전신마취하 제왕절개술시 임산부 체위가 임산부와 태아에 미치는 영향

Abstract

The aortocaval compression by the gravid uterus in late pregnancy lead to acute hypotension in the mother and decrease of uteroplacental blood flow which may affect on neonatal outcome. The purpose of this study was to evaluate maternal hemodynamic changes, and which affect neonatal outcome under general anesthesia for elective cesarean section depending on maternal position-supine or left lateral 15°tilting by wedge. The forty women undergoing elective cesarean section under general anesthesia were delivered through supine position or left lateral 15° tilting by wedge position group randomly. The maternal brachial blood pressure and heart rate were recorded, and maternal arterial blood, umbilical artery and vein blood were sampled. Induction-to-delivery time(IDT), uterine incision-to-delivery time(UDT), and Apgar score were measured. The two groups had no maternal hypotension less than 100 mmHg, and were no significant differences in brachial blood pressure and heart rate of mother, acid-base status and Apgar score of neonate. In conclusion, maternal position had no clinical significant effects on maternal brachial blood pressure and heart rate, and neonatal outcome.

; 임신말기 임산부의 합병증 중 하나인 앙와위 저혈압 증후군(supine hypotensive syndrome)은 임신으로 팽대된 자궁이 대동정맥의 압박을 유발하여 임산부의 급성 저혈압을 유발하고 또한 자궁태반의 혈류량을 감소시켜 태아의 안전에도 심각한 영향을 미칠 수 있으므로 마취기법에 무관하게 제왕절개술전, 중에는 예방하여야 한다. 이에 저자는 선택적 제왕절개술을 위한 전신마취시 이론상 이점이 있다고 알려진 부분적인 좌측경사위의 방법을 오른쪽 둔부에 쐐기를 거치학고 체위에 의한 임산부의 혈역학적 변화와 신생아에 미치는 영향을 연구하고자 하였다. 전신마취하 선택적 제왕절개술을 받는 40명을 무작위로 두군으로 나누어 앙와위군과 쐐기를 이용한 좌측 15。 경사위군으로 나누어 체위변화전, 체위변화후, 기관내 삽관직후, 그후 1분 간격으로 임산부의 우측 상완부의 혈압과 심박수를 측정하고, 태아만출직후 임산부의 요골동맥혈과 제대동정맥의 혈액을 가스분석 하였으며, 마취유도로부터 태아만출까지의 시간, 자궁절개에서 태아 만출까지의 시간, 1분, 5분 Apgar점수를 각각 측정하였다. 앙와위군과 좌측경사위군의 상완부혈압과 심박수는 두군간에 유의한 차이를 보이지 않았으며, 또한 수축기혈압 100 mmHg 미만의 저혈압도 보이지 않았고 두군의 신생아의 산염기 상태와 1분, 5분 Apgar 점수도 유의할 만한 차이를 보이지 않았다. 결론적으로 임산부의 체위는 임산부의 상완부 혈압, 심박수의 혈역학적 변수와 신생아의 상태에 임상적으로 유의할만한 영향을 주지 못하였다.
