건강한 성인에서 혈압에 따른 혈중 adiponectin농도의 비교

Abstract

연구배경 아디포넥틴은 대사증후군이 고혈압의 발병에 영향을 미치는 중요한 연결고리로서의 가능성이 제시되고 있다. 본 연구는 심혈관계 질환의 과거력이 없는 건강한 성인을 대상으로 하여 혈압과 아디포넥틴과의 관련성을 알아보고자 하였다. 방법 일개 대학병원 건강검진센터를 방문한 건강한 성인에서 자가설문지 및 신체검사, 혈액검사소견을 분석한 후 이차성 고혈압, 심근경색, 뇌졸중, 만성 신질환, 만성 간질환, 악성종양의 병력과 검진 당시 급성 염증성 질환이 없는 3113명을 대상으로 정상, 고혈압 전단계군, 고혈압군으로 분류하여 아디포텍틴 수치와의 관련성을 분석하였다. 결과 전체 3113명 중, 정상 혈압 1145명 (37%), 고혈압 전단계 1449명 (46%), 고혈압 519명 (17%)의 분포를 나타내었으며 혈압이 높을수록 연령, 체질량 지수, 허리둘레, 공복 혈당, 혈중 총콜레스테롤, 중성지방, LDL-콜레스테롤의 경우 정상군에 비하여 고혈압전단계군, 고혈압군에서 더 높게 측정되었으며 HDL-콜레스테롤은 더 낮게 측정되었다. (P<0.001) 심혈관 질환과 관련된 변수들을 통제한 다중 선형 회귀 분석에서 로그치환 혈청 아디포넥틴은 수축기 혈압 (β=-3.37, P<0.001) 및 이완기 혈압 (β=-5.154, P<0.001) 과 음의 상관 관계를 보였다. 결론 혈청 아디포넥틴은 건강한 성인에서 혈압과 유의한 연관성이 있었으며 이러한 연관성은 고혈압 전단계에서도 나타났다.;Background Adiponectin may play an important role in the interplay between metabolic changes and hypertension. Our aim was to establish an association between plasma adiponectin and hypertension in healthy adults. Methods Study subjects were 3113 adults older than 20 years who visited the university hospital health care center. Those patients with secondary hypertension, cardiovascular disease or other systemic disease were excluded. They were devided into three groups by blood pressure based on the Joint National Committee (JNC) 7 classification. The association of adiponectin with hypertension were investigated by multiple logistic regression analysis. Results Adiponectin concentrations were significantly lower in prehypertensives compared with those in normotensives, but hypertensives exhibited the lowest adiponectin concentrations of all. Blood pressure had a positive correlation with age, body mass index, waist circumference, fasting glucose, total cholesterol, LDL-cholesterol, triglycerides, HOMA-IR(homeostasis model assessment of insulin resistance), aPWV (aortic pulse wave velocity). Moreover, it had a negative correlation with HDL-cholesterol and adiponectin. A multivariate linear regression analysis adjusted for cardiometabolic factors showed that systolic blood pressure increased by 3.370 mm Hg and diastolic blood pressure increased by 5.154 mm Hg for 1 μg/mL reduction in adiponectin plasma levels and (P<0.001). Conclusion Our findings have shown that hypoadiponectinemia is associated with blood pressure.
