Abstract
신장은 갑상선 호르몬의 대사와 배설에 중요한 역할을 담당하는 기관이므로 만성 신부전 환자에서 갑상선 호르몬 변화는 예상되는 문제이며 이에 대한 많은 연구가 진행 되었으나 아직 일치된 결과와 만족할만한 기전의 규명이 이루어지지 못하였다. 특히, 만성 신부전 환자의 장기간 생존률이 증가되고 신이식이 중요한 신대체 요법으로 확립 되면서 장기간의 혈액투석 및 신이식이 감상선 호르몬 변화에 미치는 영향을 평가할 필요가 있으리라 생각된다. 저자는 1987년 5월이후 중앙 길병원 내과에서 치료 중인 만성 신부전환자 중 투석을 받기전 외래 추적 증인 환자 (Ⅱ군) 22명, 규칙적으로 혈액 투석 중인 환자(Ⅲ군) 85명, 신이식을 받은 환자(Ⅳ군) 10명 및 대조군(Ⅰ군) 41명을 대상으로 혈청 T_(3),T_(4), FT_(3), FT_(4), rT_(3)및 TSH의 기저치를 측정하고 이들 중 Ⅰ군 8명, Ⅱ군 10명, Ⅲ군 29명, Ⅳ군 10명에서 TRH 자극검사를 시행하여 다음과 같은 결과를 얻었다. 1. 혈정 T_(3)치는 Ⅰ군에 비해 Ⅱ군 및 Ⅲ군에서 유의하게 감소되었으나 (각각 p<0.01, p<0.001) Ⅱ군과 Ⅲ군 사이에는 유의한 차이가 없었고, 혈청 T_(4)치는 Ⅰ군과 Ⅱ군 사이에서 유의한 차이가 없었으나 Ⅰ군 및 Ⅱ군에 비해 Ⅲ군에서 유의하게 감소되었다(각각 p<0.001, p<0.05). 2. 혈청 FT_(3) 및 FT_(4)는 Ⅰ군에 비해 Ⅱ를 및 Ⅲ군에서 유의하게 감소되었으나(p<0.00l) Ⅱ군과 Ⅲ군 사이에는 유의한 차이가 없었고, 혈청 rT_(3)치는 Ⅰ군, Ⅱ군 및 Ⅲ군 사이에서 통계적으로 유의한 차이가 없었다. 3. 혈청 TSH 기저치는 Ⅰ군, Ⅱ군 및 Ⅲ군 사이에서 유의한 차이가 없었으나 TRH 자극 후혈청 TSH의 반응은 Ⅰ군에 비해 Ⅱ군 및 Ⅲ군에서 최대치에 도달하는 시간이 지연되었고(p<0.005) 기저치에 대한 최대치의 비율도 감소되었다(p<0.05). 4. 신이식을 받은 환자(Ⅳ군)에서 혈청 T_(3) 및 FT_(3)치는 Ⅰ군에 비해 유의하게 감소되었으나(각각 p<0.05, p<0.01) Ⅱ군 및 Ⅲ군과는 유의한 차이가 없었고, 혈청 T_(4)치는 Ⅰ군, Ⅱ군 및 Ⅲ군에 비해 유의한 차이가 없었으나 Ⅱ군 및 Ⅲ군에 비해 유의하게 증가되었다(p<0.001). TRH 자극 후 혈정 TSH가 최대치에 도달하는 시간은 Ⅲ군에 비해 유의하게 개선되었고(p<0.05) 기저치에 대한 최대치의 비율은 Ⅰ군, Ⅱ군 및 Ⅲ군에 비해 유의하게 감소되었다(p<0.01). 5. 혈액투석 중인 만성 신부전 환자(Ⅲ군)에서 혈정 BUN 및 creatinine치와 FT_(4)치는 역상관관계(p<0.001)가 있었고 혈청 creatinine치와 T_(4)치도 역상관관계(p<0.05)가 있었으나, 혈청 T_(3) 및 FT_(3)치와 BUN 및 creatinine치는 무관하였다. 6. 혈액투석 중인 만성 신부전 환자(Ⅲ군)에서 투석기간이 길수록 혈청 FT_(4)치는 감소되었고(p<0.005), 혈청 T_(3)및 FT_(3)치는 증가되었다(각각 p<0.05, p<0.01).;The changes of thyroid hormones are expected in patients with chronic renal failure(CRF) because the kidney plays an important role in metabolism and excretion of thyroid hormones. In spite of many studies about this issue there have not been full consensus about the nature and mechanism of thyroid hormones changes in patients with CRF. To evaluate the effects of long-term hemodialysis and kidney transplantation on the changes of thyroid hormones, basal levels of serum T_(3), T_(4), FT_(3), FT_(4), rT_(3), TSH and TSH response after TRH stimulation in patients with CRF who were managed at the-department of internal medicine Chungang Gil Hospital were examined. Study population composed of 41 cases of normal controls(Group Ⅰ), 22 cases of prehemodialysis CRF patients(Group Ⅱ), 85 cases on maintenance hemodialysis (Group Ⅲ), and 10 cases who received kidney transplantation(Group Ⅳ). Results were as follows; 1. Basal serum T_(3) level in Group Ⅱ and Ⅲ was decreased significantly compared to Group Ⅰ(p<0.0.1, p<0.001 respectively), but there was no difference between Group Ⅱ and Ⅲ. Basal serum T_(4) level in Group Ⅲ was decreased significantly compared to Group Ⅰ and Ⅱ(p<0.001, p<0.05 respectively), but there was no difference between Group Ⅰ and Ⅱ. 2. Serum FT_(3) and FT_(4) levels in Group Ⅱ and Ⅲ were decreased significantly compared to Group I(p<0.001), but showed no difference between Group Ⅱ and Ⅲ. There was no difference in serum rT_(3) level among the groups. 3. There was no difference in serum basal TSH level among the groups, but in Group Ⅲ TSH response after TRH stimulation was delayed(p<0.005) and blunted(p<0.05) compared to Group Ⅰ. 4. In Group Ⅳ serum T_(3) and FT_(3) levels were decreased significantly compared to Group l(p<0.05, p<0.01 respectively), but were not different from Group It and Ⅲ. Serum T_(4) level was not different from other groups. Serum FT_(4) level was higher than Group Ⅱ and Ⅲ(p<0.001) but was. not different from Group Ⅰ. The time to reach maximum TSH level after TRH stimulation was shortened compared to Group Ⅲ (p<0.05) and was not different from Group Ⅰ. Ratio of maximum to basal TSH level was decreased compared to Group Ⅰ, Ⅱ, and Ⅲ(p<0.01). 5. In Group Ⅲ serum BUN and creatinine levels correlated negatively with serum FT_(4) level(p<0.001), but serum BUN and creatinine levels did not correlate with serum T_(3) and FT_(3). 6. Duration of hemodialysis correlated negatively with serum FT_(4) level(p<0.005), and correlated positively with serum T_(3) and FT_(3)(p<0.05, p<0.01 respectively). Above findings showed that hemodialysis and kidney transplantation have various effects on the changes of thyroid hormones and TRH stimulation test. Further studies are needed to elucidate the mechanism and biological or clinical significance of thyroid hormonal changes in patients with CRF.
