dc.description.abstractNow Korea is not only surrounded by the negative recognition and anxious psychology because of the high cancer occurrence rate and death rate but also suffering from the great loss on the national level with the increasing medical expenses. The prevention of colorectal cancer is needed above all, the death rate of which is increasing as its finding is late in spite of high occurrence rate of all cancers. The reason for colorectal cancer has not been clearly explained, but hereditary element, excessive fat intake on account of the westernized eating habit, and overweight because of the decreased physical activities are representative reasons. But many preceded studies in other countries have shown that physical activities and eating habits have positive influences on the prevention of colorectal cancer, but in Korea very few researches have been carried out. The purpose of this study is to examine the causes of colorectal cancer, to find out how many effects right eating habits and physical activities have on the prevention of colorectal cancer, and to secure the basic data that are needed for the prevention of colorectal cancer. This research examined 100 colorectal cancer patients in D hospital in Seoul who received at least two times of anti-cancer chemical treatments, 100 hemorrhoids patients, and 100 general people, and compared the eating habits and vegetable intake quantities between colon disease patients and general healthy people. The subjects consist of 51.0% of men and 49.0% of women, and their average ages are 53.71 (colorectal cancer patients), 53.02 (hemorrhoids patients), and 53.12 (general people). The overall BMI index of all the subjects proved 57.3% in the normal case. Of their jobs, office clerks proved most of all, and hemorrhoids patients showed the highest household income of all. The relationship between the stress level felt in daily lives before disease occurrence and family cancer history proved significantly close in the colon cancer group, and main activity type and level were light in most cases. The eating habit survey result was that breakfast-skipping rate was significantly high and the regularity of eating was low, eating quantity was one bowl of rice in most cases, and occasional overeating proportion was significantly high. Their favorite food was meat in most cases, and fish and vegetables were often taken in proportionally for nutrition balance. Other cased foods such as ‘ramen’ were most common in the case of instant food intake, and its intake frequency was significantly as high as more than once a week. Meat was most commonly eaten in eating out, and colorectal patients showed less water intake than hemorrhoids patients or general people. Colon & anus diseases are not significantly related to smoking and drinking, and colorectal cancer patients drink wine more frequently and significantly more than general people. The eating habits before disease diagnosis proved that general people go less unbalanced diet than colorectal cancer and hemorrhoids patients, and they are trying to take in various foods. In case they take in beans & bean curd and eat Kimchi more than two times a day, general people and hemorrhoids patients take it the foods