말기 암환자의 체험에 관한 현상학적 연구

Abstract

Human health is an integral part of experience in the process of Human Becoming. Through continual interaction with the environment human beings freely choose experience and develop as responsible beings. The process of the health experience of patient with terminal cancer is a unique. The objective of this study is to understand the lived experience of patients with terminal cancer in order to provide basic information for nursing care in the clinical setting and to develop a theoretical background for clinical practice. This study is to describe and define the lived experience of patients with terminal cancer in order to provide a foundation for nursing research and education. Data collection has been done between December 1993 and November 1994. The subjects included five persons ? four females and one male: one who was in her sixties, one in his fifties, two in their forties, and one who was in her thirties. The researcher has met with these patients 35 times, but at eight times the patient was in a stuporous condition and not able to participate, so these were not included in the data analysis. Parse’s “Human Becoming Methodology”, an existential phenomenological research methodology is used for this study. Data has been collected using the dialogical engagement process of “I and You”, the participant researcher and the participant subject. Dialogical engagement was discontinued when the data was theoretically saturated. Data was analyzed using the extraction-synthesis and heuristic interpretation. The criteria of Guba and Lincoln(1985), and Sandelowski(1986); credibility, auditability, fitness and objectivity were used to test the validity and reliability of the data. The following is a description of the structure of the lived experience of patients with terminal cancer as defined by this study: 1. Structure: 1) Suffering through the reminiscence of past experience 2) The appearance of complex emotions related to life and connectedness 3) The increasing importance of significant people and of the Absolute Being 4) The increasing realization of the importance of health and belief 5) Desire for a return to health and a peaceful life or for acceptance of dying and a comfortable death In summary the structure of the lived experience of these patients can be said to be: suffering comes through reminiscence of past experience, and there are complex emotions related to life and connectedness. Significant people and the Absolute Being become increasingly important along with a realization of the importance of health and faith. And finally there is a desire for either a return to health and a peaceful life or for the acceptance of dying and a comfortable death. 2. Heuristic Interpretation: Using Parse’s Human Becoming Methodology, the structure of the lived experience of patients with terminal cancer identified in this research is interpreted as: The lived experience of patients with terminal cancer involves the solving of past conflicts, and the experience of the healing and valuing of sorrow and pain. Through the relation of life and health, and the complex emotions that arise, the lived experience of revealing-concealing is of paradoxical emotions. The increasing importance of significant others and of the Absolute Being shows Connecting and Separating an on-going process of nearness and farness. Revision of thoughts about health and faith is interpreted as transforming and desire for restoration to health and a peaceful life or acceptance of dying and a comfortable death, as powering. In summary, it is possible to see, in the lived experience of patients with terminal cancer, the relationship of the five concepts of Parse’s theory: valuing, revealing-concealing, connecting-separating, transforming, and powering. From Parse’s theory, the results of this study show that meaning is related to valuing, rhythmicity to revealing-concealing and connecting-separating, and cotranscendence to transforming and powering. ; 인간이 건강하다는 것은 ‘인간 되어감(human becoming)’ 과정의 경험 중에서 가장 중요한 체험이다(Parse, 1991). 인간은 끊임없는 환경과의 상호작용을 통하여 자유로이 경험을 선택하고 책임을 지는 존재이다. 말기 암환자는 이러한 건강 체험의 과정 중에서 독특한 경험을 한다. 본 연구의 목적은 ‘말기 암환자의 체험’을 이해함으로써 암환자 간호실무에 도움이 되는 이론적 근거를 개발하며, ‘말기 암환자의 체험’에 대한 구조를 서술하고 확인함으로써 말기 암환자 간호를 위한 연구 및 교육에 기여하고자 한다. 본 연구 기간은 1993년 12월부터 1994년 11월까지였다. 연구참여자는 60대 1인, 50대 1일, 40대 2인, 30대 1인 모두 5인의 말 암환자로 총 35회 면담을 실시하였는데 이중 혼미한 의식 상태에서 실시한 8회 면담은 자료에서 제외하였다. 연구 방법은 실존적 현상학의 한 방법인 Parse의 ‘인간 되어감 방법론(human becoming methodology)’을 적용하였으며, 자료는 연구자와 참여자의 <나와 너>의 대화 과정(dialogical engagement)을 통하여 수집하였고, 참여자의 구술언어에서 추출한 내용을 연구자의 서술언어로 종합하는 단계와 발견적 해석의 단계를 통해 자료를 분석하였다. 자료의 신뢰성 및 타당성을 높이기 위해 Guba & Lincoln(1985)과 Sandelowski(1986)가 제시한 기준에 따라 확실성, 감사 가능성, 적합성, 객관성에 대한 평가절차를 거쳤다. 분석 과정을 거쳐 밝혀진 ‘말기 암환자의 체험’의 구조는 다음과 같다. 1. 구조 (1) 과거의 경험을 회상하며 애통해함. (2) 삶과 연관을 지으며 복합된 정서를 나타냄. (3) 의미 있는 타인과 절대자가 더욱 소중해짐. (4) 건강과 신앙(종료)이 재인식됨. (5) 건강 회복과 평화로운 삶을 원하거나 또는 죽음을 수용하고 편안한 죽음을 기대함. 이러한 말기 암환자 체험의 구조를 통합하면 다음과 같다. 말기 암환자의 체험은 과거의 경험을 회상하며 애통해 하면서, 삶과 질병을 연관 지으며 복합된 정서를 나타내고, 의미 있는 타인과 절대자를 더욱 소중히 여기면서, 건강과 신앙이 중요하게 재인식되고, 건강 회복과 평화로운 삶을 원하거나 또는 죽음을 수용하고 편안한 죽음을 기대하는 과정이다. 2. 발견적 해석 이상의 통합적 구조를 Parse의 ‘인간 되어감 이론’과 관련시켜 다음과 같이 해석한다. 말기 암환자의 체험은 과거 경험을 회상하며 그 갈등을 해결하고자 슬픔이나 아픔의 치유를 가치화하며(가치성), 삶과 건강을 연관 지으며 복합된 정서를 역설적으로 나타내고(노출성-은폐성), 의미 있는 타인과 절대자가 더욱 소중해지는 ‘가까워짐과 멀어짐’이 반복되는 관계(연결성-분리성)속에서, 건강과 신앙(종교)이 재인식되는 등 견해의 변화가 일어나고(변형성), 건강 회복과 평화로운 삶을 원하거나 또는 죽음을 수용하고 편안한 죽음을 기대하는 과정(강화성)이다. 이상과 같은 결과를 요약하면, 말기 암환자의 체험은 Parse 이론의 5가지 개념인 가치화(valuing), 노출성-은폐성(revealing-concealing), 연결성-분리성(connecting-separating), 변형성(transforming), 강화성(powering)에 관련되어 있음을 볼 수 있다. 그리고 Parse이론 중 의미(meaning)에 관한 원리에서는 ‘가치성’만이 나타났고, 관계 형성(rhythmicity)에 관한 원리에서는 ‘노출성-은폐성’과 ‘연결성-분리성’이 나타났으며, 공동 초월(cotranscendence)에 관한 원리에서는 ‘변형성’과 ‘강화성’이 발견되었다.
