여성우울증환자의 신체증상에 관한 연구

Abstract

The author studied the somatic discomforts Wittenborn 69-Item Somatic Discomfort Inventory among 120 depressed women that received inpatient and out-patient medical treatment in Department of Neuropsychiatry, Soon Chun Hyang Medical College and Paik Hospital of In Je Medical College from Sept. 1, 1979 to Dec. 31, 1980. The results were as follows: 1. As to age distribution, the highest frequency was observed in the age group between 30 and 39. 2. Somatic discomforts reported by at least 50 % of all patients were 13 items including "anorexia"(70%), "awake during the night"(69%), "can't fall asleep"(64%), "indigestion and stomach cramps" (63 96), "headache"(58 %), "periods of weakness/extreme fatigue"(57 %), and "heart pounding"(56 %), of all 69 items. 3. The classes of discomforts with higher pretreatment scores were "wakefulness"(2.08±0.80), "gastric"(1.59±0.67), "fatigue"(1.57±0.64), "dry mouth"(1.53±0.64), "climacteric"(l.40±0.59). and all of these classes showed statistically significant improvement during the treatment period. (p<0.001) Each items of discomforts with higher pretreatment scores were "awake during the night"(2.19±0.83), "anorexia"(2.15±0.83), "can't fall asleep"(2.06±0.79), "feel faint or dizzy"(1.95±0.80), "headache"(1.94±0.92), "indigestion and stomach cramps"(1.91±0.83). 4. 8 Classes of discomforts such as "autonomic", "intestinal", "gastric", "wakefulness", "dry mouth", "climacteric", "constipation" and "muscular" showed statistically significant higher scores in the depressed group at admission than in the non-patient control group. (p.<0.05). In the distributions of ages, the most characteristic somatic discomforts were "intestinal" and "menstrual difficulties" in 20_(s), "muscular discomforts" in 30_(s), "dry mouth" in 40_(s), "climacteric", "constipation" and "fatigue" in 50_(s), Sleep disturbances showed generally high scores among depressed women of all ages.;우울증은 우리가 임상에서 가장 흔히 경험하는 정신과적 질환중의 하나이다. 이러한 우울증이 남자에 비해 여자에서 더 많이 발생하며 우울감정 이외의 다양한 신체증상으로 나타나는 경우가 많기 때문에 본 저자는 여성 우울증환자를 대상으로 신체증상 호소에 관한 다음 몇가지의 의문점을 밝혀 보고자 하였다. 첫째, 정상대조군에 비해 여성 우울증환자에서 특히 많이 호소하는 신체증상은 무엇인가? 둘째, 치료기간중 우울증세가 호전됨과 동시에 현저한 감소를 보이는 신체증상은 무엇이며 매주마다 증상의 변화양상은 어떠한가? 셋째, 연령분포에 따른 우울증의 신체적 증상의 차이는 어떠한가? 이상 세가지의 의문에 대한 해결을 위하여 본 연구를 시도하였으며 이로써 우울증의 진단과 치료 및 예후의 판정에 도움을 얻고자 하였다. 저자는 1979년 9월 1일부터 1980년 12월 31일까지 순천향의대 부속병원 및 인제의대 부속 백병원 신경정신과에서 입원 및 외래치료를 받은 여성 우울증 환자 120명과 동 수인 여성 정상인군을 대상으로 하였다. 조사방법은 Wittenborn(1979)의 69-Item Somatic Discomfort Inventory를 사용하여 환자의 치료 3주까지 매 1주 간격으로 증상의 평가를 하였다. 그 결과는 다음과 같다. 1. 우울증은 30∼39세의 연령범위에서 가장 높은 빈도를 나타냈다. 2. 69항목의 증상분포는 「입맛이 없다」(70%), 「잠이 깊이 들지않고 쉽게 깬다」(69%), 「소화가 안되고 위가 아프다」(63%), 「머리가 무겁다」(58%), 「항상 피로하다」(57%), 「가슴이 두근거린다」(56%)등에서 높게 나타났다. 3. 우울증은 신체계통별 분포는 「수면상태」(2.08±0.80), 「위계통」(1.59±0.67), 「피로감」(1.57±0.64), 「구강건조」(1.53±0.64), 「갱년기 증상」(1.53±0.70), 「변비증상」(1.47±0.66), 「자율신경계통」(1.40±0.59)등에서 높게 나타났으며 이들 모두 3주의 치료기간동안 현저한 증상의 호전을 보였다. (p<0.001> 4. 우울증군의 신체증상은 정상대조군에 비해 자율신경계통, 위, 장계통, 수면상태, 구강건조, 갱년기 증상, 변비증상 및 근육계통의 8개 증상군에서 유의하게 높게 나타났으며 (p<0.05), 연령별 신체증상은 20대에선 장계통 및 월경상태가, 30대에선 근육계통, 40대에선 구강건조, 50대에선 갱년기증상과 변비증상 및 피로감이 특히 높게 나타났다. 수면장애는 모든 연령층에 걸쳐 높게 나타났다.
