가토의 뇌동맥류 모형에서 모동맥 및 동맥류 협자에 따른 동맥류 내압의 변화

Abstract

Temporary clipping on parent artery is widely accepted useful method to prevent intraoperative aneurysmal rupture, dissect the aneurysm safely, and obtain the clear operation field during intraoperative aneyrysm rupture. However, the exact role and adequate the technique of the temporary clipping has yet to be determined. The author performed an experimental study to investigate the effect of temporary clipping on intra-aneurysmal pressure. The experimental aneurysms, using side-to-side anastomosis between common carotid artery and the jugular vein, were made in 24 rabbits. The intra-aneurysmal pressure was monitored through a catheter inserted in the aneurysm. The intra-aneurysmal pressure was significantly deceased to 31.8±4.39mmHg after temporary clipping on proximal common carotid artery (p<0.05 on Kruskal-Wallis Oneway ANOVA on ranks). On the contrary, intra-aneurysmal pressure was increased to 73.3±4.39mmHg after distal parent temporary clipping. In case of the temporary clipping on both proximal and distal parent artery, the intra-aneurysmal pressure was markedly increased to 81.0±11.7mmHg (p=0.0036 on t-test). In conclusion, the temporary clipping on the parent artery should be performed only on the proximal site of the aneurysm. Temporary clipping on both proximal and distal site would be rather dangerous since it evokes sharp increase in intra-aneurysmal pressure during the clipping of aneurysm itself.;모동맥의 일시 협자는 수술중 뇌 동맥류 파열을 예방하고자 하는 노력중 하나로 현재 보편적으로 사용되고 있는 유용한 방법이다. 모동맥의 일시 협자는 동맥류 내로 유입되는 혈류를 차단시켜 동맥류 파열을 방지하고 안전하게 박리를 시행할 수 있게 하며 수술중 동맥류 파열을 방지하고 안전하게 박리를 시행할 수 있게 하며 수술중 동맥류 파열이 일어나도 시야를 쉽게 확보할 수 있게 한다. 일시 협자가 동맥류 내압에 어떠한 영향을 미치는지 알고자 24 마리의 가토의 총 경 동맥에 외 경 정맥으로 만든 정맥 낭을 통합하여 동맥류를 만든 후 동맥류 내에 도관을 삽입하고 혈압 측정기에 연결하여 동맥류 내압을 지속적으로 기록하면서 모동맥인 총 경 동맥에 일시 협자와 동맥류 협자를 시행하여 보았다. 그 결과 근위부 모동맥에 일시 협자를 한 때 만 동맥류내압이 31.8 ± 4.39 ㎜Hg 로 상승하였고 모동맥의 근위부와 원위부 혈관 모두에 일시 협자를 시행하고 동맥류 협자를 시도한 결과 1337.±10.43 ㎜Hg의 동맥류 내압을 보여 근위부 협자 보다 원위부 협자를 먼저 시행하였을 때 유의하게 동맥류 내압이 증가하였다 (p=0.0036 on t-test). 이것으로 미루어 볼 때 동맥류의 일시 협자는 동맥류의 근위부 모동맥에서만 시행되어야하며, 동맥류의 근위부와 원위부 양쪽에 일시 협자를 시행하는 것은 동맥류 내압의 심각한 상승을 야기하여 오히려 더 위험할 수 있다.
