침습적 외상 기억이 경험 회피와 해리를 매개로 외상 증상 및 심리적 부적응에 미치는 영향

Abstract

본 연구에서는 외상 경험자들의 침습적 외상 기억과 외상 증상, 심리적 부적응의 관계에 경험 회피와 해리가 매개하는지 알아보았다. 이를 위해 경험 회피가 외상 증상과 심리적 부적응의 취약성 요인으로 작용한다는 선행 연구와 해리가 경험 회피로 작용할 수 있다는 수용 중심적 접근의 이론적 근거를 바탕으로 침습적 외상 기억과 외상증상, 침습적 외상 기억과 심리적 부적응의 관계에 경험 회피와 해리가 영향을 줄 것이라는 연구 모형을 설정하였다. 연구 대상은 서울, 경기 지역의 대학생과 직장인을 포함한 일반 성인 528명이며 2012년 4월과 5월에 걸쳐 약 한 달간 설문지 자료를 배부, 수집하였다. 질문지 내용으로는 외상 사건을 묻는 외상 체크리스트, 외상 증상 수준을 측정하기 위한 외상 후 스트레스 증상 진단 척도(PDS), 외상 사건과 관련된 침습적 기억을 측정하기 위한 침습적 기억 척도(IES-R-K 침습 문항), 기억상실, 몰입상상, 이인화의 해리 수준을 측정하는 해리 척도(K-DES), 경험 회피 수준을 측정하는 수용행동척도(AAQ-Ⅱ), 우울, 불안, 신체화의 심리적 부적응 수준을 측정하는 간이정신진단검사(BSI-18)로 구성된 설문 문항과 성별, 연령 등 인구통계학적 특징을 포함하였다. 회수된 681명의 자료 중 무성의하게 응답한 것으로 판단되거나 무응답이 포함된 53명을 제외한 528명의 자료를 사용하여 구조방정식 모형을 분석한 결과, 침습적 기억과 경험 회피, 경험 회피와 해리, 경험 회피와 외상 증상, 경험 회피와 심리적 부적응, 해리와 심리적 부적응의 관계는 유의한 것으로 나타났다. 그에 반해 해리와 외상 증상의 관계는 유의하지 않은 것으로 나타났다. 연구 결과를 살펴보면 먼저 연구 모형을 검증 결과 해리와 외상 증상을 제외한 모든 변인 간 관련성이 유의한 것으로 나타났다. 즉, 침습적 기억과 경험 회피, 경험 회피와 외상 증상/심리적 부적응/해리, 해리와 심리적 부적응 간에는 모두 정적인 설명력을 갖는 것으로 나타났다. 구조 모형에 대한 개별 매개 경로의 유의성을 확인한 결과는 다음과 같다. 첫째, 경험 회피는 침습적 외상 기억과 외상 증상, 침습적 외상 기억과 심리적 부적응 수준에 영향을 미치는 것으로 나타났다. 이것은 경험 회피가 심리적 취약성으로 작용하며 외상 증상에 대해서도 증상 악화에 영향을 미친다는 선행 연구와 일치하는 결과이다. 둘째, 침습적 외상 증상과 심리적 부적응의 관계에 경험 회피와 해리는 영향을 미치는 것으로 나타났다. 경험 회피와 해리의 직접효과를 함께 고려할 때 수용 중심적 접근의 이론적 접근대로 해리는 경험 회피의 한 형태로 볼 수 있다. 셋째, 침습적 외상 기억과 외상 증상의 관계에서 경험 회피와 해리는 매개하지 않는 것으로 나타났다. 해리와 외상 증상의 직접적인 설명력은 나타나지 않았으며 이것은 연구 대상의 특성, 또는 해리의 기능에 대한 일관되지 않은 선행 연구 등과 관련이 있을 것으로 여겨진다. 종합하면, 본 연구는 외상 증상의 구조적 이해에 수용 중심적 접근에서 핵심 변인으로 제시하고 있는 경험 회피의 작용을 확인하고자 하였다. 이를 위해 외상 경험자들을 대상으로 침습적 외상 기억과 심리적 부적응, 침습적 외상 기억과 외상 증상의 관계에 경험 회피와 해리가 갖는 매개효과를 검증하였다. 연구 결과 경험 회피가 외상 경험자들의 심리적 증상에 미치는 영향력을 밝힘으로서 외상 연구에 중요 시사점을 줄 것으로 기대한다. 또한 상담 장면에서 침습적 외상 기억으로 고통을 호소하는 내담자들에게 도움이 되는 치료적 원리와 상담 접근을 제공한다는데 의의가 있다.;This study tested a hypothesized model where experiential avoidance and dissociation mediated the relationship between intrusive memory and traumatic symptoms/ psychological distress among Korean adults who experienced traumatic events. The research model was set up based on the previous research that experiential avoidance works as a vulnerability of traumatic symptoms and psychological distress, which is the basic theory of acceptance based approach(e. g. Acceptance Committment Treatment, Dialectical Behavior Treatment). To examine the relationship among variables and role of experiential avoidance which is the core variable of the Acceptance-Based Treatment, survey data from 528 adults exposed to traumatic events was analyzed using structural equation modeling(SEM). The results of data analysis were summarized as follow: First, the results indicated that each variable was positively related, but dissociation was not significantly associated with traumatic symptoms. Second, in this research model, experiential avoidance has an mediated effect on the relationship between intrusive memory and traumatic symptoms, and intrusive memory and psychological distress. Third, experiential avoidance and dissociation significantly mediated the effects of intrusive memory on psychological distress simultaneously. On the other hand, the relationship between intrusive memory and traumatic symptoms was not significantly mediated by experiential avoidance and dissociation at once. To sum up, the purpose of this study is not only to explore the structural relationship among intrusive memory, experiential avoidance, dissociation, traumatic symptoms, and psychological distress but also to examine the role of experiential avoidance, the core psychopathological variable of the Acceptance-Based Treatment. This study investigated mediating effects of experiential avoidance and dissociation on the relationship between intrusive memory and traumatic symptoms/ psychological distress among Korean adults exposed to traumatic events. The findings of this study shows that intrusive memory influence on traumatic symptoms and psychological distress through the experiential avoidance. In addition, results indicated that experiential avoidance and dissociation was influenced on between intrusive memory and psychological distress. Several implications and limitations of this study are discussed and counseling intervention for treatment of traumatic symptoms are suggested.
