고 있지만 그들의 건강한 성을 재활의 영역으로서 관심을 갖고 개입하는 것에 대해서는 매우 소극적이다. 따라서 이번 연구는 이미 미국 등 서구에서 진행된 정신장애인의 성에 대한 선행연구를 통해 정신장애인의 성과 관련된 여러 가지 성문제를 이해하고 현재 진행되고 있는 성에 대한 개입방법들을 소개하고자 하였다. 또한 지역사회에서 통원치료 및 재활서비스를 받고 있는 한국 정신장애인을 대상으로 한 설문조사를 통해 그들의 성의식과 성생활의 특성, 성지식의 수준을 확임 함으로써 정신장애인의 성을 올바르게 이해하는 기초자료를 제공하고자 하였다. 이와함께 연구 결과를 바탕으로 앞으로 정신건강전문가들이 정신장애인의 성문제에 대하여 어떠한 방향으로 접근해 나가야 할 것인지에 대한 방향성을 제시하고자 하였다. 이번 연구를 위해 15개의 사회복귀시설과 정신보건센터에서 지역사회재활프로그램에 참여하는 정신장애인 162명이 설문조사에 참여하였다. 본 연구에서 조사된 정신장애인의 성의식과 성생활, 성지식의 특성은 다음과 같다 첫째, 정신장애인의 성의식은 순결의식이 다소 강하면서, 인간의 성과 성욕에 대해서는 자연스럽고 당연한 것으로 받아들이는 것으로 나타났다. 또한 정신장애인의 성에 대해서도 편견 없이 자연스러운 것으로 생각하는 것으로 나타났다. 피임의 필요성과 중요성에 대해서도 높게 인식하고 있으며, 자위행위에 대해서도 어느 정도 자연스러운 성행위로 받아들이는 것으로 나타났다. 여성에 대한 성편견은 여성의 성욕과 순결에 대해서만 다소 높은 것으로 나타났다. 윤락행위에 대해서는 허용적이지 않은 것으로 나타났다. 둘째, 정신장애인의 성생활은 대부분 이성과의 관계를 통해서라기 보다는 자위행위 중심이거나 거의 없는 것으로 나타났다. 남성의 경우 절반에 가까이 성기능장애를 경험하고 있었고 여성의 경우 성기능 장애를 더 흔하게 경험하고 있는 것으로 나타났다. 정신장애인은 현재의 성생활에 대해 만족하지 못하는 경향이 높은 것으로 나타났으며, 앞으로 활발한 성생활에 대한 기대감이 있는 것으로 나타났다. 여성의 경우, 활발한 성생활이나 이성에 대한 성적 욕구가 그다지 없는 반면, 남성의 경우 여성과 성생활에 대한 기대감과 욕구가 높은 것으로 나타났다. 이성을 대하는 사회기술은 부족한 것으로 나타났으며, 성문제에 대해서도 전문가와 거의 상담을 하지 않는 것으로 나타났다. 또한 성정보와 음란물에 대한 접근성도 매우 낮은 것으로 나타났다. 셋째, 정신장애인의 성지식 수준은 높지 않은 것으로 나타났다. 과거에 성교육을 받아본 경험이 있더라도 성교육을 받지 않은 사람과 성지식 수준은 차이가 나지 않았다. 현재 활동하고 있는 기관에서 진행한 성교육을 받은 정신장애인과 그렇지 않은 정신장애인간의 성지식 수준도 차이가 나지 않아 지금까지 진행되었던 성교육이 성지식을 높이는데 효과적이지 않음을 시사하였다. 이번 연구의 결과를 바탕으로 한 사회사업 실천에 대한 제언은 다음과 같다. 첫째, 정신건강전문가들은 인간의 성에 대한 이해를 바탕으로 정신장애인의 성문제에 대하여 정확한 이해와 지식을 갖추어야 한다. 둘째, 성상담 및 교육에 필요한 지식 및 기술을 바탕으로 정신장애인의 성문제에 대한 적극적인 개입이 이루어져야 한다. 특히 성에 대해 좀더 취약한 대상이라고 할 수 있는 여성정신장애인에 대한 적극적인 개입이 요구된다. 셋째, 성생활과 관련된 정신장애인들의 사생활 보장에 노력해야 한다. 넷째, 성과 관련된 이슈에 대해 전문가간, 그리고 전문가-정신장애인들 간에 개방적이고 자연스러운 의사소통이 이루어져야 한다. 정신장애인 성과 관련된 후속 연구를 위한 제언은 다음과 같다. 첫째, 정신장애인들이 성생활과 관련되어서 어떠한 욕구들이 있는지에 대한 조사가 필요하다. 둘째, 성문제에 대한 개입방법으로서 정신장애인에게 적합한 성교육프로그램의 개발과 그 효과성에 대한 연구가 필요할 것이다. 셋째 , 여성정신장애인의 성에 대한 연구가 필요하다. 네째, 정신건강전문가들의 성태도, 성의식, 성지식 수준과 정신장애인의 성문제를 바라보는 시각에 대한 연구가 필요하다. 마지막으로, 정신장애인의 성을 측정, 연구할 수 있는 여러 가지 도구 및 척도의 개발이 선행되어야 할 것이다. ; In the Korean society today, the discussion on sexuality is being proceeded in various aspect and in depth. But we barely find any the study and intervention on the sexuality of the mentally ill. Mental heath professionals in Korea acknowledge the reality that the mentally ill are far from living a healthy and normal sexual life, but are very passive on looking at the matter in the aspect of rehabilitation and intervention. To understand the various issues concerning the sexuality of the mentally ill, this thesis reviews the studies on the sexuality of the mentally ill being proceeded in America. Various methods of intervention in America on such matters are also introduced in the study. To provide basic material for the understanding on the sexuality of the mentally ill in Korea, questionnaire inquiry for research on the actual conditions of their sexual attitude, life and knowledge have been carried out on the mentally ill living within community. On the grounds of such studies, the thesis seeks to presents to mental health professionals the direction of approach as to the sexual issues of the mentally ill. 170 mentally ill people in 15 different rehabilitation institutions participated in the questionnaire inquiry. On their sexual attitude, the study shows that the mentally ill have a tendency of valuing chastity and being against prostitution, whereas they take sexual impulses and masturbation as natural. The inquiry on their sexual life shows that the mentally ill are very inactive as to actual sexual intercourse, and that their sex lives are mainly composed of masturbation. They are discontent with their present sexual live and hope to become more active. Males turned out to have a greater hope on the improvement of their sexual lives. We can see through the results of the inquiries that the mentally ill lack sexual knowledge. And whether they had sexual education or not doesn t seem to make a substantial difference as to this matter. For the practice of social work, the results of the study suggest the following: 1. Mental health professionals must have concrete knowledge concerning the sexuality of the mentally ill. 2. They need to actively intervene in the sexuality of the mentally ill on grounds of professional knowledge and skills needed for counseling and education on such matters. Especially, these efforts are more called for the females. 3. The privacy of the mentally ill concerning sexuality should be fully protected. 4. There must be open and free communication between professionals and the mentally ill on the issues regarding sexuality. For following studies, the results of this study suggest the following: 1. Study on the development and effect of education programs as a tool of intervention should be proceeded. 2. The sexuality of the mentally ill females should be looked into more intensively. 3. Research on the sexual attitude and knowledge of mental health professionals and their aspect on the sexuality of the mentally ill is needed.
