Halothane 마취와 균형마취에서 SGOT와 SGPT 및 혈당의 변화

Abstract

The effect of halothane and balanced anesthesia on hepatic function and blood glucose were studied in forty patients. The results were as follows 1. There was no statically significant difference in the changes of SGOT between halothane and balanced anesthesia during postoperative period. 2. There was no statically significant difference in the changes of SGPT between halothane and balanced anesthesia during postoperative period. 3. Blood glucose was increased during postoperative period in halothane and balanced anesthesia. But, there was no statically significant difference in halothane and balanced anesthesia.;1956년 Suck1ing에 의해 halothane이 합성된 후 비교적 안전한 홉입마취제로 널리 사용되어 왔으나 halothane이 간기능 저하를 일으킬수 있다는 보고가 있다. 수술후 간기능 장애를 초래하는 요인으로서는 만성 간질환, 바이러스성 감염, 패혈증, 심한화상, 영양부족, 저산소증, 과탄소증, 저혈압과 수술조작 등이 있다. 이러한 많은 요인이 있음에도 불구하고 halothane이 수술후 간기능 장애의 주원인인것 처럼 생각되는 것에 대하여 아직도 논란이 많다. 이에 저자는 미국 마취과학회의 환자상태분류법에 따른 ASA(American Society of Anesthesiologists) class 1에 해당되는 건강한 40례를 무작위로 선택하여 halothane 마취군(20례)과 균형마취군(20례)으로 나누어 마취유도전과 수술후 serum glutamic oxalacetic transaminase (SGOT)와 serum glutamic pyruvic transaminase (SGPT) 및 혈당을 측정하여 다음과 같은 결론을 얻었다. 1. SGOT는 halothane 마취군에서 수술후 제 1 일에 대조치에 비하여 그 변화가 의의있게 증가하였으나 (P<0.01)정상범위에 속하였고 균형마취군에서는 대조치에 비하여 통계학적 의의가 없었다.(P>0.01) 2. SGPT는 halothane마취군에서 수술후 제 1일에 대조치에 비하여 의의있게 증가하였으나 (P<0.01)정상범위에 속하였고 균형 마취군에서는 대조치에 비하여 통계학적 의의가 없었다. (P>0.01) 각 마취군에서 마취제에 따른 변화도 통계학적 의의가 없었다.(P>0.05) 3. 혈당은 양군 모두에서 대조치에 비하여 수술후 시간에 따라 의의있게 증가하였다.
