dc.description.abstract본 연구는 신경심리학적 검사와 뇌측화 실험을 통해 읽기장애 하위 유형인 우반구가 주로 우세한 지각형 읽기장애 집단과 좌반구가 주로 우세한 언어형 읽기장애 집단의 분류가 타당한지 알아보았다. 유치원부터 초등학교 6학년사이에 속하는 정상 아동, 지각형 및 언어형 읽기장애 아동들이 각 집단별로 12명씩 참가하였다. 모든 아동들의 지능 지수는 90이상에서 124이하 였다. 교과서 읽기 검사에서 보이는 오류패턴에 근거하여 지각형과 언어형 읽기장애로 나누었다. 교과서 읽기 과제에서 생략, 잘못 읽기와 같은 실제적인 오류를 정상집단보다 많이 보인 집단을 언어형 읽기장애 집단으로 하고, 반면 반복해서 읽거나 수정하는 오류를 정상집단보다 많이 보인 집단을 지각형 읽기장애 집단으로 분류하였다. 연구 1에서 모든 피험자들은 개별적으로 KEDI-WISC, 기초학습기능검사, 청각언어학습검사, Rey-Osterrieth 복합도형검사, 언어유창성검사, 그리고 정신추적검사를 받았다. 그 결과 지각형 읽기장애 집단은 단어에 대한 해독 능력과 시각적 도형기억능력은 양호하지만 언어적 개념 형성, 이해 능력, 시간지연후 청각언어적 기억의 쇠퇴, 인지적 융통성의 부족을 보였다. 반면 언어형 읽기장애 집단은 언어적 개념 형성과 이해 능력은 양호하지만 주의집중력, 세부적인 운동 협응 능력, 단어에 대한 해독 능력, 청각적 언어기억과 시각적 도형 기억, 인지적 융통성의 부족을 나타내었다. 이와 같이 연구 1을 통해 좌반구가 주 결함인 지각형 읽기장애 집단은 언어적 개념형성이나 이해능력이 정상집단에 비해 부족한 반면, 우반구가 주 결함인 언어형 읽기장애 집단은 지각적 특성에 대한 해독능력이 정상집단에 비해 부족함이 밝혀졌다. 연구 2에서 정상집단, 지각형, 그리고 언어형 읽기장애 집단들이 단어와 비단어가 좌측, 우측 시야에 무선적으로 제시되는 뇌측화 과제를 제시받고 그 단어들을 정확하게 읽는 읽기 과제와 그 단어들이 단어인지 비단어인지를 판단하는 어휘판단과제를 수행하였다. 종속변인은 오류수와 반응시간이었다. 읽기과제에서 나온 결과는 다음과 같다. ① 지각형은 비단어에 대해서는 집단간 차이를 보이지 않았지만, 단어가 좌측시야에 제시되었을 때 정상집단과 언어형집단보다 오류수가 많았다. ② 언어형은 단어에 대해서는 집단간 차이를 보이지 않았지만, 비단어에 대해서는 좌측, 우측 시야둘다에서 지각형집단보다 오류수가 많았다. ③ 단어와 비단어 둘다에서 지각형이 정상집단보다 반응시간이 의미있게 느렸다. 이와 같은 결과를 통해서 지각형 집단은 주로 단어의 지각적 특성을 주로 처리하는 유형이므로 의미적 통합적 처리에 유리한 단어에 대해 다른 집단보다 오류를 많이 낸 반면, 언어형 집단은 단어에 대해 의미 위주로 처리하는 유형이므로 단어보다는 비단어에서 지각형집단보다 오류수가 많은 것으로 해석할 수 있다. 또한 지각형은 단어이든 비단어이든 지각처리수준에 집착하여 정확히 읽는데 더 시간을 많이 보내고 있음을 시사해준다. 어휘판단과제에서는 다음과 같은 결과들이 나타났다. ① 오류수에서 집단간 차이는 유의하지 않았다. ② 단어와 비단어 둘다에서 언어형이 정상집단보다 의미있게 느렸다. 이는 어휘판단과제가 주어진 단어와 비단어에 대해 뜻이 있는지 없는지를 빨리 판단한 다음 선택하는 반응을 해야하기 때문에 단순히 읽는 과제와는 달리 더 많은 주의집중능력을 요할 수 있어서 아마도 언어형이 지각형이나 정상집단보다 주의집중력을 요하는 판단과제에서 시간이 오래 걸릴 가능성을 보여준다. 연구 3에서는 정상집단과 읽기장애의 두 하위 우형간의 차이보다는 지각형과 언어형간의 차이에 더 초점을 두었다. 연구 3에서는 지각형과 언어형 읽기장애 집단이 음절의 순서를 뒤바꾸어 만든 단어들을 좌측과 우측 시야에 무선적으로 제시받고, 이들 비단어들을 정확하게 읽는 과제 와 의미 단어로 전환시켜 읽는 과제 에 참여하였다. 종속변인은 연구 2와 마찬가지로 오류수와 반응시간이었다. 다음과 같은 결과를 얻을 수 있었다. ① 비단어들을 정확히 읽는 과제 에서 언어형은 우측시야에서 지각형보다 음절의 순서가 뒤바뀐 단어 를 단어로 바꾸어서 잘못 읽는 오류 를 더 많이 보였다. ② 비단어들을 정확하게 읽는 과제 에서 언어형은 좌측시야에서 지각형보다 잘못 읽기 오류 를 더 많이 하였다. ③ 비단어들을 정확하게 읽는 과제 에서 언어형은 지각형보다 의미있게 더 오랜 반응시간을 보였다. 이상의 결과들은 언어형의 의미처리 위주의 읽기 방략은 주로 좌반구와 더 관련이 있으며 지각적 해독능력의 결함은 주로 우반구와 관련이 있음을 보여준다. ④ 비단어를 의미 단어로 바꾸어서 전환시켜 읽는 과제 의 오류수에서 집단간 차이는 유의하지 않았다. ⑤ 비단어를 의미 단어로 바꾸어서 전환시켜 읽는 과제 에서 언어형은 우측시야에서 지각형보다 의미있게 느린 반응시간을 보였다. 이와같은 결과에 대해서는 언어형 읽기장애 집단이 비단어(음절의 순서를 뒤바꾼 단어) 에 대한 지각적 해독능력의 결함 때문에 지각형보다 반응시간이 느린 것으로 해석할 수 있다. 본 연구는 신경심리학 검사와 뇌측화 실험을 통해 지각형과 언어형 두 집단의 상이한 읽기 결함이 주로 어느 반구의 결함에서 비롯된 것인지 추론해 보았다. 이러한 연구를 통해 지각형과 언어형의 각각 취약한 반구를 활성화시키는 자극의 내용이 달라야 하고, 다른 어떠한 처치보다도 각 집단의 신경심리학적 약점을 자극할 수 있는 반구 특정적인 자극을 해주는 것이 필요하다는 점을 인식시켜주기 위한 기초 지식을 제공하는데 본 연구의 중요성이 있다. ; This paper investigated the validity of the L/P classification by neuropsychological tests and cerebral lateralization tasks that present subjects with words or nonwords to read in lateral visual fields(left/right) on computer. Subjects consisted of P-type dyslexics(n=12), L-type dyslexics(n=12), and normal readers(n=12) with IQ scores between 90 and 124 from preschool to the sixth grades in primary school. P-type dyslexics and L-type dyslexics are classified with the error patterns of text reading test. In the text reading test, P-type dyslexics made more fragmentation errors(correction and repeatation) than did normal group, whereas L-type dyslexics made more substantive errors(omission, addition, and misreading) than normal group. Three studies were performed. In Study 1, all subjects were tested individually KEDI-WISC, KEDI-IBLST, Auditorial Verbal Learning Test, Rey-Osterrieth Complex figure Test, Word Fluency Test, and Trail Making Test. P-type dyslexics had no difference with the decoding ability of words and visual complex figure memory but had difficulties with the verbal concept formation, understanding ability, auditorial verbal memory after 20-minute delay, and cognitive flexibility in contrasted with normal group. In the other hand, L-type dyslexics had no problems with the verbal concept