내러티브 탐구를 통한 간호사의 심폐소생술 거부(DNR) 경험

Abstract

회복이 불가능한 환자의 치료에 대한 자율성 존중과 삶의 질 향상을 위한 문제가 대두되면서 심폐소생술 거부(Do-Not-Resuscitate, DNR; 이하 DNR)에 대한 사회적 관심과 수용이 증가하고 있다. 우리나라는 DNR이 제도화되어 있지 않지만 실제로 병원에서 공공연히 시행되고 있어 간호사는 DNR 결정과정에서부터 환자권리, 중재범위와 관련된 다양한 윤리적 갈등상황에 직면한다. 따라서 본 연구는 DNR을 경험하는 간호사의 이야기에 초점을 두고 그들이 어떤 경험을 하며, DNR 경험이 간호사에게 주는 의미가 무엇인지를 파악하여 기술하고 이해하고자 시도되었다. 본 연구는 참여자들의 DNR 경험을 이해하기 위해 Clandinin과 Connelly(2000)의 내러티브 탐구를 적용하였다. 5명의 참여자는 14년 이상 임상경력을 가진 간호사로서 두 기관에서 의도적 표집방법에 의하여 선정하였으며, 자료수집 기간으로 1차면담은 2009년 11월부터 2010년 3월까지, 2차면담은 2010년 9월부터 2011년 2월까지 진행되었다. 자료는 심층면담과 참여관찰, 현장 일기 등을 통해 수집하였으며, 분석과정은 참여자들의 이야기를 연구문제에 따라 기술하고, 그들의 이야기를 내포하는 ‘의미’를 찾기 위해 자료와 분석을 왔다갔다 반복하였다. 본 연구에서 도출된 경험의 의미는 다음과 같다. 참여자들은 치료에 대한 의사결정이 자유롭지 않은 환경 속에서 DNR 결정을 모르는 환자에 대한 연민과 안타까움을 느꼈다. 또한 이러한 환자를 대신하여 DNR을 결정해야 하는 가족의 고뇌와 고통을 이해하면서도 가족들의 지속적인 갈등과 마주하며, 필연적인 죽음 앞에서 어려운 결정을 할 수밖에 없는 현실을 직시하고 있었다. 이렇듯 DNR 환자에 대한 참여자의 감정은 각별했기 때문에 환자의 편안함에 초점을 두었고 안위(安慰)를 위한 돌봄, 환자와 가족 간의 소중한 이별을 돕기 위한 돌봄에 최선을 다하였다. 그러나 잦은 DNR 결정과 임종 경험으로 어느덧 죽음에 타성적인 태도로 변해 버린 자신을 발견하였을 때 갈등을 겪었지만 한편으로 죽음 앞에 초연할 수도 수용할 수도 없는 자신의 입장을 고민하며 자신의 삶을 성찰해 보는 계기가 되었다. 참여자들은 무엇보다 DNR 환자에게 무의미한 심폐소생술과 연명치료가 시행되지 않도록 환자의 가치관이 반영된 생전유언과 사전의료의향서를 통해 존엄성 있는 죽음을 맞이해야 할 필요성을 강조하였다. 본 연구의 결과는 DNR에 대해 아직 제도화되어 있지 않고 구체적인 지침서가 없는 현실에서 간호사들은 DNR 환자를 돌보면서 혼란과 부담감을 느끼면서도 가족의 이해를 도우면서 환자의 안위를 위한 최선의 돌봄을 수행하였다. 따라서 DNR 환자를 위한 간호사의 옹호자적 역할이 강화되어야 하며, 추후 DNR 결정에 간호사의 참여가 적극적으로 이루어져야 한다.;There are rising some problems about the respect of the patient's autonomy and the improvement of living standards as to treating untreatable diseases. So it has been increased to be socially concerned and accepted about the Do-Not-Resuscitate(DNR). In Korea, there was not established a system of DNR, but the treatment of DNR has been put to practice in the every hospitals. So most nurses have to do many things including the determination of DNR treatment, protections of patient's right and some recovery of the interventions range. The purposes of this analysis is to find the meaning and understanding of the DNR Experiences for a nurses. This study applied the theory of the Narrative inquiry by Clandinin and Connelly(2000) to understand the patient's DNR in actual practice. Five nurses who have worked for 14 years in the field were participated in this practice. Also, they were joined from two other hospitals for this experimentation, intentionally. The interviews with them for this study were for two time from November 2009 to March 2010 and Septembe, 2010 to February 2011. The data were collected from the in-depth interviews, participation-observation, field records. The collected data were analyzed according to the research questions and it was processed to find the implications which the data contain. The following results were obtained after analysing of the implications; Above all, the study-participants said that they felt sympathy and inconvenience of the patients who did not know the announcement of DNR under the circumstance, been unable to make decisions of the treatment. Also, they said that they had faced the conflicts between their family and had understood of the family's suffering, at the same time. So they had confronted the problem boldly which they had to decided the announcement of DNR on the verge of the patient's inevitable death. Yet they had did their best to care of them, focused on helping ease the pain because of their minding of the patients. And they had pay particular attention to what they were helping for their peaceful farewells with their family. During the time of nursing the patients, the nurses had some time to reflect upon their's own life seriously. But, most of all, they emphasized the importance of being allowed to die with dignity through the will in their's living years and the signed paper of receiving beforehand-treatments, so that it does not sustain meanless treatments and do needless Cardiopulmonary Resuscitation. The above analyzing result revealed that the truth, the nurses have did their best to caring DNR patients and their family under the confused circumstance which the systems and guidelines do not be set up. So this study suggests some implications for the nursing processes to care of DNR patients that the nurse's role on taking care of DNR patients should be enforced and should be granted the right for the nurse to participate in the making decision of DNR.
