dc.description.abstractSince the protection of neuronal damage in hypoxic-ischemic lesion in central nervous system is very important to reduce the complications, there have been continuous works on mechanism of cellular damage as well as on prevention with various substances and methods. It has been well documented that calcium ion performs the major role in neuronal degeneration in cerebrovascular diseseases and other degenerative diseases and that 1,25-dihydroxyvitamin D3 (D3), active metabolite of vitamin D, has the dose-dependent protective effects in these diseases. This study was peformed to examine effects of D3 against delayed neuronal damage of hippocampus induced by 10min ligation of bilateral common carotid arteries in mongolian gerbil. Mature mongolian gerbils were injected (ip) with either 0.8 ㎍/㎏/day (group 2, n=19) for 5 consecutive days prior to artery ligation or 1.0 ㎍/kg/day (group 3, n=7) for 8 consecutive days. Control group (group 1, n=18) was injected saline for 5 days. Neuronal damage in group 2 gerbils was evaluated microscopically in subfields of hippocampus at 1, 3, and 7 days after the onset of reperfusion and compared with those of saline-injected controls. Histological examination of group 3 gerbils was peformed at 24 hours after the onset of repefusion. Immunostaining for glial cell line-derived neurotrophic factor (GDNF), basic fibroblast growth factor (bFGF) and platelet-derived neurotrophic factor (PDNF) as well as total blood calcium level and free calcium ion level determination were performed in D3 injected and saline injected gerbils. Group 2 showed highly significant attenuation of neuronal damage in lateral CAI region at 7 days after reperfusion (p=0.011). The neurons in subiculum-CAI region did not show attenuation of damage at any time. Group 3 showed unilateral or bilateral hemispheric infarcts instead of selective neuronal damage of CAI region, which was consistent with previously reported dose-dependent effects of D3. This kind of dose-dependent effects of D3 suggests the importance of D3dose determination in clinical application. Also noted were highly significant enhancement of bFGF expression far beyond the baseline levels and increased total serum calcium and calcium ion levels in D3 injected gerbils. Although the mechanism(s) of neuroprotection by D3 remains unclear, D3 may facilitate reduction of ischemia-induced oxidative stress via activation of neurotrophic factors, including GDNF and bFGF.;중추신경계의 퇴행성질환인 알쯔하이머병이나 파킨슨병에서 비타민 D 대사장애가 동반되는 것이 對혀졌고, 비타민 D가 생물학적으로 활성화된 물질인 1,25-dihydroxy-vitamin D3(이하D3)를 복강내로 투여하면 글루탐산과 칼슘이온이 관여하는 여러 종류의 병변에서 세포의 변성을 억제하고 임상증상을 호전시키는 것이 보고되었다. 