Abstract
전통적인 조음 중심의 훈련 접근법은 음운장애 아동이 나타내는 오류음소 하나하나를 목표로 하여 이를 정확히 조음하는 데 초점을 두어왔다. 그러나 이러한 접근법을 통해 훈련 목표음 자체는 빨리 획득될 수 있으나 그 향상이 더 복잡한 음운체계에 일반화되기 어렵다는 제한점이 드러났다. 이에 비해 음운과정 중심의 중재 접근법은 개별 음소가 아닌 여러 음소들에 작용하는 오류음운과정의 소멸을 목표로 삼아 이 오류음운과정이 영향을 미치는 말소리들 전반에 일반화 효과가 나타나도록 한다. 이러한 음운과정 중심의 중재 접근법 중 주기를 이용한 음운패턴 훈련법은 개별 음소들을 정확하게 발음하게 하는 차원과는 다르게 높은 말명료도를 나타낼 수 있도록 음운체계 발달을 촉진시키는 것으로서, 특히 다수의 오류음운과정들을 나타내어 보다 효과적으로 의사소통할 수 있도록 말명료도를 향상시켜야 하는 심한 음운장애 아동들에게 적합하다. 따라서 본 연구는 주기를 이용한 음운패턴 훈련법을 음운장애가 심한 세 명의 아동들에게 실시하여 이들의 음운체계에 나타난 훈련 효과 및 일반화 효과를 알아보고자 하였다. 효과를 검증하기 위해 대상자간 중다 간헐 기초선 설계를 이용하여 (1) 기초선 단계, (2) 총 세 주기(주기 Ⅰ, Ⅱ, Ⅲ) 동안의 중재 단계, 그리고 (3) 중재를 종결한지 3주 후에 실시한 유지 단계에서 목표 오류음운과정들의 출현율을 측정하였다. 일반화 효과 자료로는 비훈련 오류음운과정들 및 문장 수준의 말명료도를 전 실험단계 동안 간헐적으로 수집하였다. 검사 낱말 및 문장은 19개의 전체 자음 음소들을 포함하였고 중재 때는 검사에서 사용하지 않은 훈련 낱말 목록을 사용하였다. 본 연구에서는 음운과정 분석을 통해 세 명의 대상 아동들이 공통적으로 15% 이상의 출현율을 보인 세 개의 오류음운과정들(연구개음의 생략, 경구개음의 전설음화, 치조음의 후설음화)을 감소시키기 위하여 세 번의 주기 동안 각 오류음운과정이 영향을 미치는 음소들을 반복 훈련하였다. 하나의 주기는 6회기로 구성되었는데, 각 목표 오류음운과정당 2회기씩, 세 개의 목표 오류음운과정들을 번갈아가며 모두 훈련하였다. 이 때 매 회기별로 훈련 낱말을 달리하여 이를 다양한 놀이 활동 속에서 연습하였다. 본 연구를 통해서 얻은 결과는 다음과 같다. 첫째, 주기를 이용한 음운패턴 훈련법은 목표로 한 세 개의 오류음운과정들을 효과적으로 감소시킴으로써 대상 아동의 음운과정에 긍정적인 훈련 효과를 나타내었다. 둘째, 주기를 이용한 음운패턴 훈련법은 비훈련 오류음운과정들을 효과적으로 감소시킴으로써 긍정적인 일반화 효과를 나타내었다. 셋째, 주기를 이용한 음운패턴 훈련법은 문장 수준의 말명료도를 향상시키는 일반화 효과를 나타내었다. 넷째, 주기를 이용한 음운패턴 훈련법을 통해 감소된 오류음운과정들은 훈련이 종료된 3주 후에도 유지되었다. 이상의 결과를 종합해 볼 때 주기를 이용한 음운패턴 훈련법은 세 명의 심한 음운장애 아동들의 음운체계 전반에 긍정적인 훈련 및 일반화 효과를 나타내었으며 더 나아가 문장 수준의 말명료도를 향상시켰다고 볼 수 있다. ; This study was designed to investigate the effects of cycle approach to phonological remediation on suppression of phonological processes and improvement of speech intelligibility for three children with severe phonological disabilities. The cycle approach involved simultaneous treatment of three target phonological processes: velar omission, palatal fronting, and alveolar backing. Using the multiple probe design across subjects, the percentage of phonological processes and intelligible words on sentences were measured. Three cycles were planned for treatment and each cycle was composed of six sessions. Each phonological process was targeted for two sessions per cycle and words for training were changed each session with various drill-play activities. The generalization effect was probed five times throughout the whole sessions and follow-up data were collected to examine the maintenance effect three weeks after the treatment was completed. The results from the investigation were as follows; 1. The approach facilitated suppression of three phonological processes treated for all subjects in three cycles. 2. The untrained phonological processes also showed marked changes after the treatment. 3. Speech intelligibility on sentences were gradually improved as each cycle was implemented. 4. The changes shown on the subjects phonological systems were maintained three weeks after the treatment was completed. The cycle approach to phonological remediation was effective for the three children with severe phonological disabilities as evidenced by the training and generalization effects that occurred throughout their phonological systems. The results underscore the importance of designing an ongoing treatment program for an individual child based on the results of a detailed phonological analysis.
