Abstract
배경: 탈리도마이드로 인한 대규모 태아 기형 발생을 계기로 임부에의 안전하고 적절한 약물 사용의 중요성이 대두되었으나, 여전히 국내외의 임부들은 기형유발 가능성이 있는 약물에 노출되어 있는 것으로 나타났다. 이를 극복하기 위해 미국, 호주, 일본 등의 선진국들은 임부에의 적정한 약물 사용을 위한 가이드라인을 수립하여 제공해왔으며, 국내의 경우 2008년 한국식품의약품안전청에서 임부 금기 약품을 공고하였다. 한편 처방∙조제 과정에서 약물 치료와 관련된 위해를 효과적으로 예방할 수 있는 방법으로써 중요성이 강조되고 있는 임상의사결정지원시스템(Clinical Decision Support System; CDSS) 을 활용하여 임부 금기 약품 사용을 검토한다면 임부에의 약물 사용의 안전성 역시 높일 수 있을 거라 생각되었다. 목적: 본 연구에서는 임부 금기 약품 고시 이후의 국내 상급종합병원에서 의 임부 금기 약품에 대한 처방∙조제 시스템의 운영 현황과 아울러 병용금기 및 연령금기 의약품의 운영현황을 조사하고자 하였다. 연구 방법: 2010년 10월 기준 보건복지부에 의해 상급종합병원으로 지정된 전국 44개 병원을 대상으로 2010년 11월 1일부터 10일까지 각 병원의 약제 담당 부서의 담당자에게 온라인 자가 보고형 설문을 이메일을 통해 발송하여 조사하였다. 결과: 전국 44개 상급종합병원에 발송한 설문 중 26개의 병원에서 응답하여 59%의 회수율을 보였다. 설문에 응답한 병원의 96%가 CDSS를 운영하고 있는 것으로 조사되었으며, 이 중 85%에서 임부 금기 약품 정보가 시스템에 반영되어 있었다. 원내 자체 개발 프로그램을 통하여 임부 금기 약품 정보를 반영한 병원이 55%로 가장 많았으며, 58%의 병원에서 10세에서 55세의 여성 환자를 대상으로 임부 금기 프로그램을 운영하고 있었다. 운영 형태로는 86%에서 KFDA 1등급 약물의 처방을 제한하고 있었고, 82%에서는 2등급 약물에 대해 정보제공의 방식으로 운영하고 있는 것으로 조사되었다. 임부 금기 약품 관련 처방∙조제 시스템을 도입 및 운영하는 데에 있어서의 가장 큰 제약 사항은 전산 프로그램 운영의 어려움인 것으로 나타났다. 결론 및 고찰: 임부 금기 의약품이 공고된 이후 2년 남짓한 시간 밖에 흐르지 않았음에도 85%에 해당하는 높은 비율의 상급종합병원이 시스템에 임부 금기 의약품 정보를 반영하고 있었으나 환자의 임신 상태에 대한 정확한 정보를 시스템에 반영하는데 어려움이 있었다. 따라서 임부에게 보다 안전한 약물을 사용할 수 있도록 하기 위해서는 이에 관한 추가적인 연구를 통해, 대상을 정확히 설정하고 정확한 임부 금기 약품 정보를 제공할 수 있도록 해야겠다.;Background: Birth defects caused by thalidomide in 1960s serve as warnings to be cautious when prescribing to pregnant women. Nevertheless pregnant patients are still exposed to teratogenic drugs. For the purpose of preventing the malpractice of administering these drugs to pregnant women, drug administration agencies of developed countries such as USA, Australia, and Japan have provided guidelines for safe and proper prescriptions. In 2008, Korean Food and Drug Administration (KFDA) also published a similar guideline that includes drug categories related to pregnancy. Objective: To analyze how well Korean tertiary general hospitals update their Clinical Decision Support System (CDSS)s in response to KFDA’s update of pregnancy-related drugs. Methods: The survey was conducted from Nov 1st through 10th, 2010. Questionnaires were sent by e-mails to representatives of departments of pharmacy in 44 tertiary general hospitals and answers were collected and analyzed. Results: 26 out of 44 hospitals responded. This study showed CDSSs were actively practiced at 96% of the 26 general hospitals, and 85% of these active CDSSs have drug-pregnancy alerting systems. The majority of hospitals implemented drug-pregnancy alerting systems developed by the hospital themselves. Others implemented systems provided by either Health Insurance Review and Assessment Service(HIRA) or professional CDSS-providing companies. Most hospitals used these systems for female patients aged 10 to 55. In 86% of hospitals the drug-pregnancy alerting systems played the important role of preventing prescription of 1st class hazard drugs to patients, and also by providing drug information of 2nd class hazard drugs in 82% of hospitals. Conclusion: In 85% of the surveyed hospitals, the CDSSs have been well updated according to KFDA’s drug categories in pregnancy within two years of KFDA’s updates. To improve safety for pregnant patients and to make the most use of drug-pregnancy alerting system, we recommend further studies to fortify the systems with standardized data related to precise pregnancy status of patients and continually updated drug information.
