하나님 표상의 재구성을 통한 경계선 장애 내담자의 치료

Abstract

본 논문은 어린 시절 만성적인 좌절 경험과 충분한 돌봄의 부족으로 인하여 대상관계의 병리적인 발달을 이루게 되고, 이러한 경험으로 인하여 부정적인 대상 표상 및 하나님 표상이 형성되어 어려움을 겪는 경계선 장애 내담자에 대한 목회상담적 돌봄을 고찰하는 것을 목표로 하였다. 본 논문은 경계선 장애를 병리적인 심리적 측면에서 뿐만 아니라 하나님 표상 형성과 연결하여 고찰해 봄으로써, 하나님 표상 문제를 가진 경계선 내담자들을 위해 목회 상담적 측면에서 도움을 줄 수 있는 방법을 연구해 보았다. 경계선 장애는 대인관계, 자아상 및 정동에서의 불안정성, 그리고 심한 충동성이 지속적으로 성격화되어 나타나는 이질적인(heterogeneous)증상 조합이 특징인 장애이다. 또한 정서, 인지, 대인관계에서 뚜렷한 문제를 보이고, 자아상이 불안정하며, 충동성이 두드러지는 것이 특징이다. 경계선 장애의 원인은 양육 초기 좋은 자기 표상 및 대상 표상을 나쁜 자기 표상 및 대상 표상과 통합하지 못하고, ‘대상 항상성’을 성취하지 못하게 되어, 결국 부정적 함입의 우세라는 특징을 갖게 된 것에 기인한다. 그 결과 자기 정체감 형성, 감정 조절, 공감 능력의 발달, 초자아의 발달 등에서 장애를 초래하고 심한 방어적 분열과 투사적 동일시, 외적 대상에 대한 지나친 의존 등으로 인해 인간관계 능력을 손상시킨다. 이러한 부정적 대상 표상은 대인관계에 적용되고, 더 나아가 하나님 표상을 형성하는 데에도 영향을 미친다. 여태까지 경험한 부정적인 대상 표상이 그대로 적용 및 투사되어 부정적인 하나님 표상을 형성할 수도 있고, 반대로 부정적인 대상들과 같지 않기를 바라는 소망이 담긴 긍정적인 하나님 표상을 형성 할 수 도 있다. 본 논문에서는 부정적인 하나님 표상 문제를 가진 경계선 장애 내담자를 돕기 위하여 경계선 장애에 대해 자세히 알아보고, 또한 이들의 하나님 표상이 어떻게 형성되었는지 분석하며, 하나님 표상의 재구성을 위한 목회상담적 돌봄을 제공함으로써 내담자의 대상 표상이 변화되고, 더 나아가 하나님 표상이 변화될 수 있도록 돕는데 의의가 있다고 하겠다. 본 논문은 경계선 장애에 대한 이해를 높이기 위해서 오랜 임상경험을 바탕으로 한 오토 컨버그(Otto F. Kernberg)의 이론을 살펴보았다. 또한 초기 양육자와의 개인적인 경험의 결과로서 하나님 표상을 형성하게 된다는 사실을 밝혀낸 애너 마리아 리주토 (Ana-Maria Rizzuto)의 연구를 통해 경계선 장애 내담자의 초기 대상관계와 부정적 하나님 표상 형성간의 연결점을 찾았다. 컨버그는 경계선 성격 내담자들에게서 볼 수 있는 특수한 방어기제와 병적으로 내재화된 대상관계를 일차적 장애로 보고, ‘경계선 성격구조’ (borderline personality organization)라는 개념을 설정하였다. 또한 이를 신경증과 정신증 사이를 오가는 증상이 아닌 나름대로의 안정된 성격구조를 지닌 것으로 파악하였다. 그는 증상 기술만으로는 경계선 장애의 최종 진단을 명확히 하는데 충분하지 않음을 시사하며 체계적인 구조적 진단기준을 제시하였다. 또한 기술적 진단과 더불어 구조적 진단에 입각하여 엄격한 분석을 통해 경계선 장애를 진단해야 한다고 주장했다. 컨버그는 여러해 동안 임상경험을 통하여 연구한 결과 경계선 장애 치료에 있어서 직면, 명료화, 해석을 주요 도구로 사용하는 표현적 심리치료가 효과적이라고 주장하였다. 표현적 심리치료를 통해 경계선 내담자는 충동조절능력을 발달시키고, 불안을 참고, 감정을 조절하며, 본능적 욕구를 정화하고, 약한 자아를 강화시킴으로써 뚜렷한 치료의 진전을 가져 올 수 있다고 보았다. 그는 경계선 장애 치료의 임상 실제와 이론적 개념화를 적절하게 연결하여 경계선 장애에 대한 이해를 포괄적으로 조직화하였다. 애너 마리아 리주토는 임상적 연구를 통해 인간이 어린 시절 부모와의 개인적인 경험의 결과로 하나님 표상을 형성하게 되며, 부모와의 관계와 그의 자기지각을 하나님 표상에 채색하게 된다는 사실을 밝혀냈다. 또한 ‘종교는 원망에 기초한 신경증이다.’ 라는 프로이트의 견해를 따르는 상담자들에게 내담자의 정신적 균형을 유지하기 위해서는 특별히 ‘하나님 표상’을 다뤄야 한다는 사실을 새롭게 인식해야 한다고 주장하였다. 리주토는 상담자가 신앙의 중요성을 더 깊이 인식하고, 이러한 통찰이 고려됨으로써 내담자를 위한 더 깊이 있는 상담 및 치유가 이루어질 수 있다고 강조하였다. 또한 내담자의 하나님 표상의 형성과정을 분석하고, 하나님 표상을 재구성하는 과정은 내담자의 치료에 도움을 줄 수 있다고 주장 하였다. 목회상담의 중요한 임무는 내담자의 심리적 어려움을 돕는 것과 더불어 내담자가 하나님과의 관계를 바르게 정립하도록 도와주는 것이다. 목회상담은 공감, 지지, 격려를 통해 내담자를 돕는 지지적인 치료의 특성에 가깝다. 그러나 컨버그는 경계선 내담자의 상담에 있어서 지지적인 상담기법은 내담자의 무의식적 욕구를 해석하기 보다는 지나친 병리적 의존 욕구를 만족시키기 때문에 경계선 장애 치료에 적합하지 않다고 지적하였다. 하지만 직면, 명료화, 해석을 통한 상담만으로는 한계가 있다. 왜냐하면 내담자가 충분한 심리적 자원을 갖추고 있지 않은 상태에서 해석과 직면을 이용하는 상담을 진행하게 되면, 치료 자체가 제대로 진행되지 않거나, 내담자의 문제가 더 악화되는 일이 발생하기 때문이다. 그러므로 본 논문에서는 상담 초기에는 ‘안아주기’와 ‘충분한 어머니’의 지지적 상담을 통해 내담자가 자신을 변화시킬 수 있는 심리적 역량을 갖추도록 한 후, 내담자의 불안을 담아주면서 직면과 해석, 그리고 분석을 통하여 내담자를 도울 수 있는 목회상담적 방법을 제시하였다. 이러한 목회상담적 돌봄이 경계선 장애 내담자로 하여금 자아 정체성을 확립하게 하고, 대상에 대한 신뢰감을 형성하여 긍정적 대인관계를 맺는데 도움을 줄 수 있다. 