Perkins 압평안압계와 Goldmann 압평안압계에 의한 안압측정의 비교연구

Abstract

Perkins applanation tonometer has been known to be useful in mass screening program in respect to its easiness and simplicity. To evaluate the clinical usefulness of Perkins applanation tonometer, the intraocular pressures of 214 eyes of 107 men and women at the age range of 8 to 82 years without eye disease were measured with the Goldmann applanation tonometer as well as Perkins applanation tonometer(sitting and supine position). The results were as follows 1. The difference of the intraocular pressure between the Perkins tonometer(sitting position) and the Goldmann tonometer is less than 3mmHg in 91.1% of the estimation and less than 1mmHg in 43.9%. 2. The average intraocular pressure measured by the Perkins tonometer(sitting position) is lower than that by the Goldmann tonometer. In the higher intraocular pressure zone, the difference of intraocular pressured between the two method is more increased 3. In the supine position, intraocular pressures measured by Perkins tonometer were increased by 2.99mmHg and the pressure increment was statistically significant(P<0.0001). 4. The correlation between the intraocular pressured measured by the Perkins tonometer(sitting position) and those by the Goldmann tonometer is very high(correlation coefficient 0.87). According to the above result, we conclude that Perkins applanation tonometer is not only Portable and easy to use. but also clinically useful for glaucoma screening.;운반이 용이하면서도 환자의 위치에 관계없이 비교적 정확하게 안압측정이 가능한 Perkins 압평안압계가 집단검진이나 안정이 필요한 환자의 안압측정에 유용하다고 알려져 있다. 이러한 Perkins 압평안압계의 정확도를 알아보고자 1997년 3월부터 1997년 5월까지 이화여자대학교 의과대학부속 동대문병원 안과에 내원한 8세에서 82세사이의 안질환이 없는 남녀 107명 214안을 대상으로 안압을 측정한 후, 그 측정치를 임상적으로 가장 정확한 Goldmann 앞평안압계와 비교하여 다음과 같은 결과를 얻었다. 1. 좌위에서 측정한 Perkins 안압 측정치중 91.1%가 Goldmann 안압측정치와 3mmHg이내의 차이를 보였으며, 43.9%가 1mmHg이내의 차이를 보였다. 2. Perkins 안압측정치의 평균은 Goldmann 안압측정치 평균보다 낮게 나왔으며, 양 군간의 안압차는 안압이 높은 구간에서 더 큰차이를 보였다. 3. Perkins 안압측정치중 좌위에서보다 앙와위에서 측정한 안압이 평균 2.99mmHg상승하였으며 이는 통계적으로 의의가 있었다 (p<0.0001). 4. Perkins 안압측정치와 Goldmann 안압측정치는 높은 상관관계(상관계수 0.87)를 보였다. 이상의 결과로 Perkins 압평 안압계는 집단검진이나 안정이 필요한 환자의 안압측정등 임상적으로 유용하게 쓰여질 수 있을 것으로 생각된다.
