Abstract
금속단백효소는 태생기 발달이나 상처 치유등과 같은 생리학적 상태, 류마티스 관절염 같은 병리상태 그리고 종양의 성장, 침윤 및 전이등에 관여하는 효소로 세포외 기질의 단백분해에 중요한 역할을 한다고 알려져 있다. 최근 상처의 치유, 자외선조사로 의한 피부 노화에 MMP가 관여함이 보고된 바 있으나 이온화 방사선조사에 의한 조직손상시에 관여 가능성에 대한 연구는 없는 실정이다. TIMP는 상처치유과정에서 역할을 한다고 하며 방사선에 의한 조직손상시 재생과정에서 관여 가능성 역시 연구된 바가 없다. 따라서 이온화 방사선조사에 의한 조직손상과 재생시에 MMP와 TIMP의 관련여부를 확인하고자 본 연구를 시행하였다. 실험동물로는 암수구별 없이 생후 4-5개월, 체중 250-300gm의 흰쥐(Sprague-Dawley) 36마리를 대상으로 하였고 이중 6마리는 대조군, 30마리는 실험군으로 분류하였다. 전체 실험군에 1회 8 Gy의 방사선을 전복부조사한 후 각각 5마리씩으로 나누어 1, 2, 3, 5, 7, 14일에 희생시켜 소장을 적출하여 조직병리검사를 통해 조직손상과 재생의 정도를 관찰하였고 면역조직화학염색 및 면역블로팅 방법을 이용하여 MMP-2와 TIMP-2의 발현여부와 그 변화 정도를 관찰하였다. 동시에 혈액을 채취하여 ELISA 검사법을 통해 혈청내 MMP-2와 TIMP-2의 수치를 측정하였다. 소장 점막의 손상은 광학현미경상 염증세포 침윤과 점막위축으로 확인하였고 점막의 재생여부는 유사분열 수 증가와 기저부 증식으로 측정하였다. 면역조직화학염색과 면역블로팅 방법으로 측정된 점막조직내 MMP-2와 TIMP-2의 발현과 ELISA 검사법을 통한 혈액내 MMP-2와 TIMP-2의 발현여부를 관찰하여 다음과 같은 결과를 얻었다. 첫째, 조직병리학적 소견상 점막의 위축과 염증세포의 침윤으로 나타난 소장 점막의 손상정도는 1일부터 평균 1.1점으로 관찰되기 시작되어 2일, 3일에 2.1점, 2.8점으로 가장 심하게 관찰되었다. 둘째, 조직병리학적 소견상 재생의 증거인 기저부 증식과 유사분열 수 증가는 3일에 1.8점으로 시작하여 5일에 3.2점으로 가장 활발한 것으로 보였다. 셋째, 면역조직화학염색법에서 MMP-2는 방사선조사 후 1일에 0.8점으로 나타나기 시작하여 3일, 5일에 2.8점, 2.6점으로 가장 강하게 나타났으며 TIMP-2는 1일에 0.8점으로 나타나기 시작하여 5일에 3.0점으로 가장 강하게 나타났다. 넷째, 면역블로팅 결과 MMP-2는 72kDa에서 확인되었는데 3일에 강한 양성을 보였다. TIMP-2는 21kDa에서 양성으로 확인되었는데 5일에 강한 양성반응을 보였다. 다섯째, 혈액에서의 MMP-2와 TIMP-2의 ELISA 검사 결과 실험군이 대조군에 비해 MMP-2와 TIMP-2 측정치가 각각 유의한 수준의 차이를 보였다. 방사선조사 후 1, 2, 3, 5, 7, 14일의 MMP-2와 TIMP-2 측정치의 변화도 각각 유의한 차이를 보였으며 5일에 가장 높은 수치를 보였다. 따라서 방사선조사 후의 조직병리학적 검사에서 손상의 정도를 나타내는 점막의 위축 및 염증 반응과 재생의 정도를 나타내는 기저부 증식 및 유사분열 수의 변화가 면역조직화학염색, 면역블로팅, ELISA검사에 의한 MMP-2, TIMP-2의 증가 정도 및 시기와 일치하는 것으로 나타나 이들이 방사선조사후의 손상과 재생 기전에 중요한 역할을 하고 있음을 확인할 수 있었다. 그러나 여러 형태의 손상 기전에 따른 조직손상과 재생 기전의 다양성으로 보아 MMP, TIMP의 세부적인 기전에 관한 연구가 추후 시행되어야 할 것으로 생각된다. ; Matrix metalloproteinases(MMPs) are a family of enzymes of which main function is degradation of the extracellular matrix. These enzymes play a significant role in the processes such as wound healing, pregnancy, parturition, bone resorption, mammary involution, tumor invasion, and metastasis. Several studies revealed that MMPs and TIMPs are related in wound healing process and photoaging by ultraviolet irradiation. However, the expression of MMP and TIMP after irradiation has not been studied to our best knowledge. This study investigates the expression of MMP-2 and TIMP-2 in rat intestinal mucosa after irradiation. The entire abdomen of Sprague-Dawley rats were irradiated with a single fraction of 8 Gy. The rats were sacrificed on 1, 2, 3, 5, 7 and 14 days after irradiation. The histopathologic findings were observed using hematoxylin & eosin stain. The expression of MMP-2 and TIMP-2 was examined by the immunohistochemistry, immunoblotting and ELISA test. The results are summarized as follows; 1) Radiation induced damage associated with atrophic villi and infiltration of inflammatory cells was observed from the first day of postirradiation, and a severe tissue damage was observed on the second and the third postirradiation day. 2) Increase of mitosis and number of regenerating crypts as the evidence of regeneration was mostly noticeable on the fifth postirradiation day. 3) In the immunohistochemistry, the expression of MMP-2 was observed from the first postirradiation day and was mostly conspicuous on the third and the fifth postirradiation day. The expression of TIMP-2 was mostly conspicuous on the fifth postirradiation day. 4) In immunoblotting, the expression of MMP-2 was strongly positive on the third postirradiation day and the expression of TIMP-2 showed strong positive response on the fifth postirradiation day. 5) In ELISA test, the expression of MMP-2 and TIMP-2 was increased in postirradiation groups compared to those of normal control group and showed the maximum increase on the fifth postirradiation day. These results were statistically significant. In conclusion, the expression of MMP-2 and TIMP-2 was increased in the intestinal mucosa of the rats after irradiation and these results were correlated with histopathologic findings such as tissue damage and regeneration. So, this study suggests that MMP-2 and TIMP-2 seem to play a role in the mechanism of radiation-induced damage and regeneration of intestinal mucosa of the rat.
