후방견인시 후크의 수직적 위치에 따른 전치부 이동양상

Abstract

본 연구는 동적인 치아이동평가가 가능한 칼로리픽머신과 치아모델을 이용하여 공간폐쇄호선상에서 후크의 수직적 위치에 따른 치아의 이동양상에 대해 실험하고 평가하였다. 실험은 상악 제2소구치의 근심측 호선 상방 10mm에 식립한 교정용 미니임플란트에 동일한 힘을 적용하여 실행하였는데, 후크를 치은 방향으로 부착한 gingival hook군과 후크를 교합면 방향으로 부착한 occlusal hook군 두 군으로 나누었다. 실험 전, 실험 후의 모델을 CT촬영한 후 V-Works 4.0™를 이용하여 3차원 영상으로 변환 후 Rapidform 2004™를 이용하여 중첩 및 개개 치아의 이동거리를 계측하였다. 각각의 실험을 통해 다음과 같은 결론을 얻었다. 1. 상악 6 전치의 압하량은 gingival hook군에서보다 occlusal hook군에서 더 많았다(P<0.05). 2. 상악 6전치의 후방견인량은 occlusal hook군에서보다 gingival hook군에서 더 많았다(P<0.05). 이상의 연구를 통하여 교정용 미니임플란트의 위치가 비교적 상방에 위치할 경우 치은 방향의 후크와 교합면 방향의 후크 모두 전치부의 압하가 일어나며, 그 양은 교합면 방향의 후크에서 더 많이 일어난 다는 것을 볼 수 있었다.;The purpose of the study was to compare the difference in three dimensional tooth movement of anterior teeeth using gingival hook and occlusal hook. Orthodontic Mini-Implant(OMI) was inserted on the mesial side of the second premolar, 10mm above from the working wire and the 250g force was applied. The experiments were performing using the Calorific machine which can simulate dynamic tooth movement. Computed Tomography image of the experimental model were taken before and after tooth movement after in 0.75mm thickness and reconstructed into a three dimensional model using V-Works 4.0™. These reconstructed images were superimposed using Rapidform 2004™ and the direction and amount of tooth movement were measured. The findings were as follows: 1. The amount of intrusion was larger in the occlusal hook group than the gingival hook group(p < 0.05). 2. The amount of retraction was larger in the gingival hook group than the occlusal hook group(p < 0.05).
