폐렴구균에 의한 토끼 상악동염 유발시 점막에 대한 기계적 손상이 비용 형성에 미치는 영향

Abstract

비용은 비과영역의 임상에서 흔히 접하게 되는 병변으로 이에 대한 정확한 병인 및 정의는 아직도 확실하지 않으나 병인에 대한 가설 중 상피파열설(epithelial rupture theory)이 현재로서는 가장 널리 받아들여지고 있다. 임상적으로 비용이 중요한 이유는 비용에 대한 치료가 아직 완전하지 않다는 데에 있으며, 따라서 많은 연구가 진행되어오고 있다. 최근 비용이 유발된 동물모델에 대한 연구결과가 일부 보고되어왔지만 그 비용의 양적 차이를 비교한 연구결과는 거의 없다. 본 연구의 목적은, 토끼 상악동에 폐렴구균을 주입하여 상악동염을 유발시킬 때 상피파열설에 근거하여 점막에 가한 기계적 손상 유무가 상악동내 비용 및 비용모양 점막 형성에 미치는 영향을 반정량적으로 분석하고 발생율을 비교하므로써, 향후 비용의 연구에 보다 적합한 동물모델을 개발하고자 하는 것이다. 10마리 정상 토끼(Pasturella-free New Zealand white rabbit)의 양측 상악동 자연공을 폐쇄한 후, 우측 상악동(대조군) 점막은 솔질하지 않고 좌측(실험군)은 솔질한 후 각각 폐렴구균을 주입하여 상악동염을 유발시켰다. 유발 4주후 상악동을 재개방한 후 거시적 비용 및 광학현미경 관찰상 비용과 비용모양 점막에 대하여 반정량적으로 분석한 후 두 군을 비교하였다. 상악동 1개당 평균 거시적 비용수는 대조군(0.1개)보다 실험군(1.1개)에서 의미있게 많았고 각각 10% 및 60%의 상악동에서 비용이 유발되었다. 광학현미경관찰에서도, 대조군보다 실험군에서 비용 및 비용모양 점막이 의미 있게 더 많이 유발되었다. 즉, 폐렴구균을 사용하여 토끼에 실험적으로 상악동염을 유발할 때 그 점막에 대하여 기계적 손상을 준 경우가 주지 않은 경우보다 비용 및 비용모양 점막을 의미있게 더 많이 형성시켰다. 결론적으로, 폐렴구균을 사용하여 토끼 비용모델을 제작할 때 점막에 솔질과 같은 가벼운 기계적 손상을 주는 것이 비용 유발에 더 효과적이다.;Sinonasal polyps are often encountered in clinical bases. However, their pathogenesis and definition remains uncertain. At present, the epithelial rupture theory is more widely accepted than the other theories of pathogenesis. One of the reasons why polyps are clinically important is that there has been no definitely curable way in their treatment until now. To elucidate them, a great number of studies on polyps have been or are being made. Although in some studies it is possible to deal with human beings as materials, other studies need animals as materials in ethical respects. Therefore, it is necessary to develop animal models of polyps. A few studies have recently been reported on polyps in animal models. However, they seldom provided with the comparative yield on polyp formation between various conditions. The aim of this study is to develop an animal model of polyps which is more suitable for further polyp studies by semiquantitatively analyzing the effect of mechnical trauma to the sinus mucosa, based upon the epithelial rupture theory, on formation of polyps and polypoid mucosa in induction of maxillary sinusitis in rabbits by Srreptococcus pneumoniae and comparing the prevalences of polyps under the different conditions. For induction of maxillary sinusitis, the natural ostia of both sinuses were occluded in 10 Pasturella-free New Zealand white rabbits. Both sinuses were inoculated with S. pneumoniae, without brushing the mucosa in the right maxillary sinuses (control group) and with brushing in the left sinuses (experimental group). After four weeks, all sinuses were re-opened. Macroscopic polyp and light microscopic polyps and/or polypoid mucosa were semiquantitatively analyzed and each prevalence was compared between two groups. The number of macroscopic polyps per sinus was significantly larger in experimental group (1.1 polyps) than that in control group (0.1 polyps) and polyps were found in 60% and 10% of the maxillary sinuses, respectively. Microscopic examination revealed significantly more polyps and or polypoid mucosa in experimental group than in control group. The results of this study suggest that mechanically traumatizing the sinus mucosa, such as brushing, is more helpful to polyp formation than not doing when a rabbit model of polyps is to be induced by S. pneumoniae.
