Abstract
연구배경 최근 다제내성결핵의 증가는 공중 보건과 결핵 관리에 큰 위협이 되고 있다. 일반 감수성이 있는 결핵치료와 다르게 다제내성결핵은 아직 세계적으로도 표준화된 치료가 없으며 일반 초치료 약제에 잘 반응하지 않는다. 우리나라의 결핵관리는 공공의료 및 민간의료 부문으로 양분되어 있다가 민간병원의 비중이 점점 증가하여 현재는 결핵 환자의 75%이상이 민간병원에서 치료를 받고 있으나 일개 민간병원이 아닌 다기관에서의 다제내성결핵 환자의 치료 결과에 관한 연구는 아직 많지 않다. 방법 본 연구에서는 후향적인 방법으로 2004년 1월 1일부터 12월 31일까지 대한결핵 및 호흡기학회 소속 결핵연구회 참여 민간병원 21곳에서 다제내성결핵으로 새롭게 진단되거나 재치료를 시작한 170명을 대상으로 하였다. 모든 환자는 일차 결핵약제와 이차약제에 대해 약제감수성검사를 시행하였으며 과거 결핵치료력과 약제감수성검사 결과에 따라 개별화된 약제로 치료하였다. 치료 결과, 누적생존율, 치료성공의 예측인자를 조사하고 장기 생존율 및 재발율을 분석하였다. 결과 170명 중 광범위내성결핵환자는 11명, 그외의 다제내성결핵 환자는 159명이었다. 남자가 64.1%이었고 평균 연령은 44.5세였으며 20-30대의 연령이 약 절반을 차지하였다. 평균 체질량지수는 20.2 ㎏/㎡ 이었으며 HIV 항체 검사를 시행한 81명에서 모두 음성이었다. 환자들은 과거 평균 4가지 약을 사용한 병력이 있었고 이번 치료에는 중앙값 5개의 약을 병합하여 치료를 시작하였다. 치료 성공한 환자에서 평균 치료기간은 24.5개월이었다. 다제내성결핵의 치료 결과는 완치, 치료완료, 단기치료종료, 실패, 전입, 중단, 사망 24.1%, 21.2%, 5.3%, 4.1%, 16.5%, 21.8%, 7.1%로 치료성공률은 49.4%이었다. 수술적 절제술은 10명(6.1%)에서 시행되었으나 수술유무에 따른 치료 성공률의 유의한 차이는 없었다. 중증의 방사선학적인 중증도는 치료 성공에 있어 불량한 예측인자로 작용하였고 낮은 체질량지수와 더불어 낮은 생존율에 관여하였다. 결론 최근 국내 민간병원에서 광범위내성결핵의 비율은 과거에 비하여 호전이 없었다. 광범위내성결핵과 다제내성결핵의 치료성공률은 여전히 낮았으나 중단율은 상당히 높았다. 낮은 체질량지수와 진행된 방사선학적 소견은 전체 사망률에 있어 위험인자로 작용하였다. 국내에서 민간병원에서의 광범위다제내성결핵을 포함한 다제내성결핵의 전파 및 진행을 예방하기 위해서는 적절한 결핵 관리 정책 강화가 필요하다.;Background The increasing rate of drug-resistant tuberculosis (TB) is a threat to the public health and TB control. In Korea, about 75% of TB patients are treated in private hospitals and the rate is continuously increasing since 2000. We retrospectively enrolled multidrug-resistant TB (MDR-TB) patients treated in private hospitals joined to Korean TB Study Group (KTBSG) and analysed the clinical characteristics and treatment outcomes in those MDR-TB patients. Methods From January to December, 2004, one-hundred-seventy patients who were newly diagnosed with or retreated for MDR-TB at 21 private hospitals were enrolled in this study. All these patients were confirmed as having MDR-TB by drug-susceptibility testing (DST) for first- and second-line anti-TB drugs. All patients were treated with an individualized regimen based on DST results and previous anti-TB treatment history. We retrospectively reviewed medical records of the patients, initial treatment outcomes and cumulative survival rates were analyzed, and predictors of treatment success were analysed. Results Of 170 patients with MDR-TB, male was dominant (64.1%), mean age was 44.5 years old and mean BMI was 20.2. No patients were positive for HIV serology. On hundred sixty eight(61.3%) patients had a history of previous anti-TB treatment. Eleven (6.5%) were confirmed to have extensively drug-resistant TB (XDR-TB) at treatment initiation. In XDR-TB, older age and lower BMI compared with non-XDR MDR-TB. Median 4 drugs were used previously and, in this treatment, the patients were initially treated with median 5 drugs. The mean duration of treatment in treatment success group was 24.5 months. Treatment success, failure, transfer out, default and death rate was 84 (49.4%), 7 (4.1%), 28 (16.5%), 37 (21.8%), 12 (7.1%). There was no significant difference in the success rate between XDR-TB and non-XDR-MDR-TB. Adjunctive surgical treatment was done in 10 patients(6.1%) and the treatment success rate was 6 patients(60%) (no difference between surgical and non-surgical group). radiologic severity was significant predictor for treatment success. Conclusion In private hospitals in korea, the proportion of XDR-TB was comparable t previous data. The treatment success rate of MDR-/XDR-TB was still poor. The default rate was quite high. Low BMI and advanced radiologic finding were risk factor for all cause mortality. Adequate TB control policies should be strengthened to prevent the further development and spread of MDR-/XDR-TB in private hospitals in Korea
