dc.description.abstractCommunity-acquired pneumonia (CAP) is a major infectious disease with significant morbidity and mortality worldwide and therefore, it is important to provide early and proper antimicrobial therapy. However, conventional methods including culture lack sensitivity and take long time whereas advanced molecular diagnostic test using polymerase chain reaction (PCR) method has great sensitivity and provide result rapidly. Streptococcus pneumoniae and Haemophilus influenzae are most prevalent pathogens of community-acquired pneumonia and those are commonly detected in PCR assay because of its high sensitivity. In Korea, study that used molecular test for detection of pathogens in pneumonia patients has not been reported. We evaluated implication of condition with detection of S. pneumoniae and H. influenzae using PCR method in respiratory specimen. From October 2008 to April 2009, patients tested for S. pneumoniae and H. influenzae using multiplex PCR system, Pneumobacter ACE Detection assay (Seegene, Korea) in our hospital were enrolled. 99 Patients with CAP and 99 patients with other pulmonary diseases were randomly selected. In addition to, 99 healthy adults without pulmonary disease were collected. Urinary antigen test for S. pneumoniae, culture in respiratory specimen and blood were performed, if possible. In CAP patients, PCR assay showed positive results with 53.5%(53/99) and 31.3%(31/99) for S. pneumoniae and H. influenzae, respectively. In pulmonary disease patients, similar results with 48.5%(48/99) and 37.4%(37/99) for S. pneumoniae and H. influenzae were revealed. Healthy adults group also showed similar results with 53.5%(53/99) and 40.4%(40/99) for S. pneumoniae and H. influenzae, and were negative for Mycoplasma pneumoniae, Legionella pneumophila, Bordetella pertussis and Chlamydophila pneumoniae. Positive rates for S. pneumoniae and H. influenzae among 3 groups showed no significant difference. Sensitivity, specificity, positive and negative predictive values of PCR for S. pneumoniae in CAP patients were 100%, 58.2%, 37.7% and 100%, respectively based on urinary antigen test, those were 100%, 53.5%, 24.5% and 100%, respectively according to culture in respiratory specimen, those were 100%, 48.4%, 7.5% and 100%, respectively based on blood culture, and those were 100%, 65.7%, 54.7% and 100%, respectively according to all 3 tests with urinary antigen test, culture in respiratory sample and blood. In conclusion, molecular diagnostic test for S. pneumoniae and H. influenzae is very sensitive and thus can exclude infection when its result is negative. Specificity is relatively low and hence, in case of positive result it should be considered clinical manifestation and other test result, concerning possibility of false positive result. In addition to, in throat specimen of healthy adults, PCR assay detected S. pneumoniae and H. influenzae and thus, it is presumed that molecular diagnostic method can detect colonization. To overcome these limitation, PCR assay using other specific marker or quantitative PCR assay should be developed and evaluated. M. pneumoniae, C. pneumoniae, L. pneumophila, and B. pertussis were not detected in healthy adults. Isolation of these pathogens with culture method is known to be difficult. Therefore, PCR test would be useful for detection of these pathogens. Keywords : molecular test, community-acquired pneumonia (CAP), Streptococcus pneumoniae, Haemophilus influenzae, PCR;지역 사회 획득 폐렴은 전세계적으로 유병률과 사망률이 높은 주요한 감염 질환 중의 하나로 적절한 치료를 신속히 수행하는 것이 중요하다. 그러나 배양 등의 전통적인 방법은 예민도가 낮고 시간이 오래 걸리는데 비해 중합효소연쇄반응 원리를 이용하는 분자 진단법은 신속하고 민감도가 높아 사용이 증가하고 있다. Streptococcus pneumoniae와 Haemophilus influenzae는 지역 사회 획득 폐렴의 가장 흔한 원인균들로, 예민한 분자진단 검사에서 검출되는 경우가 흔하다. 국내에서는 이제까지 폐렴의 원인균을 주로 배양 등의 전통적인 방법으로 밝혀 왔으며 실제 폐렴 환자를 분자 진단법으로 검사한 연구가 없었다. 본 연구에서는 호흡기 검체에서 중합효소연쇄반응 검사법으로 S. pneumoniae와 H. influenzae가 검출되는 경우 그 의의에 대해 알아보고자 한다. 2008년 10월에서 2009년 4월까지 본원을 방문하여 다중 중합효소연쇄반응 시스템인 Seeplex?? Pneumobacter ACE Detection assay (Seegene, Korea)를 이용해 S. pneumoniae와 H. influenzae 검사를 시행한 환자들을 대상으로 하였다. 지역 사회 획득 폐렴군 99명과 폐질환군 99명을 무작위로 선정하였고 호흡기 질환이 없는 정상인군 99명을 모집하였다. 가능한 경우 소변 항원 검사, 호흡기 검체와 혈액 배양 검사를 실시하였다. 중합효소연쇄반응 검사 결과 지역 사회 획득 폐렴군에서는 53.5%(53/99)에서 S. pneumoniae가 양성이었고, 31.3%(31/99)에서 H. influenzae가 양성이었다. 호흡기 질환군에서도 S. pneumoniae가 양성인 경우가 48.5%(48/99), H. influenzae가 양성인 경우가 37.4%(37/99)로 유사한 결과를 보였다. 정상인군에서의 결과도 S. pneumoniae와 H. influenzae의 양성률이 각각 53.5%(53/99), 40.4%(40/99)로 비슷하게 나타났고, 그 외 Mycoplasma pneumoniae, Legionella pneumophila, Bordetella pertussis 및 Chlamydophila pneumoniae는 검출되지 않았다. 세 군에서 S. pneumoniae와 H. influenzae의 양성률에는 유의한 차이가 없었다. 지역 사회 획득 폐렴군에서 S. pneumoniae의 중합효소연쇄반응 검사에 대한 민감도, 특이도, 양성예측도, 음성예측도는 소변 항원 검사를 기준으로 했을 때 100%, 58.2%, 37.7%, 100%, 호흡기 검체 배양 검사를 기준으로 했을 때는 100%, 53.5%, 24.5%, 100%, 혈액 배양 검사 기준시는 100%, 48.4%, 7.5%, 100%, 소변 항원 검사, 호흡기 검체 및 혈액 배양 검사를 모두 고려했을 때는 100%, 65.7%, 54.7%, 100%였다. S. pneumoniae와 H. influenzae에 대한 분자 진단 검사는 민감도가 우수하여 결과가 음성인 경우 감염을 배제할 수 있다. 특이도는 상대적으로 낮아서 결과가 양성인 경우 위양성의 가능성도 고려하여 임상 양상과 다른 검사 소견과 함께 고려하여 해석할 필요가 있다. 한편 건강한 성인의 인후 검체에서 중합효소연쇄반응 검사로 S. pneumoniae와 H. influenzae가 검출되어 분자 진단 검사는 집락만 형성한 경우에도 균주를 검출할 수 있는 것으로 생각되며 결과 해석시 이를 고려해야 한다. 이를 극복하기 위해 다른 표지자를 사용하는 분자 진단 검사나 정량 중합효소연쇄반응 검사에 대한 개발 및 평가가 더욱 필요할 것이다. 그 외 Mycoplasma pneumoniae, Chlamydophila pneumonia, Legionella pneumophila, Bordetella pertussis 등의 다른 균주는 정상인에서 1예도 검출되지 않았고, 배양하기도 어려워 중합효소연쇄반응 검사로 감염을 진단하는데 유용할 것으로 사료된다.-