뿐만 아니라 하나님 표상의 재구성 과정을 통해 하나님 표상이 적절히 변화되도록 도와줌으로써 내담자의 성장을 이끌어 낼 수 있다고 기대해 본다.;This thesis aims at making the pathological development of object relationship due to the lack of childhood chronic frustrating experience and of adequate care, with which is formed the negative object representations and God representation to inquire into the pastoral counseling care on the borderline personality disorder client in difficulty. This thesis inquires into the borderline personality disorder not only connected to the pathological and psychological side but also to God representation, and studies the way to help in the pastoral counseling side for the borderline personality disorder client with God representation. The borderline personality disorder is that is characterized by the heterogeneous symptom appeared continuously with interpersonal relations, instability of self-image and affect, and severe impulsivity. And, it is shown the defined problems in the emotion, recognition, interpersonal relations, characterized by the instable self-image and severe impulsivity. The cause of the borderline personality disorder contributes finally to the superior characteristics of negative depression when can not integrate the good self-image and object representations with the bad self-image and object representations, and with ‘object constancy’ not obtained. As a result, it causes the disorder in self-identity formation, emotional control, empathy skill development, superego development, and damages the human relation skill due to critical defensive splitting, projective identification, over-reliance on external target. Such a negative object representation is applied to the human relations, and furthermore affected even to form God representation. The negative object representation already experienced can be applied and projected as it is for the negative formation of God representation, or in the other side for the positive formation of God representation desiring it not to be the same as the negative objects. In this thesis, I will inquire into the borderline personality disorder to help client with the negative problem of God representation, and analyzing how was their God representation formed, and offering the pastoral counseling care to reform the God representation in order to help the client change the object representation, and furthermore the God representation. This thesis observed the theory of Otto F. Kernberg based on the long-time clinical experiences to understand better the borderline personality disorder. And, as the result of personal experience with the initial nurturer I found the connection point between the initial objective relation of the borderline personality disorder and the negative formation of God representation through the study of Ana-Maria Rizzuto that revealed the fact to form the God representation. Kernberg saw the special defense mechanism for the borderline personality disorder clients and the object relations pathologically internalized as the primary dysfunction, and established the concept of ‘borderline personality organization’. And, he grasped this not as the symptom recurring between the neurosis and psychosis but what has the stable personality organization. He suggested that it is not enough to make sure of final diagnosis for the borderline personality disorder only with the description of symptom, presenting the systematic structured diagnostic criteria. And, he insisted that the borderline personality disorder should be diagnosed through strict analysis based on the structured