dc.description.abstractThis study was conducted in order to survey injuries caused by injection needles and the patterns of needle stick injuries in nurses who are at a high risk of blood borne infectious diseases, to identify risk factors related to needle stick injuries, and to provide materials for educating hospital employees on infection and developing prevention programs. The subjects of this study were 100 cases reported from January 1, 2004 to December 31, 2008 by the nurses of a university hospital who had a needle stick injury and consulted with the nurse of the Infection Control Office or were treated by a medical specialist at the Infectious Disease Department according to the control guidelines of the Infection Control Office. The questionnaire used in the survey consisted of explanation and report of exposure to infection on duty, asking questions about the nurse and the patient involved in the accident, information on the accident, treatment, etc. Collected data were processed with SPSS WIN 13.0 pregram, real numbers and percentages were obtained and difference in frequency was analyzed through Chi square test. The results of this study were as follows. 1. As to the subjects' general characteristics, all the subjects were female, their average age was 24.9, and their average length of service was 23.7 months. 2. As to time related patterns, the years when needle stick injuries occurred most frequently were 2007 and 2008 (27.0%), and the month was April (17.0%). As to job related patterns, the departments where needle stick injuries had most frequently were wards related to internal medicine (25.0%), and the shift in which needle stick injuries had most frequently under the three shift system was the day shift (48.8%). As to the situation when needle stick injuries had, 28.0% of the injuries had after the use of an injection needle and before its disposal. In connection to patient treatment, 63% of the injuries had after treatment. Among the injuries, 71.0% were caused by a disposable syringe, and 52.1% had in the right hand and, more specifically, 45.8% had in the right hand fingers. As to the first treatment after the occurrence of an injury, 76.0% disinfected the site injured by a needle with disinfectant, and 71.0% of the nurses reported the injury on the same day. Among the cases, 69.2% performed all the required processes including reporting the Infection Control Room of the injury and treatment and follow up examinations at the Infectious Disease Department. Among the involved patients, 38.0% had hepatitis B, and among the involved nurses, 69.0% carried anti HBV. 3. In the results of Chi square test, the frequency of needle stick injuries was statistically significantly different according to age (x²=5.285, p=.022). In addition, the frequency of needle stick injuries at each department was statistically significantly different according to year (x²=10.741, p=.016), shift (x²=15.334, p=.018), work on the occurrence of the injury (x²=526.722, p=.000), and injection needle type (x²=81.795, p=.000). In conclusion, needle stick injuries had more frequently among nurses with short experience, at wards where direct contacts with patients and invasive treatments were frequent, and during the day shift when injections were administered frequently. In addition, they had frequently after the use of an injection needle and before its disposal, and hepatitis C was the most common causal disease. Thus, it is required to develop education and prevention programs so that if one has a needle stick injury he/she makes an immediate report to the Infection Control Room and gets appropriate treatment for preventing infection with blood borne diseases.;지금까지 주사바늘 등에 의한 상해와 관련된 연구에서 연구 대상은 병원직원들이 보고한 자료, 병원직원, 수술실 의료인, 전공의, 임상병리사를 대상으로 하는 연구 등이 있었으나 환자와 직접 접촉하고 혈액을 취급하는 경우가 많은 간호사를 대상으로 주사바늘에 의한 상해를 보고한 자료를 분석한 연구는 미비한 실정이었다. 이에 간호사를 대상으로 주사바늘에 의한 상해에 대한 역학적 특성을 파악하고 이러한 사고에 대한 예방지침을 마련하고자 본 연구를 수행하였다. 본 연구의 목적은 혈액매개성 감염질환의 위험이 높은 간호사들의 주사바늘에 의한 상해 실태와 노출 양상을 조사하여 병원직원들의 감염에 대한 교육의 자료를 제시하고 예방 프로그램 개발의 기초자료로 활용하기 위한 후향적 조사연구이다. 연구대상은 일 대학병원에 근무하는 간호사들 중 2004년 1월 1일부터 2008년 12월 31일까지 주사바늘 등에 의해 외상을 받아 감염관리실의 관리지침에 따라 감염관리실 간호사의 상담을 받거나 감염내과 전문의의 진료를 받은 직원들이 보고한 자료 100건을 연구 대상으로 하였다. 본 연구에 사용된 도구는 간호사에 대한 정보, 환자와 관련된 정보, 사고와 관련된 정보, 처치 등의 문항으로 구성된 직무 중 감염노출 경위서 및 보고서를 이용하여 조사하였다. 수집된 자료를 분석하기 위해 SPSS WIN 13.0 program을 이용하여 실수와 백분율을 구하였으며, 빈도에 따른 차이 검증은 Chi-square test로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 대상자의 일반적 특성은 성별이 여자가 100%이고, 평균연령은 24.9세, 평균 근무경력은 약 23.7개월이었다. 2. 주사바늘에 의한 상해의 근무시기별 상해 양상은 2007년과 2008년에 27.0%로 가장 많았고, 4월이 17.0%로 가장 많았다. 근무특성과 관련하여 근무부서별 상해 양상은 내과계 병동이 25.0%로 가장 많이 차지하였고, 근무시간별 상해 양상을 살펴보면 3교대 근무를 한 경우, 낮 시간 근무 조가 48.8%로 많이 차지하였다. 상해 발생 관련 업무는 사용 후 폐기 전이 28.0%로 가장 많았고, 처치 전후 상황에서는 처치 후에 발생하는 경우가 63.0%로 많았다. 주사바늘 종류는 일회용 주사기로 인한 사고가 71.0%로 가장 많았고, 상해 부위는 오른손이 52.1%로 가장 많았고, 오른손에서도 손가락이 45.8%로 가장 많았다. 상해 후 초기에 시행한 처치는 주사바늘로 찔린 부위에 소독제를 사용하여 소독을 한 경우가 76.0%로 가장 많았고, 상해를 당한 후 당일 보고한 경우가 71.0%로 가장 많았다. 상해 후 감염관리실에 보고한 다음 감염내과 진료와 추후 검사를 모두 시행한 경우는 69.2%이었다. 간호사들의 주사바늘 상해에 의한 혈액매개 감염질환 노출은 환자의 감염질환이 확인 된 경우 70.0%로 나타났고 환자의 원인 질환은 C형 간염이 38.0%로 가장 많았고, 직원의 B형 간염 항체 보유 상태는 69.0%에서 양성이었다. 3. 일반적 특성에 따른 주사바늘에 의한 상해 발생 빈도의 차이를 검증 한 결과, 근무경력(x²= 9.000, p=.003), 근무부서(x²= 13.520, p=.004), 근무시간(x²= 13.071, p=.001), 상해 발생 관련 업무(x²= 47.540, p=.000), 주사바늘 종류(x²= 64.580, p=.000) 등에 따라 유의한 차이가 있는 것으로 나타났다. 결론적으로 주사바늘에 의한 상해는 경력이 적고, 환자의 침습적인 처치가 많이 이루어지는 병동에서 많았고, 주사 처치가 많은 낮 근무 때에 사고가 많은 것으로 나타났다. 또한 처치 후 주사바늘을 폐기하기 전, 오른손에 사고가 많은 것을 알 수 있었다. 또한 환자의 원인 질환은 C형 간염이 많았다. 이에 주사바늘에 의한 상해를 경험한 후에 감염관리실에 즉시 보고를 하여 적절한 처치를 받고 혈액매개 질환의 감염을 예방할 수 있도록 보고체계와 감염 예방을 위한 교육 프로그램 개발이 요구된다.-
