백반증이 환자의 생활에 미치는 영향

Abstract

Vitiligo is a distressing skin disease that involves depigmentation of the skin and leaves the patient visibly disfigured by uneven color. The impact of vitiligo on the quality of life of patients is likely to be principally related to alterations in individual appearance and consequent psychosocial disability. The purpose of this study was to evaluate the impact of vitiligo on the quality of life of patients. We interviewed 154 vitiligo patients, and tried to find out the impact of vitiligo quality of life using a questionnaire. The results are summarized as follow: 1. Among 154 cases of vitiligo, the number of male and female patients were 46(30.0%) and 108(70.0%) respectively, and the mean age was 39.8 years. 2. The psychological wellbeing of the patients was moderately or highly affected by the vitiligo more often than any other criteria. 3. The scores of physical, social and psychological criteria of women were significantly higher than those of men 4. The scores of physical, social and psychological criteria of those who married were significantly higher than those of single 5. The scores of physical, social and psychological criteria of those who are employed were significantly higher than those of those who are unemployed. 6. The scores of social and psychological criteria of patients older than 30 years were significantly higher than those of patients younger than 30 years. 7. The period of illness was correlated with the scores of physical, social and psychological criteria. 8.The severity of vitiligo was correlated with the scores of physical, social and psychological criteria. Vitiligo has a substantial impact on the quality of life. These indices can be practically used are to reveal area of particular concern in patients with vitiligo, and to identify individuals who may require more intensive treatment, and to provide a patients oriented information on the quality of life expected. ; 최근 여러 질환에 대한 정신 사회과학적 연구가 증가하고 있지만, 주로 신체적으로 심한 장애가 있는 질환에 초점이 맞추어져 왔으며, 피부질환이 일상생활에 영향을 미침에도 불구하고 신체적 장애가 적은 피부질환의 정신 사회과학적 연구는 그 관심이 적었다. 백반증은 후천적으로 멜라닌 세포가 파괴, 소실되면서 피부에 다양한 크기의 원형 내지는 불규칙한 모양의 탈색반을 초래하는 질환으로, 백반증과 같은 색소 침착 감소 질환은 외관상 타인에게 혐오감을 주어 환자의 일상생활에 영향을 줄 수 있다. 본 연구는 백반증이 환자의 삶에 얼마나 영향을 미치는 지에 관해 물리적, 사회적 및 정신적인 측면으로 나누어 알아보고, 어떤 유형의 환자에서 삶의 질에 영향을 많이 받았는지에 관해 알고자 1998년 7월 1일부터 1998년 8월 31일까지 이화여자대학교 의과대학 부속 동대문 병원 피부과에 내원한 백반증 환자 154명을 대상으로 이학적 검사와 설문조사를 통하여 다음과 같은 결과를 얻었다. 1. 조사대상자 154명 중 남자는 46명(30.0%), 여자는 108명(70.0%)이었으며 평균연령은 39.8세였다. 2. 물리적, 사회적 및 정신적 척도중 정신적 측면이 가장 영향을 많이 받음을 알 수 있었다. 3. 물리적, 사회적 및 정신적인 척도 모두에서 여자인 경우, 배우자가 있는 경우 및 직업을 가지고 있는 경우에서 점수가 높았다. 4. 환자의 연령을 청년층(29세이하), 장년층(30세-49세), 노년층(50세이상)으로 나누어 비교하였을 때 물리적 척도의 점수에서는 각 군간에 차이가 없었지만, 사회적 및 정신적 척도의 점수에서는 장년층과 노년층이 청년층보다 점수가 높았다. 5. 이환기간이 길수록 물리적, 사회적 및 정신적 척도의 점수가 높았다. 6. 노출부나 비노출부에 관계없이 백반증의 크기가 클수록 물리적, 사회적 및 정신적 척도의 점수가 높았다. 이상의 결과로 백반증이 환자의 물리적, 사회적 및 정신적 삶에 영향을 미침을 알 수 있었고 백반증이 삶의 질에 미치는 영향을 조사함으로써 각각의 백반증 환자들이 어떤분야에 관심이 많은지 알 수 있었으며, 치료의 기술적 측면에서 환자 중심의 척도를 얻을 수 있었다.
