dc.description.abstract연구 배경 및 목적: 회음 하강 증후군의 치료는 환자 개개인의 원인에 따른 증상 개선을 목적으로 하지만 회음 하강의 증가가 발생하는 병태 생리나 이와 연관된 인자들에 대한 이해는 부족한 상황이다. 이에 본 연구를 통해 여성의 회음 하강 증가와 연관된 인자들을 밝히고자 하였다. 연구 대상 및 방법: 연구 대상자는 배변 장애로 내원하여 배변조영술을 시행 받은 여성 201 명을 대상으로 하였다. 각 연구대상자의 나이, 분만력, 수술력과 방사선학적 검사 결과를 후향적으로 분석하였다. 연구 결과: 환자의 나이 (p<0.01), 질식 분만 횟수 (p<0.01) 그리고 휴식기의 직장항문각의 크기 (p<0,01)가 휴식기의 회음 하강 증가와 유의한 연관성이 있었으며 직장류 (p<0.05)나 장중첩증 (p<0.05)의 소견을 보이는 환자에서 유의하게 증가된 휴식기의 회음 하강 증가가 관찰되었다. 반면, 동적 회음 하강의 증가는 연령의 증가 (p< 0,05), 휴식기 항문 내압 (p<0.01) 과 관련이 있었으며 에스결장류가 있는 환자들에서 유의하게 증가된 소견을 보였다 (p<0.05). 그 외의 인자들, 치골직장근 이완부전증의 여부, 수술력 등은 회음 하강의 증가와 연관성이 없었다. 또한 변실금이 있는 환자군에서 다른 군에 비해 평균 휴식기의 회음 하강이 큰 것으로 나타났으나 통계적 유의성은 없었다. 결론: 휴식기의 회음 하강의 증가는 연령의 증가, 질식 분만 횟수의 증가, 휴식기 항문직장각의 증가 및 직장류, 장중첩증의 소견과 연관성이 있었으며 동적 회음 하강의 증가는 연령의 증가, 휴식기의 항문 내압의 증가, 에스결장류 소견과 연관성이 있었다. 이번 연구 결과는 여성에서의 회음 하강 증가의 원인 및 병태생리에 대한 이해를 높임으로써 근본적인 치료법을 찾는데 기초자료로 활용될 수 있을 것으로 기대된다.;Purpose: Treatment of descending perineal syndrome is focus on the personal etiology and improvement of symptoms. However, the etiology of increased perineal descent (PD) is unclear. Therefore the aim of the present study was to evaluate factors associated with increased resting and dynamic PD in women. Methods: From January 2004 to August 2010, defecographic findings in 201 female patients were reviewed retrospectively. Patient’s age, obstetric and pelvic surgery history, manometric results and defecographic findings were compared with resting and dynamic PD. Results: The mean age of the patients was 50.4 ± 15.4 (range, 18-81) years and the mean number of vaginal deliveries was 1.84 ± 1.44 (range, 0-5). The chief complaints were constipation (n=127, 63.2%), fecal incontinence (n=49, 24.4%), anal pain (n=11, 5.5%) and others (n=14, 6.9%). Mean resting PD was 5.5 ± 1.3and dynamic PD was 1.5 ± 1.3cm. Age (p<0.01), number of vaginal delivery (p<0.01) and resting anorectal angle (p<0.01) were correlated with increased resting PD. Also, findings of rectocele (p<0.05) and intussusceptions (p<0.05) were significantly correlated with increased resting PD. On the other hands, Increased dynamic PD was correlated with age (p<0.05), resting anal pressure (p<0.01) and sigmoidocele (p<0.05). There was no significant correlation between non relaxing puborectalis, history of pelvic surgery and increasing PD. Also, there’s no significant difference in PD according to the symptoms. Conclusion: Increased number of vaginal delivery and increased resting rectoanal angle are associated with increased resting PD. Whereas, increased resting anal pressure is correlated with increased dynamic PD. Older age correlates with both resting and dynamic PD. Defecographic findings such as rectocele and intussusceptions are associated with resting PD. And sigmoidocele correlated with dynamic PD. These results can serve as foundational research to better understand the pathophysiology and causes of increasing perineal descent in women and to find a fundamental method of treatment.
