배란 예측을 위한 요 LH 검사의 임상적 유용성에 관한 연구

Abstract

불임여성에서 정확한 배란 시기를 아는 일은 무배란에 의한 불임의 원인을 알 수 있을 뿐 아니라 자연 배란 주기에서 성교 시기의 결정, 좀더 적극적인 방법으로 인공수정 시술에 매우 중요한 문제이다. 배란 시기를 알기 위하여 연속적인 초음파 관찰, 혈중 에스트로겐, 황체형성호르몬, 프로제스테론의 측정 및 자궁 내막검사 등이 사용되어 왔으며 그 정확도와 간편성, 반복성이 요구되는 새로운 방법의 연구가 진행되어 왔다. 따라서 이 노력의 일환으로 요중 황체 형성 호르몬을 이용하여 배란시기를 예측하려는 시도가 있었으며 이 방법은 반복적인 채혈과 검사에 소요되는 시간을 단축시켰다. 본 연구는 1991년 8월 1일부터 1992년 9월 30일까지 이화대학 병원산부인과 불임상담실에 내원한 15명의 환자를 대상으로 15주기의 자연 배란주기를 관찰, 요 황체형성 호르몬(LH)검사를 이용하여 배란 시기를 예측하려는 임상 실험으로 다음과 같은 결론을 얻었다. 1. 요 LH 검사법은 LH의 짧은 반감기의 성질을 이용한 것으로 본 연구에서 사용된 OvuQuick Self Test의 경우 요중 LH치가 40mIU/m1인 경우 양성을 보인다. 2. 요 LH 양성 반응은 질식 초음파상 배란 18-71시간(평균 28.8시간)전에 볼수 있었다. 3. 혈중 에스트로겐(E_(2))치는 요 LH가 9시에 양성을 보인 경우 Day -1에 최대치를, 요 LH가 오후 3시에 양성을 보인 경우 Day0에 최대치를 보였다.(p<0.001) 4. 요 LH 검사법의 민감도는 87%, 특이도는 50%였다. 이는 소집단의 결과에 의한 것으로 사료되며 집단 수가 많아짐에 따라 증가하리라 예상된다. 5. 질식 초음파에서 배란의 증거가 있었으나 요 LH검사상 음성을 보인 경우가 1 예로 7.1%를 보였으며 이에 대한 원인은 보다 연구되어야 할 것으로 사료된다. 6. 따라서 배란시기의 예측을 위한 방법으로는 기초체온 측정법, 경부 점액관찰법, 질식 초음파관찰법 및 요 LH검사법이 병용될 수 있으며 필요한 경우 여기에 혈청학적 검사가 이용될 수 있다. ;The prediction of ovulation time is very important for detection of anovulatory menstrual cycle or for timing of coitus or other procedure in spontaneous cycle of infertile women. Many different parameters have been used to predict ovulation including basal body temperature, cervical mucus, circulating levels of luteinizing hormone (LH), estradiol(E_(2)), and progesterone(P_(4)), and ultrasound profile(US). Since radioimmunoassay of serum LH, E_(2), P_(4)take too long for repeat measurement on the same day, we evaluated the possibility of adapting a rapid and fully automated assay of serum LH for use with urine samples. The data to be presented are base on 15 cycles in 15 infertile patients with regualr mestrual period in natural cycle and their serum E_(2)levels, P_(4), levels, US and twice daily urinary LH levels were determined(OVuQuick Self Test) from day 10 of their menstrual cycle. These patients were followed until the follicular rupture were noted by US. The results in this study were as follows. 1. Urine LH test (OVuQuick Self Test) was used the property of short half life of serum LH and was showed positive test in urine LH concentration over 40 mIU/ml. 2. The positive test was followed by follicular rupture 18-71 hour (average 28.8 hour) later. 3. The patient with urine LH positive at 9 AM showed a significant elevation of serum E_(2)leves on Day-1, whereas the patient with urine LH positive at 3 PM showed a significcant elevation of serume E_(2)level on Day 0 (P<0.001). 4. The sensitivity and specificity of OVuQuick Self Test was 87%, and 50 % respectively, and specificity would be improved if the sample size were large. 5. The patient with urine LH negative with follicular rupture by US was noted 1 case (7.1%) and needed further study for urine LH test. 6. We conclude that urine LH test was a reliable, rapid and readily acceptable method for the prediction of ovulation with basal body temperature, cervical mucus test, and ultrasoud profile. If need, we added further study for more accurate data same as serum E_(2), P_(4), and LH levels.
