dc.description.abstractOwing to advances in neonatology and developments in neonate intensive care units (NICU), the survival rate has increased for the premature babies. Nevertheless, 24-hour brightly-lighted surroundings of NICU, unlike that of the endometrium, leads to a long-term risk of undeveloped visual system of premature infants inclusive of the retina, the light-receptors, and behavioral state. This investigation, thus, was attempted to learn the luminous intensity of NICU and examine how room illumination affects the behavioral state of premature infants. Included in this study were thirty individuals who met the subject selection criteria among the premature infants admitted to Seoul S-Hospital from Oct 1st through Nov 30th 2001. The differences of behavioral states of the premature babies secondary to indoor illuminations of day and night were ascertained, and changing states of the behavioral conditions were examined. With respect to data analysis, various characteristics were obtained by using SPSS-PC (version 9 .0) and the mean and standard deviation of room illumination for each NICU were gathered. A paired T-test was used for the differences of behavioral states of premature infants according to room illuminations. A repeated measured ANOVA was used to examine changes in the behavioral states depending on the room illumination. The study results were written as follows. The mean NICU room illumination for the control group measured at night was 67.77±49.97 Lux while that of the daytime was 15.00±5.18 Lux. For the experimental group for which room illumination was reduced by covering with sheets, the mean illumination at night was 9.89±5.95 Lux while that during the day was 15.00±5.18 Lux. The results of the examining the study hypothesis were as follows: 1. The behavioral states of premature infants depending on room the illumination showed significant differences statistically for both day and night (Day: t=-7.40, p=0.001; Night: t=-3.35, p=0.002). As room illumination increases, they were getting closer to an awakened state from a sleeping state. 2. Changes of the behavioral states of the premature babies following room illuminations showed a significant difference. (F=2.49, P= 0.03). When the room illumination was reduced, changes of the behavioral state for each hour showed a constant modality starting from the breastfeeding hour that occurred every three hours. From the above results, this author learned that the behavioral scores for the premature infants had been low in the reduced luminous environment of NICU, and that the awaken and sleeping state, with respect to changes in behavioral states, had occurred in a constant modality from the starting point of the breastfeeding time. Accordingly, clinicians should recognize the effects of room illumination have on the behaviors of the premature infants, and try to arrive at various systems or to arrange the surroundings. It is important that an environment suitable for growth and development is provided for the premature infants. Based on this study results, suggestions are provided as follows: 1. It is necessary to conduct studies on targeting on severely-premature infants cared in an incubator in the coming days. 2. It is also necessary to have further studies on the effects of room illumination on the physiologic index of premature infants in NICUs. 3. There is a necessity for the studies on proper room illumination of NICU for the growth and development of premature babies in the future.;신생아 중환자실의 미숙아와 고위험 환아를 돌보는데 있어 기술적인 진보와 함께 발달된 의료진의 역할은 미숙아의 생존율을 많이 향상시키고 신생아의 장기적인 결과에 영향을 미친다. 그러나 자궁 내부와는 달리 24시간 내내 밝은 신생아 중환자실 환경은 미숙아들의 광수용체인 망막을 포함한 미성숙한 시각체계와 행동상태에 장기적으로 영향을 미칠 수 있는 위험에 놓여 있다. 이에 본 연구는 신생아 중환자실의 실내 조도를 파악하고 실내조도가 미숙아의 행동상태에 미치는 영향을 알아보기 위해 시도되었다. 본 연구는 2001년 10월 1일부터 11월 30일까지 서울 J병원에 입원한 미숙아 중 대상자의 선정기준에 적합한 30명을 대상으로 밤과 낮의 실내 조도에 따른 미숙아의 행동상태의 차이를 확인하고 행동상태의 변화상태를 파악하였다. 자료 분석은 SPSS-PC(version 9.0)을 이용하여 대상자의 제 특성과 신생아 중환자실의 실내 조도는 각 평균과 표준편차를 구하였다. 실내 조도에 따른 미숙아의 행동 상태의 차이는 paired t-test로, 그리고 실내 조도에 따른 행동상태의 변화는 repeated measured ANOVA로 가설을 검정하였다. 연구 결과는 다음과 같다. 신생아 중환자실의 대조군에서 실내 조도는 밤에는 평균 67.77±49.97 Lux이고 낮에는 평균 142.20±71.67 Lux 였으며, 덮개를 이용하여 실내 조도를 감소시킨 실험군에서 실내 조도는 밤에는 평균 9.89±5.95 Lux이고 낮에는 평균 15.00±5.18 Lux 이었다. 그리고 연구 가설의 검정 결과는 다음과 같다. 1. 실내 조도에 따른 미숙아의 행동상태는 밤과 낮 모두에서 통계적으로 유의 한 차이(낮: t= -7.40, P= 0.001, 밤: t= -3.35, P= 0.002)가 있었고, 실내 조 도가 높을수록 수면상태에서 각성상태쪽으로 가까워지고 있었다. 2. 실내 조도에 따른 미숙아의 행동상태 변화는 유의한 차이(F= 2.49, P= 0.03)가 있었으며 실내 조도를 낮추었을 때의 각 시간에 따른 행동상태의 변화는 3시간마다 이루어지는 수유시간을 기점으로 수면/각성의 일정한 형 태를 이루고 있다. 이상의 결과를 통해 신생아 중환자실의 조도를 낮춘 환경에서는 미숙아의 행동 점수가 낮고, 행동상태의 변화가 수유를 기점으로 수유 전 각성상태와 수유 후 수면상태가 일정한 형태로 나타나는 것을 알 수 있다. 따라서 실내조도가 미숙아의 행동에 미치는 영향을 의료진이 인식하고 여러 가지 적절한 제도나 환경조성이 이루어지도록 노력하여 성장 발달에 알맞은 환경이 미숙아에게 제공되도록 하는 것이 중요하다. 본 연구결과를 기초로 다음과 같은 제언을 하고자 한다. 1. 인큐베이터 내의 극미숙아를 대상으로 한 추후 연구가 필요하다. 2. 신생아 중환자실의 실내 조도가 미숙아의 생리적 지표에 미치는 영향에 대 해서 추후 연구가 필요하다. 3. 미숙아의 성장 발달을 위한 신생아 중환자실의 적정 실내 조도 기준 설정 을 위한 추후 연구가 필요하다.-