formation, and understanding ability but showed difficulties in attention, detail motor coordination ability, the decoding ability of words, auditorial verbal memory, visual memory, and cognitive flexibility in comparison with normal group. Thus the results in Study 1 indicate that P-type dyslexics with the deficit in left hemisphere are poor at verbal concept formation and understanding ability, while L-type dyslexics with the deficit in right hemisphere are poor at the decoding ability of perceptual characteristics in comparison with normal group. In Study 2, all subjects performed individually the cerebral lateralization task of words or nonwords presented randomly through left/right visual field(LVF/RVF) on computer. They performed two kinds of tasks: reading task and lexical decision task. The number of errors and reaction time were measured. The results in reading task were as follows: ① For the errors of nonwords, P-type dyslexics had no difference with L-type dyslexics and normal group. But for the words through LVF, they made more errors than did the other groups. ② For words, L-type dyslexics had no difference with P-type dyslexics and normal group. But for the nonwords through both LVF and RVF, they made more errors than did P-type dyslexics. ③ For both words and nonwords, P-type dyslexics showed longer reaction time than did normal group. It is interpreted that P-type dyslexics with right-hemispheric visuoperceptual type strategies made more errors for words than did the other groups, while L-type dyslexics with left-hemispheric semantic strategies made more errors for nonwords than did P-type dyslexics. It is inferred that P-type dyslexics showed longer reaction time for both words and nonwords because of their reading strategy for processing dominantly perceptual characteristics of letters. The results in the lexical decision task were as follows. ① For errors, there was no significant difference among groups. ② For both words and nonwords, L-type dyslexics showed longer reaction time than normal group. All subjects had to decide whether given a word or nonword has meaning or not in the lexical decision task unlike reading task. These results inform that reading task and lexical decision task may include independent mental process each other. In study 3, only two groups(P-type dyslexics & L-type dyslexics) participated in the lateralization task of nonwords(arranged randomly the order of letters in a word) through LVF/RVF. All subjects performed two kinds of tasks:reading task & task for transforming nonwords into words. The number of errors and reaction time were measured. The study 3 was interested in the difference between P-type dyslexics and L-type dyslexics rather than the difference between norrmal group and two kinds of reading disabilities ① For the reading task of nonwords through the RVF, L-type dyslexics made more errors(transforming nonwords into words) than did P-type dyslexics. ② For the reading task of nonwords through the LVF, L-type dyslexics made more errors(reading nonwords incorrectly) than did P-type dyslexics. ③ For the reading task of nonwords, L-type dyslexics showed longer reaction time than did P-type dyslexics. It is inferred that semantic strategies of L-type dyslexics is dominantly related with left hemisphere, while visuoperceptual strategies of P-type dyslexics is dominantly related with right hemisphere. ④ For errors in the task transforming nonwords into words, no differences were found between the P-type and the L-type dyslexics. ⑤ For the task transforming nonwords into words through RVF, L-type dyslexics showed slower reaction time than did P-type dyslexics. It is interpreted that L-type dyslexics showed slow RT because of their perceptual decoding deficit of letters. On the basis of these findings, the conclusion seems warranted that the L/P-classification of dyslexia has validity. This paper lend support to the contention that the main neuropsychological factor impeding reading acquisition varies according to dyslexia subtype.-
