Abstract
이 논문은 공공 정책을 둘러싼 사회집단과 국가간의 관계가 제도적인 통로를 통해서 어떻게 사회적으로 조직화 되는지에 주목하였다. 복지국가의 사회정책 가운데 특히 의료정책에 관심을 가지고 있는 이 논문은 의료부문에서 새로운 정책의 도입과 실천을 둘러싸고 국가와 의사 양자간의 권력의 지형이 개혁 과정에서 어떻게 변화되고 모색되는지 추적해 보고자 하였다. 이를 위해 이 논문은 의료 개혁의 과정을 의료정치를 형성하는 배경적 요인, 국가의 개입과 의사의 대응 방식을 통한 의료정치 그리고 그 산물로서의 정책 결과의 과정으로 나누고 이러한 과정들간의 상호관련성을 영국의 의료개혁을 사례로 삼아 고찰하였다. 이 논문의 구체적인 연구 문제는 다음과 같다: 1) 의료 개혁을 형성하는 구조적인 요인들은 무엇이었는가, 2) 구조적인 요인들을 배경으로 형성된 국가-의사간의 보건의료정치에서 국가는 어떠한 개입 방식을 전략적으로 채택하였는가, 3) 의료전문직을 중심으로 한 전문조직은 국가의 개입에 대해 어떠한 대응 방식을 구사하였는가 4) 의료개혁의 결과는 무엇이며 개혁은 의도한 목표를 달성하였는가. 연구 방법은 영국의 국가보건서비스에 대한 사례연구 방법을 사용하였으며, 분석 자료는 필드웍을 통해 수집된1, 2차 자료 및 인터뷰가 병행되었다. 본 논문의 분석은 다음과 같은 결과를 도출하였다. 첫째, 사회정치적 측면에서 영국의 중앙집중화된 강력한 정치체제는 1990년대 의료 개혁의 드라이브를 급속도로 진전시키는데 기여하였다. 둘째, 제도적인 측면에서 국가가 의료서비스의 재정과 공급에 전적으로 책임을 지는 체계를 통해 정부는 의료를 강력하게 통제할 수 있는 권력을 확보할 수 있었다. 셋째, 신자유주의적인 이념적 조류는 합의정치에 도전하면서 국가의 개혁 방향을 결정짓는데 중요한 영향을 미쳤다. 넷째, 영국의료전문조직의 구조적인 특성 및 취약한 대응 방식은 국가가 의료개혁의 과정에서 의료전문직의 권력을 제한할 수 있는 실마리를 제공하였다. 다섯째, 시장경쟁원칙과 관리주의에 입각한 의료 개혁 전략은 의료전문직의 집합적 권력에 영향을 미쳤고 결과적으로 의료전문직은 정책 결정에 개입할 수 있는 근거를 상당 부분 상실하게 되었다. 아울러 개혁은 개별 의사의 임상적 자율성에도 영향을 미쳤으며 국가-의사간 권력관계의 지형을 새롭게 변모시켰다. 마지막으로, 의료 개혁은 대기자 명단과 대기 시간을 줄이는데 기여하였으나 일부 환자들간의 의료서비스 접근의 불평등을 야기하였다. 이 논문의 결과를 통해, 우리는 공공 정책을 둘러싼 사회집단과 국가간의 상호 협조와 갈등의 역동적인 과정이 개혁의 경로와 방향을 결정짓는데 중요한 역할을 함을 알 수 있다. 이와 더불어 영국의 사례는 의료와 사회가 불가분의 관계를 맺고 있으며 의료의 전문성은 사회적으로 보장 받을 때 확보될 수 있다는 중요한 함의를 던져주고 있다. 주제어: 의료개혁, 사회정책, 이익집단, 의료전문직, 국가, 보건의료정치 ; Given alarming fiscal imperatives, states and interest groups in all advanced industrial democracies have struggled over health care policy. This study explored the tension in the relationship between state seeking autonomy in health care policy formulation and the political mobilisation of concerned interest groups to protect their own interests. Specifically, it examined the political dynamics between state and medical profession over the recent National Health Service reforms in the UK. The NHS reforms were built on the basis of managed competition and a market model of public administration that included performance measures, contracts and increased external scrutiny and control. The reforms represented the state’s attempt to govern the health care system, which may lead to threaten the power and autonomy of medical profession. Key study questions were: What factors account for the proposed reforms, and how did socio-political climates, ideological factors, and institutional contexts shape the reforms? What were the state’s reform strategies to govern the health care system, and how did the medical profession mobilise their political resources to influence the policy-making process? What were the changes following the reforms, and did the reforms achieve their goals? The analysis is based on published and unpublished documentary materials, together with interviews and other contacts. Evidence from UK illustrates that the state has tried to increase state governance in health care and this has triumphed over even effectively mobilised health care providers. The impact of reforms on medical professions was modest. Nonetheless, managers did have greater influence than their predecessors on matters such as control over physician’s treatment and work pattern. The reforms also challenged the collective power of the medical profession and reconfigured the autonomy of individual practitioners in distinctive way. Following the reforms, there were increased outpatient services, reduced long waiting lists and time. Unintended consequences of reform included decreased access for some patients, and changes in the culture of health care system. The research finding suggests that health care politics in which policy is embedded provides both the impetus for, and constraints to, reform of that policy. It also highlights that the dynamics between the state and organised interests surrounding the reforms are seen to have important implications for the scope and direction of health care reform efforts.
