dc.description.abstract본 연구는 반복 자살 시도자의 생존 경험의 의미는 무엇이며 어떠한 과정으로 진행되는가를 이해함으로써 생존 과정에 관한 실체이론을 개발하는 것을 목적으로 실시되었다. 본 연구의 참여자들은 서울 및 경기지역에 거주하는 23세~46세의 성인 남녀 7명(남성3명, 여성 4명)으로 연구 참여자 선정은 인터넷 포탈 사이트의 우울증 까페와 정신보건센터, 사회복지관, 가정폭력 상담소 및 정신의료기관에서 반복 자살 시도의 경험이 있고 현재 일상생활에 특별한 문제가 없는 이를 추천받아, The Minnesota Multiphasic Personality Inventory (MMPI)와 Beck의 자살사고 척도인 Scale for Suicidal Ideation(SSI)를 적용하여 인격장애가 아니면서 현재 자살사고가 없는 자로 본 연구에 대하여 충분한 설명을 듣고 자발적 의사에 의해 연구 참여 동의서에 서면 동의한 자를 최종 선정하였다. 자료 수집은 심층면담을 통해 이루어졌으며 면담 내용은 녹취를 한 후 필사하였다. 1인당 면담 횟수는 2~3회 이루어졌고, 1회 면담시의 시간은 60분에서 120분 정도 소요되었다. 자료의 분석은 Strauss와 Corbin(1998)의 근거이론에 의한 개방코딩(open coding), 축 코딩(axial coding), 선택 코딩(selective coding) 및 과정분석에 의하여 80개의 개념과 24개의 하위범주, 15개의 범주들이 도출되었다. 각 단계에서 나온 결과는 질적 연구 경험이 있는 정신간호학 교수 1인과 자살 상담 및 위기 중재 3년 이상의 경험이 있는 정신보건간호사와 정신보건사회복지사 2인, 자살 연구 경험이 있고 실무 경력이 5년 이상인 임상심리전문가 1인의 자문을 받아 수정하였으며 연구 참여자 2인의 합의를 받았다. 이어서 유형 및 과정 분석이 이루어졌으며 상황모형이 형성되었다. 이에 따른 본 연구의 결과는 다음과 같다. 1. 반복 자살 시도자의 생존 경험의 인과적 조건은 ‘혼자됨’, ‘이해받지 못함’, ‘포기하고 체념함’이었고, 이로 인해 나타나는 현상은 ‘수렁으로 빨려들어감’이었다. 이러한 현상의 맥락적 조건에는 ‘가족의 지지’가 있었고, 중재적 조건에는 ‘과거를 묻어버림’이 있었다. 반복 자살 시도자가 생존 과정에서 ‘수렁으로 빨려들어감’을 해결하기 위해 사용하는 작용/상호작용 전략은 ‘스스로 방어함’, ‘세상과 소통함’, ‘종교에 몰두함’, ‘자신을 추스림’, ‘일상에 충실함’, ‘지지체계를 만듬’등인 것으로 밝혀졌다. 그리고 이들 작용/상호작용 전략의 결과로는 ‘자유로워짐’, ‘희망을 품음’, ‘자아가 성장함’이 도출되었다. 2. 반복 자살 시도자의 생존 경험에서 현상인 ‘수렁으로 빨려들어감’을 해결해 나가는 과정이면서도 다른 모든 범주들을 아우르는 핵심범주로는 ‘늪에서 헤어나옴’이 도출되었다. 3. 반복 자살 시도자의 생존 경험의 유형은 자신이 처한 상황에 대하여 적극적으로 대처하는 ‘등불형’, 생존에 대한 의지는 있으나 상황에 따라 문제해결 방식에 차이를 보이는 ‘노력형’, 그리고 혼자만의 문제로 견디고 참아내는 ‘안개형’의 세 유형으로 분류되었다. 4. 반복 자살 시도자의 생존 경험에서 핵심범주인 ‘늪에서 헤어나옴’의 과정은 ‘허우적거림’, ‘버티어 냄’, ‘나아감’을 통해 진행되는 것으로 밝혀졌다. 이들 과정은 연속적이고 반복적인 패턴을 나타내면서 반복 자살 시도자들이 효율적으로 ‘늪에서 헤어나옴’을 할 수 있도록 도와준다. 5. 반복 자살 시도자의 생존 경험은 개인-가족-환경적 차원에서 이루어진 작용/상호작용의 결과 경험하게 되는 현상으로서 자살의 효과적인 예방과 개입을 위해 지역사회와 국가 차원에서 가정폭력, 성폭력, 가부장적 사회인식, 성공지상주의, 불황 등의 경제적 요인들에 대한 보다 구체적이고 실효성 있는 정책의 재조정과 새로운 제도의 도입이 요구된다. 반복 자살 시도자의 생존 경험은 ‘늪에서 헤어나옴’의 과정으로 보다 성장하고 성숙해가는 삶의 과정이었다. 자살 문제에 있어서 고위험군인 반복 자살 시도자를 돕기 위해서는 우선적으로 이들이 처해 있는 삶에 대한 깊이 있는 이해와 공감이 필수적이다. 특히, 반복 자살 시도의 경험이 있는 대상자들은 그들 스스로가 다른 사람들에게 이해받지 못한다고 느끼는 부분이 강하고 폐쇄적이라 간호사는 전문적이고 숙련된 기술과 능력을 갖추어야만 이들의 특수한 요구에 적합한 중재기법들을 적용함으로써 양질의 전인간호를 제공할 수 있다. 본 연구는 간호실무측면에서 자살의 고위험군인 반복 자살 시도자의 자살에 대한 이해를 증진시킴으로써 간호사들에게 자살 예방 및 위기개입시 자살 문제를 가진 대상자들에게 총체적인 접근을 통한 대상자 중심의 개별화된 맞춤형 관리의 중요성을 재인식할 수 있다. 한편 ‘늪에서 헤어나옴’의 실체이론은 간호교육, 연구 및 정책 수립에 대한 새로운 통찰력을 제시해 주고 앞으로 반복 자살 시도자의 생존 경험에 관한 이론 구축 및 간호 실무 수행에 도움이 되는 자료로 활용될 것으로 생각한다. 더 나아가 실제로 자살 문제로 고통 받고 있는 대상자와 그 가족들에게 자살 문제 극복에 대한 희망을 갖도록 적극적으로 돕는 기회가 되기를 기대한다.;The purpose of this study is to understand the significance of survival experiences of those who repetitively attempted suicide and the progress of the experiences, and to develop a substantive theory on the survival process. The subjects of this study were seven adult four males and three females between 23 and 46 years old who lived in Seoul and Gyeonggi-do. Those who experienced repetitive attempting of suicide and currently had no specific problems in daily life were recommended from depression cafes in Internet portal sites, Community Mental Health Centers, Social Welfare Center, Family Violence Counseling Centers, and Mental Health Hospitals and were applied of the Minnesota Multiphasic Personality Inventory (MMPI) and Scale for Suicidal Ideation (SSI) as a suicidal scale presented by Beck. Then, those who had no personality disorder and suicide ideation, took sufficient explanation on this study, and voluntarily submitted written consent to participate in this study were selected to be the subjects of this study. The data was collected through in-depth interviews, and the contents of the interviews were recorded