- 전신성 홍반성 루푸스(이하 루푸스)는 자가항체를 생성하여 다양한 장기에 임상 증상을 일으키는 자가면역 질환으로 15-40세의 젊은 여성에 호발하며 콩팥, 심장, 폐 등의 장기의 손상을 일으킬 수 있다. 루푸스 환자에서 발생하는 특정 장기의 손상이 자가항체와 중요한 연관이 있다는 증거가 제시되고 있으나, 아직까지도 루푸스의 병리기전에 있어서 자가항체의 역할과 생성기전은 거의 밝혀진 바 없다. B-1 B 림프구는 태아기와 같은 생리적인 상태에서 자가항체를 생성하는 독특한 B 림프구의 아형이다. 일부 자가면역 질환의 환자와 동물 모델에서 B-1 B 림프구의 수적 팽창이 관찰되어 자가면역 질환의 병리기전에 관여함을 시사하였으나 아직까지 루푸스 환자의 병리기전에서 B-1 B 림프구의 역할에 대해서는 확실히 밝혀진 바 없다. 본 연구의 목적은 루푸스 환자에서 B-1 B 림프구 분획의 항상성 조절에 이상이 존재하는지를 검증하고자 한다. 7명의 치료 받지 않은 활동성 루푸스 환자의 말초혈액에서 B-1a B 림프구, B-1b B 림프구, B-2 B 림프구 분획을 측정하여 나이와 성별이 일치하는 정상 대조군, 발열 대조군과 비교 분석하여 다음과 같은 결과를 얻었다.
1. B-1a B 림프구 분획은 활동성 루푸스 환자군에서 발열 대조군과 비교하여 수적 증가를 보였다(33.5±15.0% vs 20.1±5.3%, p=0.02).
2. B-2 B 림프구는 활동성 루푸스 환자군에서 발열 대조군과 비교하여 수적 감소를 나타냈다(65.6±15.1% vs 77.9±5.6%, p=0.05).
3. 활동성 루푸스 환자군에서는 정상 대조군과 비교하여 B-1a B 림프구 분획의 증가(33.5±15.0% vs 27.7±10.4%, p=0.95)와 B-2 B 림프구 분획의 감소(65.6±15.1% vs 71.4±9.7%, p=0.08)를 보였으나 통계적으로 유의하지 않았다.
4. 면역억제 치료 후 루푸스 환자의 B-1a B 림프구 분획은 의미있게 감소하였으며 (33.5±15% vs 19.8±8, p=0.04), 반면에 B-2 B 림프구 분획은 의미있게 증가하였다(65.6±15.1 vs 79.5±7.8, p=0.04).
이상의 결과로 루푸스의 활동성이 증가되면 B-1a B 림프구 분획이 팽창되고 상대적으로 B-2 B 림프구 분획이 축소됨이 관찰되어 루푸스 환자에서 B 림프구 분획의 항상성 유지에 이상이 존재함을 알 수 있었고, 면역억제치료가 B 림프구 분획의 항상성 이상을 조절할 수 있음을 보였다.;Backgroud/Aims : B-1 B lymphocytes are unique subset of B cells that produce autoreactive antibodies. Since increased numbers of B-1 B lymphocytes were found in animals and patients with some autoimmune disease, B-1 B lymphocytes were implicated in pathogenesis of autoimmunity. Howerer, the role of B-1 B lymphocytes in the pathogenesis of systemic lupus erythematosus (SLE) is still obscure. The purpose of this study was to define the abnormalities in homeostasis of B-1 B lymphocytes compartments in human SLE.
Method : Peripheral blood was obtained from 7 patients with untreated active SLE patients and same patients at the time of inactive disease after immunosuppressive therapy. The frequencies of CD5+CD45RAint B-1a B lymphocyte, CD5+CD45RAlow B-1b B lymphocyte, and CD5-CD45RAhigh conventional B-2 B lymphocyte populations were analyzed. For control, peripheral blood from 7 healthy adults and 7 patients with infectious fever were utilized.
Results : B-1a B lymphocytes population were found at high frequency in active SLE patients compared to the fever control (33.5±15.0% vs 20.1±5.3%, p=0.01). In contrast, B-2 B lymphocytes were found at lower frequency compared to the fever control (65.6±15.1% vs 77.9±5.6%, p=0.04). The same tendency of increased B-1a B lymphocytes and reduced B-2 B lymphocytes were shown in active SLE compared to the normals, although not reaching statistical significance. No difference in frequency of B-1b B lymphocytes populations was found between active SLE and the controls. Upon immunosuppressive theray, B-1a B lymphocytes were markedly decreased with increase in B-2 B lymphocytes population (p=0.05).
Conclusion : These results indicate that there are abnormalities in B cell compartments with expansion of B-1a B lymphocyte population and contraction of B-2 B lymphocytes associated with the disease activity in patients with SLE.
