원발폐쇄각녹내장에서 레이저홍채절 개술 후 초음파생체현미경을 이용한 전방각 계측치의 변화 분석

Abstract

원발폐쇄각녹내장은 서양인에 비해 아시아인에서 비교적 흔한 형태의 녹내장으로 좁은 전방각이 폐쇄각녹내장의 가장 중요한 요인으로 알려져 있고, 레이저홍채절개술은 현재 급·만성 원발폐쇄각녹내장에서 모두 표준 일차 치료로 널리 이용되고 있으며, 초음파생체현미경(UBM)은 전안부의 구조와 병태생리학적 해석에 있어 고해상력의 영상을 제공하고 미세 계측치의 정량 분석을 가능하게 하여 폐쇄각녹내장을 비롯한 각종 전안부 질환의 진단 및 경과관찰에 중요한 정보를 제공하여 많은 임상연구에 이용되고 있다. 본 연구에서는 UBM을 이용하여 만성 원발폐쇄각녹내장 환자에서 전방각 폐쇄에 관여하는 중요한 형태학적 특징들을 확인하고, 레이저홍채절개술 후 전방각 계측치의 변화를 분석하고자 하였다. 만성 원발폐쇄각녹내장으로 진단받은 환자 중 안과적 수술 및 외상의 과거력이 있거나 180도 이상의 주변홍채앞유착이 있는 경우를 제외한 25명(25안)을 대상으로 레이저홍채절개술 시행 전과 시행 2주 후에 UBM(UBM plus^(2000) system, Paradigm?? Inc.,Utah, USA)을 이용하여 동일 검사자가 전안부 영상촬영 및 최대전방깊이, 전방각, 전방각간 거리, 섬유주-홍채간거리, 홍채두께 등에 대한 계측을 시행하였다. 그 결과 평균 최대전방깊이는 2.16㎜, 전방각은 12.73˚, 전방각간 거리 (AOD_(500))은 0.128㎜, 섬유주-홍채간 거리는 0.101㎜로 이전에 정상 성인을 대상으로 하였던 연구 결과와 비교하여 모두 통계적으로 유의하게 작았으며, 레이저홍채절개술 전·후의 변화를 분석한 결과, 최대전방깊이, 홍채두께는 통계적으로 유의한 변화가 없었고, 전방각과 전방각간 거리(AOD_(500), AOD_(250)), 섬유주-홍채간 거리는 레이저홍채절개술 후에 통계적으로 유의하게 증가하였다. 이상의 연구 결과로 UBM을 이용하여 원발폐쇄각녹내장 환자에서 전방각 폐쇄에 관여하는 중요한 형태학적 특징들을 확인할 수 있었고, 레이저홍채절개술 후의 전방각 형태변화에 따른 치료 결과를 정량적으로 확인할 수 있었다.;Laser iridotomy (LI) is now the standard first-line intervention in primary angle-closure glaucoma (PACG) that is a major form of glaucoma in Asia, and ultrasound biomicroscopy (UBM) is a powerful tool for obtaining precise anterior segment images and measurement of the anterior segment parameters. My purpose was to identify the essential morphologic characteristics concerned with angle closure in chronic PACG and assess the changes in quantified values for angle relationships after LI in them using UBM. Anterior segment imaging and quantitative assessment of angle relationships were obtained in 25 eyes of 25 patients(10 males and 15 females) with chronic PACG using UBM (UBM plus^(2000) system, Paradigm?? Inc.,Utah, USA) before and 2 weeks after LI. The mean anterior chamber depth (ACD), anterior chamber angle (ACA), angle-opening distance (AOD_(500)), and trabecular-iris distance (TID) were 2.16㎜, 12.73˚, 0.128㎜, and 0.101㎜, respectively. Compared to quantified values in normal adults that were previously reported, our results all were significantly smaller. In comparison between before and after LI, ACD and iris thickness (IT) did not change significantly and ACA, AOD_(500), AOD_(250), and TID all significantly increased after LI. Using UBM, we identified the essential morphologic characteristics concerned with angle closure in chronic PACG and quantitatively demonstrated the result of treatment based on the changes of angle relationships after LI.