본 연구는 성숙 gerbil에서 10분간 양측 총경동맥을 차단한 후 재관류시켜서 유발한 해마의 지연성 신경세포손상 모델에서 D3의 보호효과를 보기 위하여 시행하였다. 연구방법은 성숙 gerbil을 세 군으로 나누어 제 1군(n=18)은 대조군으로 식염수(154mM NaCl, sigma, 1㎖/㎏/day)를 5일간 투여하였으며, 제 2군(n=19)은 D3 (Calcitriol, Abottlab) 0.8㎍/㎏/day (2㎍/㎖ × 0.4㎖/㎏/day)를 양측 경동맥을 결찰하기 4일전부터 5일간 복강내 투여하였고, 제 3군(n=7)에서는 D3 1.0㎍/㎏/day (2㎍/㎖ × 0.5㎖/㎏/day)를 동맥결찰 7일전부터 8일간 투여하였다. 세 군 모두 양측 총경동맥을 10분간 결찰한 후에 채관류시켜 일과성 전뇌 허혈을 유발하였다. 제 1군과 제 2군은 각각 재관류 후 1, 3, 7일에 해마의 subiculum-CAI, lateral CAI, CA2-2 부위에서 신경세포의 손상 정도를 평가하여 비교하였다. 제 3군은 재관류 24시간 후에 조직검사를 시행하였다. DS 투여에 따른 혈청내 칼슘농도의 변화를 보기 위하여 성숙 gerbil을 두 군으로 나누어 대조군(n=6)은 식염수를 투여하였고, 실험군은 D3 0.8㎍/㎏/day (2㎍/㎖ × 0.4㎖/㎏/day)를 5일간 연속 복강내로 투여한 후에 혈청 내 칼슘농도를 비교하였다. 또한 D3가 신경향성 물질의 분비에 미치는 영향을 보기 위하여 glial cell line-derived neurotrophic factor (GDNF), basic fibroblast growth factors (bFGF), platelet-derived neurotrophic factor (PDGF)에 대한 항체로 면역조직화학검사를 시행하여 변화를 관찰하였다. 연구결과는 다음과 같다. 1) 재관류 제 1일 및 제 3일에는 subiculum-CA1과 CA2-3에 있는 신경세포가 주로 파괴되었고, 제 2군과 대조군 사이에 유의한 차이가 없었다. 재관류 제 7일에 대조군에서 CA1 전부위에서 신경세포가 파괴된 반면, 제 2군에서는 lateral CA1 부위의 신경세포가 변성을 보이지 않아서 통계상 유의한 차이를 보였다(p=0.011). 이런 소견으로 D3 복강내 투여가 해마의 lateral CA1에 있는 추체세포에서 지연성 신경세포손상의 발생을 억제하는 것을 알 수 있었다. 2) D3 1.0㎍/kg/day (2㎍/㎖ × 0.5㎖/kg/day)를 8일간 투여한 제 3군에서는 총 7마리 중 6마리에서 한 쪽 혹은 양쪽 대뇌반구를 침범하는 광범위한 허혈성 경색을 보여서 D3의 투여량이 많으면 더 심한 병변이 발생하는 것을 알 수 있었다. 3) 5일간 D3를 투여한 실험군은 식염수를 투여한 대조군에 비해 현저하게 높은 총 칼슘치및 자유 칼슘치(Ca^(2+))를 보였다(총 칼슘치 D3: 15.3±1.4 ㎎/㎗, n=6 vs saline: 10.2±0.7㎎/㎗, n=7, Wilcoxon 2-sample test, t-test approx. significance = 0.0125; 자유 칼슘치 D3: 1.6±0.2 mmol/L, n=6 vs saline: 1.2±0.2 mmol/L, n=7, Wilcoxon 2-sample test,1-test approx. significance 1 0.0358). 4) 면역조직화학검사에서 D3 투여군이 대조군에 비해 심하게 증가된 bFGF 발현을 보였다. 양성반응은 별세포의 세포질과 신경망에서 뚜렷하였다. 이상의 소견으로 D3의 복강내 투여가 해마의 추체세포에서 지연성 신경세포손상이 진행되는 것을 억제하는 작용이 있는 것을 확인한 것은 전뇌 허혈성 병변에서 신경세포손상을 억제하기 위한 치료에 D3를 사용할 수 있는 가능성을 제시한다. 그러나 D3가 농도에 따라서 보호작용을 할 수도 있고 오히려 병변을 더욱 악화시킬 수도 있으므로 적당한 투여량을 결정하는 것이 매우 중요하다. 신경세포의 변성을 억제하는 작용이 있는 것으로 알려진 신경향성 물질의 분비가 B3에 의해 증가되는 소견은 D3의 신경보호효과의 일부가 신경향성 물질에 의할 가능성을 의미한다. 신경향성 물질은 뇌혈관장벽을 통과하지 못하므로 국소적인 투여방법을 이용해야하는 어려움이 있는데 비해, D3는 복강내 투여로 신경보호효과를 유도할 수 있어 임상적으로 훨씬 쉽게 사용할 수 있는 장점이 있다. 앞으로 D3의 정확한 작용기전을 밝히고 임상적으로 적용하기 위해서 더 많은 연구가 진행되어야 할 것으로 생각한다.-
