치매노인가족을 위한 상담방법 연구

Abstract

This research sets up following purpose. Old people with senile dementia go through slowdown of movement ability in daily life and the dementia weakens the power of self-support. However our social structure and family culture leaves the responsibility of maintenance only to family members and in addition Korea has very passive welfare policy for old people. Therefore insufficiencies in official support system for parents and their family burden the family members with maintenance duty and give much stress to them . As a result, family troubles caused by these factors can give rise to the crisis of family collapse. To solve these family problems, many solutions could be considered and methods of pastoral counseling will be one of them. This research aims at looking into the methods of pastoral counseling. The theories and techniques in methods of pastoral counseling, for example crisis counseling, short term counseling focused at settlement of problems and practice of stress management and so on, are int roduced to solve troubles of the family members with aged senile dementia people. Consequently by means of supporting the settlement of their maintenance stress with the help of those measures, the device, which enables family function to perform peacefully, can be sought for in this research. To accomplish these goals, this research analyzes methods of pastoral counseling for old senile dementia people and their family members by analysis of healing them of symptoms from mistaken recognition of figure, divorce crisis, wandering and in addition by examination of mitigation in collection symptom and recovery cases of family collapse crisis. This thesis observes briefly the definition, the cause, the branch, and the symptoms of dementia as a way of making out the dementia in chapter I. Chapter II describes present situation, illness proportion and estimation of dementia elders by means of understanding old senile dementia people. In Chapter III the peculiar characters and actual maintenance condition of the families and conflicts and controls among them are discussed in understanding of aged dementia people's families. Chapter IV investigates methods of pastoral counseling for old senile dementia people and their family. In the methods of pastoral counseling for dementia elders , the sense for the real training, the value of life, retrospection treatment, social skill training and operational treatment are argued. Also in the methods of pastoral counseling for the family with old dementia people, pastoral counseling theory of dependent family is presented. And as practices of pastoral counseling for families with dementia patients, through study of examples, pastoral counseling methods for dependent families are tried more concretely, after dividing into crisis counseling, short term counseling pointed at problem solution and stress management training and bringing up solutions. As a result of this research, family crisis represented by divorce, abscondence and family collapse ,which are originated in family discordance cumulated for maintenance of dementia elders, are found out. Thus the necessity of introducing solutions through pastoral counseling like crisis counseling, short term counseling concentrated on problem solution and stress management training can be confirmed. One of the problems clarified in this thesis is the lack of counseling agencies in Korea. They are able to give information to supporters who have got tired of long term maintenance of old senile dementia people