significantly more frequently than colorectal cancer patients. In case of taking in instant food and fast food more than three times a week, colorectal cancer patients or hemorrhoids patients eat them significantly more than general people. In taking in vegetables before disease diagnosis, colorectal cancer patients proved to eat significantly less wheat, mixed grains, green vegetables, light-colored vegetables, Kimchi, vegetable milk, soybean paste, etc. than hemorrhoids patients or general people, but drink coffee most frequently. In addition, colorectal cancer patients ate significantly less light-colored vegetables such as garlic, Chinese cabbage, green bean sprouts, cabbage, onion, radish, bean sprouts, etc., green vegetables such as sesame leaves, spinach, etc., which seemed to be related to the occurrence of colorectal cancers. As a result of this study, meat-centered eating habits, which are known as causes for colorectal cancers, didn't have significant effects on overweight, but irregular activities, increased stress, irregular eating habits, taking in unbalanced sources, instant food-centered diet, water drinking, excessive drinking, etc. proved to increase the occurrence rate of colorectal cancers.;현재 우리나라는 높은 암 발생률과 사망률로 인해 암에 대한 부정적 인식과 불안한 심리가 내재되어 있음은 물론 의료비 부담 또한 계속 증가함으로써 국가적으로도 막대한 손실을 낳고 있는 실정이다. 암 중에서도 높은 발병률에 비해 발견이 매우 늦어 사망률이 증가하고 있는 대장직장암의 예방은 무엇보다도 시급한 실정이다. 대장직장암의 원인은 정확하게 규명되지 않았으나 유전적 원인, 식생활의 서구화로 인한 과다 지방 섭취, 그리고 신체활동의 감소로 인한 비만 등이 대표적이다. 그러나 현재 선행 연구 중 신체활동과 식습관이 대장직장암 예방에 긍정적 영향을 미친다는 국외 선행 연구는 많으나 국내 연구는 아직까지 미비한 상태이다. 따라서 본 연구의 목적은 대장직장암의 발생 원인과 올바른 생활습관(신체활동과 식습관)이 대장직장암 예방에 얼마나 효과가 있는지를 알아봄으로써 대장직장암 예방에 필요한 기초 자료로 활용할 수 있는 계기를 마련하고자 한다. 본 연구는 서울지역 소재 D병원에서 최소 2회 이상의 항암화학요법 진행중인 대장직장암 환자 100명, 치질환자 100명과, 일반인 100명을 대상으로 대장질환 환자와 일반 건강인의 식습관 및 식물성 식품 섭취 실태를 조사하였다. 전체 연구대상자의 성별은 남자 51.0%, 여자 49.0%이고, 평균 연령은 대장직장암 53.71세, 치질 53.02세, 일반인 53.12세로 나타났다. 조사대상자의 BMI지수에 대한 결과는 전체적으로 정상이 57.3%,로 가장 많았다. 직업은 사무직이 가장 많았으며, 가계소득은 치질 환자가 가장 높았다. 발병 전 일상생활에서 느끼는 스트레스 정도와 가족의 암 병력은 대장직장암 군에서 유의적으로 높았던 것으로 나타났으며, 주요 활동 형태 및 활동 수준은 가벼운 활동이 가장 많았다. 식습관 조사 결과 아침식사 결식 비율이 유의적으로 높아 식사의 규칙성이 떨어졌으며 식사량은 밥 1공기가 가장 많았고, 가끔 과식하는 경우가 유의적으로 높았다. 좋아하는 식품으로는 육류의 비율이 가장 높았으며, 균형 식 으로 어육 류 반찬과 야채반찬을 골고루 먹는다는 경향이 유의적으로 높았다. 인스턴트 식품 섭취의 경우 기타(라면 등 용기 류)가 가장 많았으며 섭취 횟수로는 주 1회 이상이 유의적으로 높았다. 외식에서 고기류(숯불구이, 불고기, 삼겹살 등)의 선호도가 가장 높았고, 평소 물 섭취량은 전체적으로 대장직장암 환자의 경우 치질 환자나 일반인 보다 물 섭취량이 낮은 것을 알 수 있었다. 대장․항문 질환 유무와 흡연, 음주 섭취실태는 유의적 차이가 있었고, 대장직장암 환자는 일반인에 비해 음주 빈도와 음주량이 유의적으로 높게 나타났다. 질환 진단 전 식생활 태도 차이는 일반인이 대장직장암 환자와 치질 환자에 비해 편식을 하지 않으며 다양한 식품을 섭취하기 위해 노력하고 있는 것으로 나타났다. 콩, 두부, 순두부 등 콩으로 된 음식 섭취의 경우와 김치를 매일 두 끼 이상 먹는 다의 경우 일반인과 치질환자가 대장직장암 환자에 비해 더 자주 섭취한 것으로 유의적인 차이가 나타났다. ‘인스턴트음식(라면)이나 패스트푸드(햄버거 등)를 일주일에 3회 이상 먹는다’는 대장직장암 환자나 치질 환자가 일반인에 비해 유의적으로 높게 섭취하였다. 질환 진단 전 식물성 식품의 섭취빈도는 대장직장암 환자에서 곡류의 통밀, 혼합잡곡과 채소류의 녹색채소, 담색채소, 배추김치, 두류의 두유, 양념류의 된장 등을 치질 환자나 일반인에 비해 유의적으로 더 적게 섭취하였고, 음료 류의 커피는 상대적으로 가장 섭취빈도가 높았던 것으로 나타났다. 또한 채소류의 대파, 마늘, 배추, 숙주나물, 양배추, 양파, 무, 콩나물 등 담색채소와 깻잎, 시금치 등의 녹색채소 섭취가 대장직장암 환자 군에서 유의하게 낮은 것이 대장직장암의 발병과 관계되는 것으로 나타났다. 본 연구 결과 대장직장암의 위험인자로 알려진 육류 위주의 식습관, 비만 등의 요인에서 유의한 결과를 보이지 않았지만, 불규칙적인 활동, 스트레스의 증가, 불규칙적인 식습관, 반찬의 섭취방법, 인스턴트 위주의 식단, 물 섭취량, 과도한 음주로 인해 대장직장암 발병 율을 높일 수 있다는 점을 알 수 있었다.-
