심초음파도를 이용한 임신부의 좌심실 기능에 관한 연구

Abstract

Cardiac size and myocardial performance were studied in 52, the third trimester of pregnant and 60 nonpregnant women for the purpose of evaluation of cardiac functions during pregnancy (natural volume overload state). All the studied subjects were in the third decade of age and were free of cardiovascular symptoms. This study used noninvasive echocardiography without subjecting the pregnant woman to cardiac catheterization or angiography or exposing her to X-ray irradiation. The following results were obtained. In the pregnant group heart rate, left ventricular end-diastolic dimension, end-systolic dimension, end-diastolic volume, end-systolic volume, cardiac output, left ventricular wall mass and aortic root motion were significantly elevated. However, fractional shortening, ejection fraction, mean velocity of circumferential fiber shortening and anterior mitral valve E-F slope which indicate left ventricular performance were significantly altered. This observation is evidence that left ventricular performance remains within normal range despite marked alterations in left ventricular volume.;임신으로 인한 지속적인 혈액량의 증가로 심근에 가해지는 생리적 긴장으로 기인된 혈역학적 변화를 관찰하기 위하여 위험부담이 있는 심도자법, 혈관조영술 또는 방사선조사등에 비하여 위험부담이 없으면서 비관혈적 방법인 심초음파를 이용하여 심장크기와 좌심실 기능등의 심기능을 측정하였다. 저자는 1980년 2월부터 9월까지 이화대학교 부속병원에서 산전관리를 받고 있는 20대 임신말기 임신부 52명과 대조군으로 심·맥관계 질환이 없는 이화대학교 의과대학생 60명을 관찰대상으로 심기능을 측정하여 다음과 같은 결과를 얻었다. 심박동수, 확장기종말좌심실치수, 수축기종말좌심실치수, 확장기종말좌심실용량, 수축기종말좌심실용량, 심박출량, 좌심실질량 및 대동맥근저운동은 대조군에 비하여 임신군에서 유의한 증가를 보였다. 좌심실 기능의 척도인 fractional shortening, 구출율, 평균원주심 근섬유단축속도 및 승모판전막 E-F slope은 두군 사이에 별 유의한 차이를 발견할 수 없었다. 결론적으로 심초음파도를 이용하여 관찰한 임신부에서의 좌심실확대는 현저하였지만 좌심실 기능 자체의 변화는 없음을 알 수 있었다.
