망막앞막 박피 시 제거된 조직의 내경계막 유무가 수술 결과에 미치는 영향

Abstract

망막앞막은 망막 뒤틀림으로 인하여 변형시와 시력 저하를 야기 시킬 수 있는 질환으로 망막의 여러 층의 단면 모습을 보여주는 빛간섭단층촬영기의 발전으로 인해 보다 정확한 진단이 내려지고 있다. 본 연구에서는 망막앞막제거술 시 박피된 조직의 내경계막 유무가 수술 결과에 미치는 영향에 대해 알아보고자 하였다. 망막앞막제거술 시 박피한 생검조직을 투사전자현미경 검사를 통하여 망막앞막만 제거된 군과 망막앞막과 함께 동시에 내경계막도 일부 제거된 군으로 분류한 후, 술 후 최대교정시력의 변화 및 중심황반두께의 변화 등을 분석하였다. 망막앞막제거술 시 망막앞막만 제거된 군은, 술 전 최대교정시력 logMAR 0.454±0.28에서 술 후 1개월째 logMAR 0.354±0.15, 3개월째 logMAR 0.264±0.17, 마지막 내원 시 logMAR 0.246±0.15로 술 전과 비교하여 각각 통계학적으로 유의하게 호전되었다(p<0.05). 중심황반두께도 술 전 359.64±97.96 ㎛에서 술 후 1개월, 3개월, 마지막 내원 시 각각 292.17±72.71 ㎛, 269.75±46.79 ㎛, 272.57±53.80 ㎛로 통계학적으로 유의하게 감소하였다(p<0.05). 망막앞막과 함께 내경계막의 일부가 동시에 제거된 군에서는 술 전과 비교하여 술 후 최대교정시력의 호전과 중심황반두께의 감소를 보였으나 그 정도가 통계학적으로 유의하지는 않았다. 두 군 모두에서 망막앞막이 재발한 경우는 각각 1명씩으로 재발율에는 차이가 없었다. 망막앞막제거술 시 내경계막의 손상 없이 망막앞막만 제거된 것이 술 후 예후가 좋다.;Epiretinal membrane (ERM) is an avascular cellular membrane over the macular area occasionally causes metamorphopsia or visual disturbance. Since the 1970s, pars plana vitrectomy has been performed to remove the membranes and good visual outcome can be achieved in many cases. In the present study, we have evaluated the clinical outcomes after ERM peeling. The surgical specimens following ERM surgery were examined by transmission electron microscopy and were classified only ERM (ERM group) and ERM contained ILM (internal limiting membrane) fragments (ERM+ILM group). Change in best corrected visual acuity and central macular thickness were measured preoperatively and postoperatively. Twelve of the 22 specimens had only ERM (ERM group) and ten of the 22 specimens had the proportion of ILM within ERM (ERM+ILM group). In both groups, best corrected visual acuity was improved and central macular thickness was decreased postoperatively. There were statistically significant differences in the mean values of preoperative and las26t follow-up best corrected visual acuity and central macular thickness in group A. Recurrent ERM developed in each one case of both groups. In conclusion, ERM peeling without a damaged ILM appears to have favorable visual and anatomic outcomes.
