Abstract
적혈구조혈의 주호르몬인 erythropoietin은 거핵구형성 및 혈소판생성의 조절에도 관련되어져 있음이 알려져 왔다. 본 연구에서는 recombinant human erythropoietin을 마우스에 투여하여 생체내에서의 거핵구형성 및 혈소판생성에 대한 영향을 평가하고자 생후 8주에서 12주 사이의 웅성마우스(ICR) 435마리를 4개군으로 나누어 실험하였다. 단기간관찰을 위하여 제I군(119마리)과 제II군(118마리)은 1 U 및 5U의 recombinant human erythropoietin을 1일 2회 5일간 복강내 주입하였으며, 장기간관찰을 위하여 제III군(51마리)은 10 U의 recombinant human erythropoietin,을 1일 1회 30일간 복강내 주입하였고, 대조군(147마리)은 단기간투여군 및 장기간 투여군으로 나누어 같은 횟수로 생리식염수를 주입하였다. 실험 시작일을 제0일로 하여 단기간투여군과 대조군은 제 1, 2, 3, 4, 5 및 8일에, 장기간투여군과 대조군은 제 5, 10, 15, 20, 25 및 30일에 실험동물들을 희생시켜 말초혈액검사, 혈소판응집능검사, 골수 및 비장 소견을 관찰하여 다음과 같은 결과를 얻었다. 1. 말초혈액에서 혈소판수는 단기간투여군인 제I군 및 제II군에서는 제 1일에서 제 3일 사이에 대조군에 비해 통계적으로 유의하게 증가하였으며 recombinant human erythropoietin의 투여중지 후에는 대조군과 동일한 수준으로 감소하였고, 투여 용량에 따른 관찰결과 1 U 투여군이 5 U 투여군보다 통계적으로 매우 유의한 혈소판수증가를 보였다(p<0.01). 장기간투여군인 제III군에서 혈소판수는 제 10일 이후 지속적으로 감소하였다. 2. 혈색소, 적혈구용적비 및 적혈구수를 포함한 적혈구지표는 단기간 및 장기간투여군 모두에서 투여용량 및 투여기간에 비례하여 지속적인 증가를 보였다. 3. 혈소판응집능검사는 단기간투여군인 제I군 및 제II군에서 ADP(33.3μM/mL), collagen(200μg/mL) 및 thrombin(10 U/mL) 등의 촉진제에 대하여 정상응집능을 보였다. 4. Recombinant human erythropoietin을 투여한 후 골수에서의 거핵구수는 단기간투여군인 제I군 및 제II군에서는 제 4일에 통계적으로 유의한 감소를 보였고(p<0.05, p<0.01), 장기간투여군인 제III군에서는 제 15일에 통계적으로 매우 큰 유의한 감소를 보였다(p<0.001). 장기간투여군에서는 유의한 변화를 보이지 않았다. 5. 전자현미경적 소견으로 골수에서는 형태적으로 대조군과 별차이가 없는 거핵구가 관찰되었으며, 성숙한 거핵구에서 중간대에 소포나 공포의 증가와 과립이 풍부한 혈소판 영역을 보이지만, 소기관이나 함입이 없는 연변대를 보이는 거핵구도 관찰되었다. 비장에서는 적혈구아세포의 증식과 비장혈관내에 혈소판들도 관찰되었지만 대조군에 비해 공포세포나 혈소판탐식세포의 증가는 관찰되지 않았다. 이상의 결과를 종합하여 보면 recombinant human erythropoietin은 투여초기에는 거핵구직경의 증가와 더불어 정상응집능을 지닌 혈소판의 일시적인 증가를 가져오나, 지속적인 투여는 적혈구지표의 증가에 반비례하여 혈소판수를 감소시킨다. ; Erythropoietin, a major hormone of erythropoiesis, has been shown to involve the modulation of megakaryocytopoiesis in vitro and thrombopoiesis in vivo. To assess the effects of the short-term and long-term administration of the recombinant human erythropoietin on megakaryocytopoiesis and the platelet production in vivo in mice, the total numbers of 435 male mice(ICR) were studied in 4 different groups: Group I(N=119 mice) and II(N=118 mice: 100μl of 1 and 5 U of recombinant human erythropoietin were administered intraperitonially twice a day for 5 days. Group III(N=51 mice), 10 U of recombinant human erythropoietin was applied once a day for 30 days, and control (N=147 mice), 100μl of saline were administered with same frequency. The day 1, was designated as the day following the outset. The mice were sacrificed on days 1, 2, 3, 4, 5 and 8 in the short-term trial, and on days 5, 10, 15, 20, 25 and 30 in the long-term trial. The peripheral blood examination, platelet aggregation, and histologic assessment of spleen and bone marrow were investigated, of which results are as summarized in below; 1. The short-term administration of recombinant human erythropoietin caused the significant increase of platelet count on days 1, 2 and 3, thereafter the count was decreased, eventually returning to the normal levels on day 8. In the long-term administration of recombinant human erythropoietin, the platelet count continued to decrease after the day 10. 2. The recombinant human erythropoietin caused the increased red blood cell production, i.e. it increased RBC count, hemoglobin, hematocrit and reticulocyte counts to the both groups of the short-term and long-term administration, as well. 3. The platelet aggregation in response to ADP, collagen and thrombin stimulation was normal in the short-term administration of recombinant human erythropoietin. 4. In the short-term administration of recombinant human erythropoietin, the megakaryocyte diameter was found enlarged highly significantly on days 1 and 2 in group I(p<0.01, p<0.001) and on days 1 and 4 in group II(p<0.01), and the megakaryocyte number decreased significantly on day 4 in both groups(p<0.05, p<0.01). The long term administration of recombinant human erythropoietin induced a very highly significant decrease in the megakaryocyte number on day 15(p<0.001),but not significant change in the megakaryocyte diameter. 5. On the electron microscopic findings of bone marrow, the marginal zone was devoid of organelles or invaginations, irrespectively of the fact that the intermediate zone displayed the granule-rich platelet territories, and the increase of the vesicles or vacules in the maturing megakaryocyte. On the electron microscopic findings of spleen, the numbers of vacuolar and platelet-phagocytoid cells are not significantly increased notwithstanding the control. The above results show that the short-term administration of recombinant human erythropoietin caused significant increases in the platelet production with normal platelet aggregation activity and the diameter of megakaryocyte, but the long-term administration caused significant decreases in the platelet production and the increases in RBC production.