to for transcript. Each subject was interviewed two or three times, and each interview took 60 to 120 minutes. The data was analyzed by open coding, axial coding, selective coding, and process analysis based on the grounded theory presented by Strauss and Corbin (1998), and 80 concepts, 24 sub-categories, and 15 categories were induced. The results from each stage were adjusted by consideration of consultations from one professor of mental health nursing having experiences in qualitative research, two psychiatric mental health nurse and psychiatric mental health social worker having three or more years of experiences in suicide counseling and crisis intervention, and one expert of clinical psychologist having five or more years of experiences in suicide research and practice, and were agreed by two participants of this study. Then, patterns and progresses were analyzed to form a situational model. The results of this study are as follows. First, the causal conditions of survival experiences of those who repetitively attempted suicide were "being alone," "taking misunderstood," and "giving up and resigning," and the consequent phenomenon was "support of his family." The contextual condition of the phenomenon was "being sucked," and the intervening condition was "forgetting the past." The action/interaction strategies used by the adults who repetitively attempted suicide to solve "forgetting the past" in overcoming suicide were "giving and taking help," "indulging in religions," "self-hypnosis," and "being faithful to daily life," and "making a support system." The consequent results of the strategies were "becoming free", "recovering hope" and "humane growth." Second, the core category, containing all the other categories, as a process to solve "being sucked," the phenomenon in survival experiences of those who repetitively attempted suicide was "getting out of the swamp." Third, the types of survival experiences of those who repetitively attempted suicide were divided into three including "type of lamp" that one actively copes with his/her situation, "type of effort" that one has a will to overcome suicide but shows difference in solving problems according to situations, and "type of fog" that one endures and bears his/her situation as their own problems. Fourth, the "getting out of the swamp" as the core category of suivival experiences of those who repetitively attempted suicide was processed through "floundering", "enduring," and "progressing." The processes showed continual and repetitive patterns, helping the adult who repetitively attempted suicide to effectively conduct the "getting out of the swamp." Fifth, survival experiences of those who repetitively attempted suicide is a phenonenon as the result of action/interaction at the level of individual-family- community-state, and it is needed to readjust the existing policies and to introduce new institutions from the aspects of community and state in order to prevent and intervene suicide effectively. Survival Experiences of those who repetitively attempted suicide was a process of life in which one can grow and become an adult via the "getting out of the swamp." In order to help those who repetitively attempted suicide as the high risk group in suicide problem, in-depth understanding and empathy about their life are preferredly essential. In particular, those who repetitively attempted suicide strongly think they are not understood by others and do not open up to people, and thus nurses should have professional and expert skills and abilities in order to apply intervention techniques appropriate to their specific demands and to present superior holistic nursing.-
