유방 종괴의 필름 유방 촬영과 유방 초음파 검사의 비교

Abstract

1994년 1년간 본원을 내원하여 유방 초음파 검사에서 발견된 176종괴를 대상(평균연령 : 39.4)으로 필름 유방 촬영의 종괴 발견율을 알아보고 종괴의 성상(고형성, 낭종성)과 크기(1cm이상, 1cm이하)에 따른 발견율의 차이를 후향적으로 분석하여 다음과 같은 결과를 얻었다. 1. 유방 초음파 검사에서 발견된 총 176개의 종괴중 66개의 종괴(37.5%)만이 필름유방 촬영에서 발견되었다. 2. 유방 초음파 검사에서 고형성인 종괴는 135예로 이중 필름 유방 촬영에서 발견된 예는 56예(41.5%)이며 이중 43예가 1cm이상의 크기(76.8%, 43/56)였다. 보이지 않은 79예는 모두 촬영소견이 고음영 유방(dense breast)이었다. 3. 유방 초음파 검사에서 낭종성인 종괴는 41예로 이중 필름 유방 촬영에서 발견된 예는 10예(24.4%, 10/41)뿐이었으며, 발견되지 않은 31예는 모두 고음영 유방이었고 1cm이하인 것이 24예(77.4%, 24/31)이었다. 4. 필름 유방 촬영에서 고형성과 낭종성 종괴의 발견율과 1cm이상과 1cm이하의 종괴의 발견율은 통계적으로 유의하게 차이가 있었다(p<0.05). 유방 초음파 검사와 비교하였을때 필름 유방 촬영의 종괴 발견율은 낮았으며(37.5%), 종괴를 발견하지 못한 경우는 대부분이 부분적이거나 전체적인 고음영 유방 소견이었기 때문이라고 할 수 있다. 결론적으로 고음영 유방 소견이 없으며 1cm이상의 고형성 종괴는 필름 유방 촬영에서 대부분 발견할 수 있으나 고음영 유방인 경우는 반드시 초음파 검사를 병행하여야 할 것으로 생각된다.;The 176 masses found on breast sonogram of patients that visited our hospital, from January 1994 to December 1994(mean age: 39.4). Breast sonogram was considered as the gold standard. We analyzed detection rate of mass on mammogram, and analyzed retrospectively difference of detection rate according to nature(cystic, solid) and size(more than 1cm, less than 1cm) of mass which was showed on breast sonogram. The results are as follows : 1, 176 masses were found on breast sonogram and only 66 masses of them(37.5%) were found in mammogram. 2. 135 solid masses were found on breast sonogram and only 56 masses of them were found on mammogram and 43 solid masses were more than 1cm in size. The remained 79 masses were not found on mammogram due to dense breast. 3. 41 cystic masses were found on breast sonogram, and only 10 masses of them were found on mammogram. The remained 31 masses were not found on mammogram due to dense breast and 24 masses of them(77.4% 24/31) were less than 1cm in size. 4. There is statistically significant difference between detection rate of solid and cystic mass, and detection rate of mass more than 1cm and less than 1cm (p<0.05). The detection rate of mass was low(37.5%) on mammogram, as compared with breast sonogram, because dense breast concealed the masses on mammogram. In conclusion, most of solid mass more than 1cm in size were found on mammogram in the cases without dense breast, but breast sonogram always should be performed in the cases with dense breast on mammogram.
