Abstract
성대마비(vocal cord paralysis)는 이비인후과 영역에서 비교적 빈번히 관찰되는 징후로서, 주로 후두내재근(intrinsic laryngeal muscle)에 분포하는 되돌이후두신경(recurrent laryngeal nerve)의 병변에 의해 발생한다. 성대의 기능회복(functional recovery)을 유도하기 위한 다양한 방법의 되돌이후두 신경 재연결(reanastomosis of recurrent laryngeal nerve)이 시행됨에도 불구하고 후두의 기능회복율은 매우 낮다. 현재까지 알려져 있는 기능 비회복의 원인으로는, 크게 신경 재샛이 일어나지 못한 신경 비재생(nonregeneration of nerve)과 신경재지배는 이루어지나 잘못된 방향으로 신경섬유가 재생됨으로써 발생되는 협동운동(synkinesis)이 알려져 있으며 협동운동의 발생 빈도는 매우 다양한 것으로 보고되어 있다. 한편, 간헐적전자기장자극(pulsed electromagnetic stimulation, PEMS)은 불유합(nonunion)된 뼈의 재생을 촉진시킨다고 알려진 이후, 절단된 말초신경에서도 신경재생을 조기에 유도하고 또한 기능적으로도 보다 만족할 만한 회복을 일으키는 것으로 알려져 있다. 본 연구자는 흰쥐의 되돌이후두신경을 절단하고 재연결한 후 PEMS의 적용으로 대조군에 비해 조기에 높은 기능회복율을 관찰한 연구를 바탕으로, 성대 기능회복과 신경재생의 형태학적 변화의 연관성을 관찰하고 기능 회복의 실패 원인을 형태학적으로 규명하고자 본 연구를 수행하였다. 실험동물로는 Sprague-Dawley 흰쥐에서 되돌이후두신경을 절단하고 재연결한 후 PEMS를 적용한 군과 적용하지 않은 대조군을 대상으로 하였다. 실험종료(18주)시까지 살아남은 동물의 되돌이후두신경을 적출하여, 신경연결부위를 중심으로 위아래로 각각 2㎜ 떨어진 원위부(distal segment)와 근위부(proximal segment)의 절편을 전자현미경 표본으로 제작하여 Hitachi H-600 투과전자현미경으로 관찰하였다. 연구결과 기능회복군에서는 모두 신경의 재생이 잘 이루어진 것을 확인 할 수 있었다. 기능이 회복되지 않은 군에서도 PEMS군과 대조군에서 각각 80%(4/5), 100%(5/5)의 신경 재생소견이 관찰되었다. 본 연구로부터 되돌이후두신경의 재생과정에서 나타나는 기능 비회복의 주원인은 주로 협동운동(sinkinesis)에서 오는 것임을 알 수 있었다. 이는 PEMS 적용이 잘못된 방향으로의 신경재생을 줄임으로서 협동운동의 발생을 줄여 후두의 기능 회복을 유도할 가능성을 시사한다. 아울러, 협동운동이 기능 비회복의 주원인으로 작용하는 되돌이후두신경에서는 형태학적인 연구만으로는 정상적인 신경재생과 잘못된 방향으로의 신경재생을 감별할 수 없으므로, 형태학적 연구와 기능적인 연구가 항상 병행되어야 할 것으로 판단되었다.;Recovery of laryngeal function after a recurrent laryngeal nerve injury is an important clinical issue because of the highly coordinated activity is required for proper functioning of respiration, swallowing, and phonation. Although various reanastomosis techniques have been applied to get laryngeal reinnervation, the recovery rate of laryngeal function is reported to be very low. So far, the two major mechanisms in failure of laryngeal functional recovery are known to be failure in nerve regeneration and synkinesis, regeneration of nerve fiber in wrong direction. Since PEMS (pulsed electromagnetic stimulation) had been reported to promote regeneration of bones, it was also reported to have an accelerating effect of the regeneration of injured peripheral nerves. Recently, we found that the rate of functional recovery was higher than that of control group when PEMS was applied to after recurrent laryngeal nerve transection and reanastomosis in rats. In the present study, for investigation the relationship of recovery in vocal cord function and nerve regeneration, and the reason of functional recovery failure, we used the Sprague-Dawley rats and applied PEMS to entire body after transection and reanastomosis of recurrent laryngeal nerve, For control group, the same procedures were used except application of PEMS. We analyzed the ultrastructure of the distal and proximal segments of transected and reanastomed recurrent laryngeal nerves by transmission electron microscope. In this study, complete nerve regeneration were observed in the group of functional recovery. For the group of non-functional recovery, proper nerve regeneration were observed in 80% and 100% of PEMS-applied and control group, respectively. From the above results, synkinesis is regarded as the major cause of failure of functional recovery in recurrent laryngeal nerve regeneration. And it is considered that application of PEMS has an effect on the proper guiding of axonal sprouts. So, it reduce the incidence of synkinesis and result in higher rate of functional recovery than that of control group. Also it is considered that both the structural and the functional study will be needed to differentiate the functional reinnervarion from misdirected regeneration after recurrent laryngeal nerve injury.