and they also offer man to man service between dependent family and counselers. However there are only three agencies with those functions in Korea. In case of America , communication service ,which offers dementia related information all day long with counselers composed by dementia patients` supporters and experts, is in active progress. However Korean family culture still shifts the maintenance responsibility for patients with chronic disease, which needs long-term medical treatment like dementia, only on their family members` shoulders. This kind of culture invites a family crisis while the majority of dependent family are exposed to severe stress and discord. Therefore problems to be solved are located in following paragraphs. First, pastoral counseling especially focused at main maintenance objects is demanded. Second, informations about care institution which enable main supporters to get out of the maintenance burden for a little while are required and various interests like government`s financial assistance to escape family collapse crisis ,for instance divorce derived from dementia and abscondence, and counselings of church supporting group in local community are called for. Particularly the reality should be took into consideration that sons and daughters of the dependent family are almost young children or youngsters who are very sensitive and so care program for them should be developed variously. And as practices of pastoral counseling for families with dementia patients, through study of examples, pastoral counseling methods for dependent families are tried more concretely, after dividing into crisis counseling, short term counseling pointed at problem solution and stress management training and bringing up solutions.;우리나라는 치매노인들이 신체 및 정신기능의 저하로 일상생활의 동작능력과 자립정도가 약화될 경우 그 부양책임이 가족에게만 맡겨지는 사회적 구조와 가족문화를 지니고 있다. 거기에 소극적인 노인복지정책 등으로 인해 치매환자와 가족을 위한 공식적인 지원체계가 매우 미미하여 치매노인은 물론 환자를 부양하는 가족에게 주어지는 부양부담과 부양 스트레스로 인한 가족갈등은 가족해체의 위기까지 가져오고 있다. 본 연구는 이러한 치매노인 가족문제를 해결할 수 있는 상담방법에 대해 알아보는 것을 목적으로 한다. 이를 위해 위기상담, 문제해결중심의 단기상담, 스트레스관리 훈련 등의 치매노인 가족문제 해결을 위한 상담방법 이론과 목회적 돌봄의 기법을 도입하여 치매노인가족의 부양스트레스 해소를 원조함으로써 치매노인과 가족이 가족기능을 원활히 수행할 수 있는 방안을 모색하고자 하였다. 이러한 목적을 수행하기 위하여 본 연구에서는 인물오인증세 및 이혼위기, 배회증세 및 수집증세 완화와 가족해체 위기의 치유사례에 대한 분석을 통해 치매노인과 그 가족을 위한 상담방법을 모색해 보았다. 이를 위해 이 논문은 본 연구의 목적을 달성하기 위한 연구의 내용은 서론에 이어 다음과 같이 전개하였다. II장에서는 치매에 대한 이해로서 치매의 정의와 분류, 증상에 대해 간단히 살펴보고 III장에서는 치매노인에 대한 이해로서 치매노인 현황과 유병률, 치매노인 추계, 치매노인시설의 현황을 살펴보고 IV장에서는 치매노인가족에 대한 이해로 치매노인가족의 부양실태, 치매노인가족의 갈등 및 관리방법, 치매노인가족의 문제와 특성을 알아본 후, V장에서는 치매노인을 위한 상담방법에서는 현실감각훈련, 삶의 가치와 회상치료, 사회기술훈련, 작업치료등을 살펴보고, 그 가족을 위한 상담방법으로는 치매노인가족 상담이론과 치매노인가족 상담의 실제로서 위기상담, 문제해결중심의 단기상담, 스트레스관리훈련으로 구분하여 그 방법을 제시하였다. 무엇보다도 치매노인가족을 위한 목회적 돌봄으로서 기독교 자원을 이용한 돌봄과 나눔을 통한 돌봄, 목회자의 돌봄 등을 살펴보고, 사례연구를 통해 보다 구체적으로 치매노인 가족을 위한 상담 방법을 시도해 보았다. 본 연구의 결과 치매노인 부양으로 누적된 가족갈등으로 인하여 이혼, 가출, 가족해체등의 가족위기가 나타나는 만큼 위기상담, 문제해결중심의 단기상담, 스트레스관리 훈련 등의 상담을 통한 문제해결 도입의 필요성과 그 중요성을 확인할 수 있었다. 또한 본 연구의 분석과정에서 나타난 문제점은 우리나라의 경우 장시간 치매노인 수발로 지친 부양자들에게 쉽게 정보를 제공할 수 있으며, 치매노인가족과 상담자를 1:1의 관계로 서비스를 제공할 수 있는 상담기관이 현재 3-4 곳에 불과하다는 점이다. 미국의 경우 치매노인의 부양자와 전문가들로 상담원을 구성하여 24시간 치매에 관련된 정보를 제공해주는 통신서비스를 활발히 진행하고 있는데 비해 한국의 현실은 아직도 치매와 같은 장기요양을 요하는 만성질환자에 대한 부양을 가족에게만 전가시키는 가족 문화적 배경 속에서 대부분의 치매노인 부양가족들은 심각한 스트레스와 갈등에 노출된 채 가족위기에 처해 있다. 따라서 해결해야 할 과제로는 첫째, 주 부양대상자 중심의 상담이 요구되고 둘째, 주 부양자가 잠시라도 노인에 대한 부담에서 벗어날 수 있는 치매노인을 돌보는 보호시설에 대한 정보제공과 치매로 인한 이혼, 가출 등의 가족해체위기에서 벗어날 수 있는 정부의 재정적 지원과 지역사회 교회의 지지집단 구성 등의 주변적 관심의 필요성을 들 수 있다. 이러한 점에서 볼 때 다른 어떤 분야보다도 치매노인가족을 위한 상담의 중요성을 재인식하고 이들을위한 치유프로그램이 다양하게 개발되어야 할 것이다.
