dc.description.abstractBackground Insulin resistance is the strongest predictor of non-insulin-dependent diabetes mellitus and visceral obesity is regarded as a major cause. The purpose of this investigation is to ascertain if waist to thigh ratio can be used as a reliable marker for insulin resistance. Methods The subjects of this cross sectional study were 95 women above 20 years old who visited a university hospital obesity center between December 2009 and June 2010. History taking and anthropometric measurements were conducted by a single trained physician. Insulin resistance is defined as HOMA-IR above 3.04. Results Median age of total participants was 37.43+ 11.45 years. HOMA-IR showed a positive correlation with body weight, BMI, hypertension, waist circumference, waist to thigh ratio, triglyceride and LDL cholesterol while showing a negative correlation with HDL cholesterol by the Spearman’s rank correlation analysis. A simple linear regression analysis showed a positive correlation between waist to thigh ratio and log-transformed HOMA-IR(R2:0.07,P<0.01). Waist to thigh ratio proved to be the most useful marker for the detection of insulin resistance showing AUC 0.70±0.55(P<0.005) by the ROC curve when compared with other traditional index such as BMI, waist circumference and waist to hip ratio. The ROC curve demonstrated that the optimal waist to thigh ratio for discriminating insulin resistance was 1.46 (sensitivity:80.0%, specificity:51.4%)(P<0.001). Conclusion There is a positive association between waist to thigh ratio and HOMA-IR. Waist to thigh ratio is a useful and reliable marker for the detection of insulin resistance.;연구배경 인슐린 저항성은 당뇨병 발생의 가장 강력한 예측 인자로, 내장지방이 주 원인으로 생각되어진다. 이번 연구의 목적은 인슐린 저항성과 허리/허벅지둘레 비와의 연관성을 알아봄으로써 허리/허벅지둘레 비가 당뇨 발병의 예측인자로서 믿을만한 지표인지를 알아보기로 하였다. 방법 서울 시내 일개 대학병원 비만 센터를 내원한 20세 이상의 성인 여성 95명을 대상으로 단면연구를 시행하였고 신체 계측 및 문진은 한 명의 동일한 숙련된 의사가 시행하였다. homeostasis model assessment of insulin resistance(HOMA-IR)는 3.04이상인 경우 인슐린 저항성군으로 정의하였고, 허리/허벅지둘레 비와 인슐린 저항성과의 관계를 분석하였다. 결과 연구 대상자는 평균 37.43+11.45세였고, HOMA-IR은 Spearman 상관분석을 통해 체중, 체질량 지수, 고혈압 병력, 허리둘레, 허리/허벅지둘레 비, 중성지방, 저밀도 지단백과 양의 상관관계를 보였고 고밀도 지단백과 음의 상관관계를 보였다. 단순 선형 회귀분석에서 허리/허벅지둘레 비와 로그치환 HOMA-IR은 양의 상관관계를 보였다.(R2: 0.07, P<0.01) 인슐린 저항성을 진단하는 지표로서 허리/허벅지둘레 비의 가치를 알아보기 위해 시행한 receiver operating characteristic(ROC) curve상 area of under the curve(AUC) 0.70±0.55(P<0.005)로 기존의 지표인 체질량 지수, 허리둘레, 허리/엉덩이둘레 비와 비교해보았을 때 가장 가치 있는 지표로 분석되었으며 인슐린 저항성을 나타내는 최적의 수치는 1.46이었다.(민감도 80.0%, 특이도 51.4%) 결론 허리/허벅지둘레 비와 HOMA-IR 사이에 양의 상관 관계가 있으며 허리/허벅지둘레 비는 인슐린 저항성을 진단하는 지표로 유용하다.-
