신생아와 영아의 혈장내 Fibronectin농도 변화에 대한 연구

Abstract

미숙아를 포함하는 신생아의 면역계의 생리적 미숙은 감염 질환에 대한 저항력의 약화를 초래하는 원인중의 하나이다. 이중 fibronectin은 인체의 감염에 대한 저항에 중요한 역할을 하는 비특이적 면역물질 중의 하나이며 이 fibronectin의 혈장내 농도는 다른 면역 물질들과 마찬가지로 신생아와 영아에서 성인과 다른 소견을 보이며 감염을 비롯한 여러 상황에서 가변적임이 알려져 있다. 본 연구는 이화대학교 동대문 병원 신생아실에 입원한 37주 미만의 감염, 질식 및 호흡곤란증등의 질환이 없는 미숙아 16명, 만삭아 90명 및 CRP 음성의 건강한 영아 69명을 대상으로 혈장 fibronectin농도를 측정하였고 이를 성인에서의 fibronectin 농도와 비교해 보았다. 또한 각종 세균 감염 혹은 염증성 반응의 증거로 C-반응 단백(C-reactive protein, CRP)을 검사하여 양성으로 나온 영아 21명에서 혈장 fibronectin 농도를 검사하여 CRP 양성군과 음성군에서 fibronectin 혈장 농도를 비교하여 다음과 같은 결과를 얻었다. 1. Fibronectin 농도는 미숙아 16명에서 20.06±5.12㎎/㎗, 정상 만삭아 90명에서 22.78±5.84㎎/㎗, 건강한 영아 69명에서 26.50±5.44㎎/㎗, 정상 성인 19명에서 38.06±9.17㎎/㎗로 미숙아와 정상 신생아간에는 유의한 차이는 관찰되지 않았으나 미숙아에 대한 영아 및 성인, 만삭아에 대한 영아 및 성인 및 영아에 대한 성인간에는 유의한 차이가 있었다. 2. 신생아중 37주미만의 미숙아의 혈장 fibronectin농도는 평균 20.06±5.12㎎/㎗이였고 37주 이상의 만삭아 90명에서는 평균 27.78±5.84㎎/㎗이었으며 두 군의 통계적인 유의성은 없었다. 출생 체중 2.5㎏이하의 저출생아 평균 농도는 19.53±3.58㎎/㎗, 2.5㎏이상 신생아에서는 22.88±5.98㎎/㎗로 2.5㎏이상의 신생아에서의 fibronectin 농도가 높았다(p<0.05). 3. 영아 90명 중에서 CRP 음성이며 건강한 영아 69명의 혈장 fibronectin 농도는 26.50±5.44㎎/㎗, CRP 양성인 영아 21명에서는 19.88±5.05㎎/㎗로 CRP 양성군의 혈장 fibronectin 치가 낮았다(p<0.05). 결론적으로 미숙아, 만삭아, 영아와 성인에서의 혈장 fibronectin의 농도를 측정하였을때 혈장 fibronectin농도는 각 연령군에서 차이를 보여 미숙아, 만삭아, 영아에서 성인보다 낮은 농도를 보였고 영아에서는 CRP양성군에서 음성군에 비해 낮은 혈장 fibronectin농도를 보였다.;Plasma fibronectin has important role in the inflammatory response as non-specific opsonin and host defense. Plasma fibronectin level is different according to age and presence of infection. Sixteen preterm, ninety full term babies and ninety infants were studied who admitted to Ewha University Tongdaemun Hospital from 1994 March to 1996 April. The concentration of plasma fibronectin was studied in each groups by turbidometric immunoassay and CRP was checked in 90 infants by flourescene polarization immunoassay. The result were as follows. 1. The concentration of plasms fibronectin was 20.06±5.12mg/d1 in preform babies, 22.78±5.84mg/d1 in term babies, 24,76±5,99mg/41 in 69 infants, and 38.06±9.17mg/dl in adults, The level of plasma fibronectin was different in each age group(p<0.05). 2. The concentration of plasma fibronectin was 20.06±5.12mg/d1 in preterm babies and 22.78±5,84mg/d1 in term babies(p<0.05). And it was 19.53±3.58 mg/dl in neonates with less than 2.5kg of birth weight and 22.88±5.83mg/d1 in neonates with more than 2.5kg of birth weight(p<0.05). 3. The concentration of plasma fibronectin was 26.50±5.44mg/d1 in CRP negative infants and 19.88±5.05mg/d1 in CRP positive infants and it was significantly different between two groups. This result shows that the concentration of plasma fibronectin Is different in different ages groups, birth weight and increased CRP concentration.
