소아의 구개편도 및 아데노이드 비대가 부비동에 미치는 영향

Abstract

구개 편도와 아데노이드 비대는 소아 이비인후과 영역에서 큰 비중을 차지하며 근접 장기에 염증이 확대될 뿐 아니라 소아의 지능 발달과 학업에도 영향을 미치게 된다. 또한 만성 부비동염은 이비인후과 질환중에서 발생 빈도가 높은 질환이다. 소아의 만성 부비동염의 국소적 요인중에서 구개 편도와 아데노이드 비대가 큰 영향을 미치므로 이에 저자는 구개 편도 및 아데노이드 비대의 정도와 부비동과의 관계를 연구하여 다음과 같은 결론을 얻었다. 1. 조사 대상 61 예중 성별 분포는 남아가 40 예(65.6%). 여아가 21 예(34.4%)로 약 2:1의 비율이었고, 연령별 분포는 6∼7세가 19 예(31.1%)로 가장 많았다. 2. 주 임상 증상은 상기도 감염이 31예(39.7%)를 차지하여 가장 많았고 병력 기간은 61 예중 1년 이내가 28예(45.9%)로 가장 많았으며 3년 이상이 19예(31.2%)이었다. 3. 월별 초진 환자는 1월과 7월이 많았고 수술 환자는 8월이 많았다. 4. 조사 대상 61예중 부비동 X-선 검사상 만성 상악동염 소견을 보인 경우는 37예(60.7%), 만성 사골동염 소견을 보인 경우는 13예(21.3%), 만성전두동염 소견을 보인 경우는 18예(29.5%)이었다. 5. 구개 편도 및 아데노이드 비대가 심할수록 만성 상악동염, 만성 사골동염, 만성 전두동염의 이환율이 높았다. 이상의 성적을 종합하여 구개 편도 및 아데노이드 비대가 심할수록 만성 부비동염의 이환율이 높았음이 관찰되었다.;Hyperplasia of palatine tonsils and adenoid begins early in life and may continue until the age of 10 to 12 years when active development usually ceases and atropic changes occur. Hyperplastic tonsillitis and adenoiditis are chracterized by mechanical obstruction of pharynx; nasopharyneal air way and Eustachian tube. Of practial as well as pathologic significance are the recurrent periods of deafness that follow the obstruction of the Eustachian tubes. Initially only retraction of the tympanic membrane is noted, but this, if unrelieved, is followed by serous or purulent otitis media. Especially hperplasia of adenoid may be the offending factors causing the apparent chronic rhinitis and paranasal sinusitis in children due to adenoidal obstruction of the nasal air way. A study was revealed the rate of Paranasal Sinusitis by hyperplasia of palatine tonsils and Adenoid in 61 cases who received adenotonsillectomy during the period from Mar. 1982 to Feb. 1983 in Dept. of Otolaryngology, Ewha Womans University Hospital. The results were as followed: 1. The sex distribution, 41 cases (65.5%) were males and 21 cases (34.4%) were females, and the ratic between male and female was about 2:1 The highest incidence of age distribution was 6-7 years old (31.1%). 2. Chief complaints of patients were frequent U.R.I symtoms (39.7%), nasal stuffiness (32.1%), and purulent rhinorrhea (16.7%) in order. 3. Patient's visit to hospital was most highest in Jan. & July. Operation was most frequently performed in August. 4. The 61 cases of P.N.S. x-.ray were reported maxillary sinusitis in 37 cases (60.7%), ethmoid sinusitis in 13 cases (21.3%) frontal sinusitis in 18 cases (29.5%). 5. In the relationship of Palatine tonsils and adenoid to P.N.S., the rate of chronic maxillary sinusitis, chronic ethmoid sinusitis and chronic frontal sinusitis became in crease in concordance with the degree of hypertrophy of Palatine tonsils and Adenoid. The results reported here revealed that the more hypertrophy of palatine tonsils and Adenoid, the higher rate of paranasal senusitis.
