Abstract
간조직 microsome내의 mixed-function oxidase 또는 monooxygenase라고 불리우는 산화효소계는 약물, 발암물질, 스테로이드 및 지방산을 포함하는 여러 화합물들의 대사과정에서 전자전달계로 작용한다. 흡입마취제의 대부분은 폐를 통하여 배설되나 methoxyflurane, halothane, isoflurane 및 enflurane 등의 불소화 흡입마취제는 간조직 monooxygenase계의 대표적 효소인 cytochrome P-450에 의한 대사과정에서 무기불소를 방출하여 생체내에서 독성을 일으킬 수 있다. Cytochrome P-450은 phenobarbital, isoniazid, ethanol 등의 유도성 약물, 나이, 성별, 뇌하수체나 췌장 호르몬등의 여러 요인에 의해 영향을 받으며 다발성 특정형 isoenzyme이 존재하여 기본적 구조, 기질적 특이성(substrate specificity) 및 유도체에 대한 반응이 서로 다르다(Yamazoe 등, 1989). 당뇨병에서 이물질의 대사과정에 관여하는 간조직 microsome내 monooxygenase의 성분인 cytochrome P-450 의 특정형 isoenzyme이 증가 또는 감소하므로 간의 약물대사가 기질적 특이성을 나타내며(Past와 Cook, 1982),이러한 변화는 인슐린 치료에 의하여 억제될 수 있다. 본 연구는 흰쥐에 췌장의 β-cell을 선택적으로 파괴하는 약물인 스트렙토조토신으로 당뇨병을 유발시키고 인슐린으로 치료하는 과정에서 흡입마취제인 halothane, isoflurane 및 enflurane의 복강내 투여시 탈불소화 과정과 간조직 microsomal monooxygenase system에 관여하는 효소 활성도의 변화를 연구하고자 간조직내 무기불소치와 monooxygenase계의 구성성분인 cytochrome P-450, NADPH cytochrome C reductase 및 lipid peroxidase 활성도의 변화를 측정하였고, cytochrome P-450 중 어떤 특정한 isoenzyme이 관여하는가를 밝히기 위하여 ethanol로 유도된 cytochrome P-450의 monoclonal antibody(EtOH 1-98-1)를 가지고 Western blot법으로 측정하여 다음과 같은 결론을 얻었다. 1. 혈중 무기불소치는 halothane, isoflurane 및 enflurane 투여시 각각 당뇨병군에서 정상군에 비해 현저히 높았으며 인슐린 치료기간이 길수록 낮았고 투여한 마취제별로 비교하면, isoflurane 투여시 정상군, 당뇨병군 및 인슐린 치료군 모두에서 halothane이나 enflurane에 비해 통계적으로 의의있게 낮았다. 2. 간조직 cytochrome P-450의 함량은 각 마취제 투여시 당뇨병군에서 정상군에 비해 높았고 인슐린 치료로 점차 낮아져 치료 21일군에서는 정상군과 유사한 치를 나타내었으며, 투여한 마취제별로 비교하면 halothane 투여시 isoflurane이나 enflurane에 비해 당뇨병군, insulin 치료군에서 유의하게 낮았으며 enflurane 투여시에는 isoflurane에 비해 정상군과 인슐린치료군에서 의의있게 높았다. 3. NADPH cytochrome C reductase 활성도는 각마취제 투여시 당뇨병군에서 정상군보다 높았으며 인슐린 치료기간이 길수록 낮아져 치료 21일에는 정상군과 유사한 치를 나타내었고 마취제간에는 차이가 없었다. 4. Lipid peroxidase 활성도는 각 마취제 투여시 정상군에 비하여 당뇨병군에서 높았으며 인슐린 치료로 낮아져 치료 21일에는 정상치와 유사하였고, 투여한 마취제별로 비교하면 isoflurane 투여시 halothane에 비해 정상군, 당뇨병군 및 인슐린 치료군 모두에서 의의있게 높았으며 enflurane에 비해서는 당뇨병군과 인슐린 치료군에서 의의있게 높았다. 5. SDS-polyacrylamide gel electrophoresis와 Western blot 법으로 당뇨병군에서 증가되는 cytochrome P-450의 특정형 isoenzyme은 ethanol 특정형 isoenzyme과 같은 것임을 확인하였다. ; Volatile anesthetics are metabolized by an enzyme system known as the mixed function oxidases or monooxigenases located mainly in the endoplasmic reticulum of hepatocytes. Oxidative and reductive biotransformation of the fluorinated volatile anesthetics, halothane, enflurane and isoflurane, by the cytochrome P-450 enzyme system results in the release of inorganic fluoride. A unique form of cytochrome P-450 has been reported to appear in rat liver microsome in diabetes and the effect of diabetes on hepatic cytochrome P-450 expression can be reversed by insulin treatment. I have carried out studies in rats to determine whether streptozotocin induced diabetes and insulin treatment change the oxidative and reductive defluorination of the fluorinated volatile anesthetics and the cytochrome P-450 mediated enzyme activities. The results are as follows: 1. Significantly high level of serum inorganic fluoride than normal group was observed in diabetic rats with halothane, isoflurane and enflurane administration which was gradually reversed as insulin treatment goes on. Isoflurane administration proved to have statistically lower level of serum inorganic fluoride than the other two administration groups, halothane and enflurane. 2. Hepatic microsomal cytochrome P-450 content was high in diabetic group with all anesthetic administrations and reversed by insulin treatment. Diabetic group treated with insulin for 21 days recorded almost same value of normal group. Halothane administration showed significantly lower cytochrome P-450 level than isoflurane and enflurane in diabetic and insulin treated diabetic group. Enflurane administration showed significantly higher cytochrome P-450 level than isoflurane in normal, and insulin treated diabetic group. 3. Diabetic group had high NADPH cytochrome C reductase activity in all anesthetic administrations and reversed as insulin treatment goes on. The group marked almost same level of normal group when treated with insulin for 21 days. There were no statistically significant differences in between each anesthetic administration. 4. Diabetic group showed high lipid peroxidase activity in all anesthetic administration and reversed by insulin treatment. Isoflurane administration showed significantly high lipid peroxidase activity then halothane in normal, diabetic and insulin treated diabetic group, while showing significantly high lipid peroxidase activity than enflurane in diabetic and insulin treated diabetic groups. 5. With SDS-polyacrylamide gel electrophoresis and Western blot, diabetes specific form of cytochrome P-450 has proved to be the same one as ethanol specific isoenzyme.
