Abstract
Aspirin과 비스테로이드성 소염·진통제(nonsteroidal anti-inflammatory drugs, NSAIDs)는 과민반응을 유발하는 가장 흔한 약물 중 하나이고, 이를 확인하기 위한 방법으로는 경구 유발검사가 유일하다. 그러나 이 방법은 심각한 형태의 과민반응이 유발되었던 경우는 금기이고, 환자들의 거절로 시행 못하는 경우가 많아, 다른 안전한 대체 검사법이 절실하다. 흐름세포측정을 통한 호염기구 활성화 검사(flow cytometry assisted basophil activation test, FAST)는 환자의 호염기구를 채취하여 이를 원인 물질과 반응시켜 호염기구의 활성화 및 탈과립을 정량적으로 알아보는 방법으로, aspirin/NSAIDs 과민반응에 대해서는 유럽에서 몇 차례 시행되었으며, 특이도는 100%, 민감도는 40~70%로 보고되었다. 본 연구에서는 이 진단검사법을 한국인에 적용했을 때의 민감도와 특이도 및 양성 예측율과 음성 예측율을 측정하여 이 검사법의 유용성을 확인하고자 한다. 2007년 8월부터 2008년 8월까지 알레르기내과에 내원한 환자 중 aspirin/NSAIDs를 복용하고 과민반응을 보인 환자 20명을 환자군으로 설정하였고, 대조군은 환자군의 나이 및 성별과 맞추어 2:1 비율로 11명을 선정하였다. 환자군은 과민반응이 호전된 후 최소 2주 이상 지나고 항히스타민제와 항류코트리엔 제제 및 스테로이드제제를 1주 이상 복용하지 않은 상태에서, 대조군에서는 1주간 항히스타민제와 항류코트리엔 제제, 스테로이드제제 및 aspirin/NSAIDs를 복용하지 않은 상태에서 혈액을 채취하였다. 실험은 상용화된 Flow-CAST kit□ (Bu‥hlmann laboratories AG, Scho‥nenbuch, Switzerland)을 사용하였고, 약물은 aspirin 1.25 mg/mL, ibuprofen 50 μg/mL, diclofenac 12.5 μg/mL을 사용하였으며, 음성 대조를 위해 stimulation buffer를, 양성 대조를 위해 항 IgE 수용체 항체를 사용하였다. 결과는 IgE와 CD63에 동시에 양성반응을 보이는 호염기구의 비율을 측정하여, 약물에 의해 활성화된 호염기구 비율을 음성대조에서 보이는 호염기구의 비율로 나눈 값인 stimulation index가 2 이상이고 약물에 의해 활성화된 호염기구의 비율이 최소한 5% 이상인 경우를 양성이라 정의하였다. 1. FAST 검사법은 91%의 특이도와 93%의 양성 예측율을 보였다. 그에 비해 민감도는 65%이었고, 음성 예측율은 59%를 나타냈으며, 양성 결과에 대한 가능도 비(likelihood ratio)는 7.2로, 음성결과에 대한 가능도 비는 0.38로 나타났다. 2. 시험 약물 중 aspirin에 대한 양성율은 과민반응을 유발한 약제가 salicylates 이었던 경우 57%, propionics의 경우 67%, acetic acids의 경우 80%이었다. 그러나 ibuprofen에 대한 검사에서는 양성반응을 보인 환자는 없었으며, diclofenac은 1명의 환자에서만 양성 반응을 보였다. 3. 환자군의 아토피 상태(83% vs. 50%)나, 과민반응의 형태 및 과민반응의 중증도는 결과에 의미 있는 변화를 주지 않았다. 그러나 과민반응의 형태가 호흡기 증상인 환자 수와 과민반응의 중증도가 심한 환자수는 적었기 때문에 통계학적인 의미는 없었다. 유럽에서 시행된 같은 연구와 비교했을 때 본 연구에서 첫째, 특이도가 상대적으로 낮았다는 점, 둘째, 환자의 과민반응을 유발한 약물 종류에 상관없이 모든 환자에서 aspirin에 대한 검사 결과만 양성으로 나왔다는 점이 특기할 사항이다. 이에 대해서는 대상 환자수를 더 늘리고 특히 호흡기계의 과민반응을 보이는 환자를 더 포함하여 연구해 보거나, 가능하다면 인종간의 유전자 다형태 차이와 FAST의 관련성도 연구해 볼 수 있겠다. 전체적인 민감도가 70% 미만으로 낮아 이를 올릴 방법에 대한 모색도 앞으로 더 연구되어야 한다. 이러한 점들을 보완해 나간다면 FAST는 환자의 안전성 확보와 검사의 간편성 및 신속성을 고려할 때 충분히 임상에서 적용할 수 있는 검사법이며, 향후 경구유발검사를 대체할 수 있을 가능성이 높다.;Background/Aims : Aspirin and nonsteroidal anti-inflammatory drugs (ASA/NSAIDs) are common etiology of drug hypersensitivity. Oral challenge test is the only definitive diagnostic test for ASA/NSAIDs hypersensitivity. The purpose of this study was to assess the reliability of flow cytometry assisted basophil activation test as a safe alternative diagnostic method, which has been already proved its high sensitivity and specificity in many other drugs and allergens. Subjects and Methods : Twenty patients and eleven healthy controls were enrolled. Using the FLOW-CAST kit□(Bu‥hlmann laboratories AG, Scho‥nenbuch, Switzerland), 31 determinations were performed with 1.25 mg/mL of aspirin, 50 μg/mL of ibuprofen and 12.5 μg/mL of diclofenac. The analysis was performed by flow cytometry to detect double positive staining to anti-IgE and anti-CD63. Stimulation index was defined as activated basophil percent after drug stimulation/basally active basophil percent. Stimulation index more than 2 and absolute value of activated basophil percent more than 5 were considered as positive result. Results : Patients with ASA/NSAIDs hypersensitivity had female predominant characteristics (M:F=1:3) and their atopy prevalence was higher than controls (70% vs. 36%, P<0.05). Regardless of culprit drugs, FLOW-CAST showed positive results only in aspirin. Sensitivity of 65%, specificity of 91%, positive predictive value of 93%, negative predictive value of 59%, positive likelihood ratio of 7.2, and negative likelihood ratio of 0.38 were achieved. Ten patients showed positive FLOW-CAST results to structurally unrelated NSAIDs not implicated in adverse reactions. Conclusions : Flow cytometry assisted basophil activation test for aspirin is useful diagnostic methods for ASA/NSAIDs hypersensitivity.