diagnosis with descriptive diagnosis. Kernberg insisted that it is the efficient expressive psychotherapy that uses as main tool the confrontation, clarification, interpretation for the borderline personality disorder treatment as the result studied through the several times of clinical experiences. He saw with the expressive psychotherapy that the borderline personality disorder client can be advanced of significant treatment by developing the ability to control impulse, bearing anxiety, controlling the emotion, purifying the instinctual needs, and strengthening the weak ego. He connected the clinical practice of the borderline personality disorder with the theoretical conceptualization, and systematized generally the understanding of borderline personality disorder. Ana-Maria Rizzuto revealed that the human being forms the God representation, as the result of personal experience with the patients in his youth, through the clinical research, and colors the God representation of his relation with patients and his self-perception. And, she insisted that the counselors that follow the Freud’s view, ‘the religion is the neurosis based on the viewing afar.’ should recognize newly the fact that ‘the God representation’ should be specially treated to maintain the psychological balance of client. Rizzuto stressed that the counselor should recognize deeper the importance of faith, and with this insight considered to make the deeper consultation and cure for the client. And, he insisted that the process to analyze the formation process of client’s God representation, and to reorganize the God representation would be helpful for the client’s treatment. The important mission of pastoral counseling is to help the client release the psychological difficulty, and establish correctly the relation with God for the client. The pastoral counseling is almost characterized to the supportive treatment for the client through the sympathy, support and encouragement. However, Kernberg indicated that it is not adequate for the borderline personality disorder treatment as the supportive counseling technique for borderline client does not interpret the unconscious desire of the client but satisfy the pathological dependence needs. However, there is the limit only with the consultation through the confrontation, clarification, interpretation. It is because the counseling made with interpretation and confrontation in client’s state without psychological resource can not make treatment not well progressed, or the client’s problem worst. So, in this thesis I presented the pastoral counseling way to help the client through the confrontation, interpretation and analysis by releasing the client’s anxiety after making the client have psychological ability to change himself through the ‘holding’ and supportive counseling of ‘good-enough mother’ in the beginning of counseling. Such a pastoral counseling care can help the client establish the self-identity by the borderline personality disorder, and make the positive human relations by forming reliability for the object. And, it is expected to bring out the client’s growth by helping the God representation well-changed through the reorganization process of God representation.
