일개 대학병원 남자 건강검진 수진자에서 혈청 gamma-glutamyltransferase와 Framingham 위험 점수와의 관계

Abstract

연구배경 혈청 gamma-glutamyltransferase는 과도한 음주와 간담도계 질환의 표지자로 사용되었으나 최근에 독립적인 심혈관계 질환의 위험인자로 생각되고 있다. Framingham 위험점수는 향후 10년간의 심혈관계 위험을 예측하는 지표이다. 본 연구를 통해 혈청 gamma-glutamyltransferase와 Framingham 위험점수를 통해 구한 심혈관계 위험과의 관계를 알아보기로 하였다. 방법 서울 시내 일개 대학병원의 건강검진 수진자중 20-79세의 남자 4546명을 대상으로 단면 연구를 시행하였다. 자기 기입식 문진표, 신체계측, 혈청검사가 시행되었으며 심혈관계 위험은 Framingham 위험점수를 통해 구하였다. 결과 연구 대상자는 평균 36.82±8.61세였으며 단순 선형 회귀분석에서 로그치환 혈청 gamma-glutamyltransferase와 Framingham 위험점수는 양의 상관관계를 보였다.(R2:0.05,P<0.001)심혈관 질환과 관련된 변수들을 통제한 다중 선형 회귀 분석에서 로그치환 혈청 gamma-glutamyltransferase와 Framingham 위험점수는 양의 상관관계를 보였다.(R2:0.22,P<0.001)ROCcurve에서 향후 10년간의 심혈관계 위험10%이상(중간이상 위험군)을 나타내는 혈청 gamma-glutamyltransferase 의 최적의 값은 26.5IU/L이었다.(민감도 72.8%, 특이도 38.8.%) 결론 혈청 gamma-glutamyltransferase와 Framingham 위험점수 사이에 양의 상관관계가 있으며 혈청 gamma-glutamyltransferase가 Framingham 위험점수로 구한 중간이상 위험군을 판별하는 지표로 유용하다.;Background Serum gamma-glutamyltrasferase has been used as a marker for excessive alcohol consumption or liver disease, but it has been reported recently that serum gamma-glutamyltrasferase is associated with cardiovascular disease. The aim of the study was to investigate the association between gamma-glutamyltrasferase and Framingham risk categories. Methods We performed a cross-sectional study on 4546 Korean males aged 20 to 79 years, who underwent annual health check up in a university health promotion center. Self-administered questionnaires, physical examinations, and laboratory tests were performed. Cardiovascular risk was estimated using the Framingham risk score. Results Median age of total participants was 36.82±8.6 years. Log-transformed serum gamma-glutamyltrasferaseas has a positive relationship with Framingham risk score.(R2:0.05,P<0.001).In a multivariable analysis model, we found significant positive relation between log-transformed serum gamma-glutamyltrasferase and Framingham risk score.(R2:0.22,P<0.001) The ROC curve demonstrated that serum gamma-glutamyltrasferase as of 26.5IU/L is useful to discriminate 10-year cardiovascular risk of more than 10%.(sensitivity:72.8%, specificity:38.8%)(P<0.001) Conclusion There was a positive association between serum gamma -glutamyltrasferaseas and Framingham risk score. The serum gamma-glutamyltrasferase may be a useful marker for the detection of cardiovascular risk by Framingham risk category.
