노인장기요양보험제도하에서 방문간호사의 방문간호서비스와 직무만족도

Abstract

본 연구는 노인장기요양보험제도하에서 방문간호사의 방문간호서비스와 직무만족도를 파악하기 위해 8개 시도에서 근무하는 방문간호사를 대상으로 설문조사자료를 분석한 서술적 조사연구이다. 자료는 2009년 2월 18일부터 3월 28일까지 총 33개 기관에서 103명의 방문간호사(회수율 65%)로 부터 회수하였다. 연구도구는 Stamps, Slavitt, Piedmont와 Haase(1978)가 개발한 직무만족 측정도구(index of work satisfaction)로 이를 한국어로 번역하여 사용하였다. 수집된 자료는 SPSS 13.0을 활용하여 χ2-test, t-test, ANOVA, Pearson Correlation Coefficient로 서술통계를 분석했으며 신뢰도는 Cronbach's α로 분석했다. 본 연구의 연구결과를 요약하면 다음과 같다 1. 방문간호사의 평균 연령은 48세이며 평균 방문간호 경력은 17년으로 나타났다. 보수수준은 150만원미만이 가장 많았다. 방문간호사의 70.9%가 가정(전문)간호사 자격증을 취득했다. 2. 방문간호기관에서 가정간호서비스가 함께 제공되는 경우는 68%, 방문간호기관에서 가정간호서비스가 함께 제공 안하는 경우는 32%이었다. 방문간호사가 관리하고 있는 대상자의 질환분포는 고혈압, 편마비, 당뇨, 욕창, 치매 순으로 나타났다. 3. 방문간호사가 제공하고 있는 방문간호서비스 중 상위 30%에 해당하는 방문간호서비스는 활력증후, 간호사정 및 진단 등의 기본간호가 대부분을 차지하였고 욕창드레싱, 정체됴뇨관 삽입 등의 간호처치 순으로 나타났다반면 가정(전문)간호사 자격증이 있는 경우에는 기관 절개관 교환, 봉합선 제거, 염증성 처치 등의 간호처치와 교육훈련 검사관련 업무, 의뢰 등에 많은 간호서비스를 제공하는 것으로 나타났다. 4. 방문간호사의 직무만족도의 전체 평균은 5점 척도에서 3.09로 나타났고, 구성요소간 직무만족도는 전문직 상태 3.84, 상호작용 3.72, 자율성 3.63, 임금 2.85, 행정 2.82, 간호사-의사 관계 2.77 순이었다. 5. 방문간호사의 일반적 특성과 직무만족도의 차이는 연령, 학력, 경력, 보수수준, 고용형태, 기관유형 모두 유의한 차이가 없는 것으로 나타났다. 결론적으로 노인장기요양보험제도 방문간호는 가정에서 제공되는 간호서비스라는 점에서 보건소 방문간호와 의료기관 방문간호와 유사하나 방문간호서비스의 발전적인 방향의 틀에서 상호 연계 및 의뢰 체계가 구축되어야 할 것이다 또한 만족도에서는 전문직 상태는 높고 간호사-의사 관계 낮아서 간호사-의사의 관계 개선 대안이 마련되어야 할 것이다. ;This study is a descriptive study on visiting nurses' services and their job satisfaction by analyzing the factors that impact their role accomplishment and job satisfaction under the Long-Term Care Insurance System for the Elderly. The data was collected from January 18∼March 28, 2009. Study subjects were 103 visiting nurses working in 33 institutions in 8 cities. The Index of Job satisfaction consists of 7 sectors which were established by modifying and supplementing the tool of Slavitt et. al.(1978). SPSS WIN 13.0 program was used for the analysis of the collected data in actual number and percentage, average and standard deviation, t-test, ANOVA, Pearson Correlation Coefficient and comparative analysis to analyze the reliability of Cronbach's α. etc. Visiting nurses were found to manage a range of diseases such as hypertension, diabetes, bedsore and dementia. Visiting nursing were reported to mostly take care of fundamental nursing. Visiting nurses with home health care specialist certification were reported to perform much more competently nursing treatment service and medication management than general visiting nurses such as sore care, catheterization and wound care etc. In regard of job satisfaction level overall average was 3.09 out of 5. The relation between visiting nurses' general characteristics and their job satisfaction showed no significant difference. In conclusion, there are three types of home healthcare in Korea which are based in the public health center, hospital and long-term care insurance. However, it is necessary to classify their role and function because of similarity on the service receiver and place which is provided, even they are different to the legal basis financial resources and service provides. This study may be used as a source for data to set up future plans to improve job satisfaction of visiting nurses.
